Clinical Utility of Patient-Reported Outcome Measurement Information System Domain Scales Thresholds for Determining Important Change After Stroke

BACKGROUND: Patient-reported outcome measures are increasingly being utilized in clinical care and research to evaluate outcomes following stroke. To optimize the clinical utility of these measures, we aimed to quantify meaningful change by establishing minimal important differences (MIDs), or responder definitions, for 4 domains affected in ischemic and hemorrhagic stroke patients. METHODS AND RESULTS: We performed a retrospective cohort study of stroke patients seen in the Cleveland Clinic cerebrovascular center between September 2, 2012 and November 7, 2017. Four Patient-Reported Outcome Measurement Information System (PROMIS) scales were completed within 1 month poststroke and again at 6 months. MIDs were estimated using an anchor-based approach based on a global impression of change question and supported using 3 distribution-based methods. Cumulative distribution functions assessed responder thresholds. MIDs were additionally derived across sex, race, and varying levels of severity as defined by the modified Rankin Score and baseline PROMIS score. During the study period, 337 incident stroke patients completed at least 1 PROMIS domain scale at both time points (average age 61 +/- 14, 56% female). Estimates from the 4 methods were triangulated to provide a MID range across PROMIS domain: 2.5 to 6.5 T-score points for physical function and fatigue, 2.5 to 7.5 for social role satisfaction, and 3.0 to 8.0 for anxiety. Cumulative distribution functions plots identified between 30% and 40% of patients as having meaningful improvement based on the anchor-based estimates across all 4 domains. Meaningful change thresholds remained consistent across categories of sex and race. Anchor-based MIDs increased with increasing severity, whereas distribution-based MIDs remained consistent across severity levels. CONCLUSIONS: Our study is the first to evaluate interpretability of changes in PROMIS scores for stroke survivors. Future studies can utilize these thresholds to identify responders of stroke interventions. Based on our estimated MID ranges, researchers and clinicians can choose a responder threshold for comparing change in domain score at the group level, individual level, or by severity.

Mapping PROMIS physical function and pain interference to the modified low back pain disability questionnaire

Summary: Linear regression, linear, and equipercentile equating methods were used to map between MDQ and PROMIS-PF and PROMIS-PI scales. Results showed that using both PROMIS-PF and PROMIS-PI together had the closest estimated means, lowest RMSE and MAE, and highest correlations for estimating MDQ total scores. Equipercentile equating using the MDQ items performed best for estimating each of PROMIS-PF and PROMIS-PI T-scores.

PROMIS global health: potential utility as a screener to trigger construct-specific patient-reported outcome measures in clinical care

Summary: This study evaluated the ability of utilizing items on a global health measure to identify patients who may require additional screening. The results showed that these items can be used as screening tools to identify patients with poor scores on additional construct-specific outcomes. By directing construct-specific assessments to applicable patients, survey burden could be reduced, leading to a more efficient and targeted use of PROMs in healthcare decision-making.

A retrospective cohort study was conducted, including all stroke patients who underwent inpatient cEEG monitoring. The results showed that ASM use was associated with ASyS diagnosis, but there were no significant differences in PROMs based on ASyS status, ASM use at hospital discharge, or ASM status on the day of PROM completion.

Emergency department utilization among patients who receive outpatient specialty care for headache: A retrospective cohort study analysis

Summary: This study aimed to compare the clinical characteristics between outpatient headache clinic patients who self-report visiting the emergency department for headache and those who do not. The results showed that there were significant differences in age, race, insurance status, and area deprivation index between patients who reported emergency department utilization and those who did not. Additionally, worse patient-reported outcome measures were associated with a higher risk of emergency department utilization.

Comparison of stratification techniques for optimal management of patients with chronic low back pain in spine clinics

Association of women-specific health factors in the severity of Parkinson's disease

Summary: Parkinson's disease (PD) is an age-related neurological disorder that shows differences in risk, progression, and severity between men and women. This study aimed to identify women-specific health experiences associated with PD severity by developing and distributing a women-specific questionnaire and creating multivariable models. The results found significant associations between major depressive disorder, perinatal depression, natural childbirth, LRRK2 genotype, B12 deficiency, total hysterectomy, and increased PD severity. This study provides important insights into the etiology of PD and highlights the role of sex-specific experiences.

Summary: This study aims to explore methodological guidelines for proxy reporting in adult populations and develop relevant considerations. Through a review of literature and clinical assessment measures, it was found that there is inconsistency in the definition of proxy reporting in adult populations and a lack of considerations for proxy selection and data collection, analysis, and reporting. Therefore, a checklist of considerations for proxy reporting is provided to assist researchers and clinicians in using proxy reporting in research and clinical care.

Utilities Estimated from PROMIS Scales for Cost-Effectiveness Analyses in Stroke

Summary: This study aimed to evaluate and compare estimated utilities based on PROPr and PROMIS-GH in adult stroke survivors. The results showed that all three PROMIS-based utilities were associated with measures of stroke disability and severity, but the distributions of utilities were very different. Therefore, different utility measurement methods need to be considered when assessing health states.
