Accuracy of Fractional Flow Reserve Derived From Coronary Angiography

Background: Measuring fractional flow reserve (FFR) with a pressure wire remains underutilized because of the invasiveness of guide wire placement or the need for a hyperemic stimulus. FFR derived from routine coronary angiography (FFRangio) eliminates both of these requirements and displays FFR values of the entire coronary tree. The FFRangio Accuracy versus Standard FFR (FAST-FFR) study is a prospective, multicenter, international trial with the primary goal of determining the accuracy of FFRangio. Methods: Coronary angiography was performed in a routine fashion in patients with suspected coronary artery disease. FFR was measured in vessels with coronary lesions of varying severity using a coronary pressure wire and hyperemic stimulus. Based on angiograms of the respective arteries acquired in 2 different projections, on-site operators blinded to FFR then calculated FFRangio using proprietary software. Coprimary end points were the sensitivity and specificity of the dichotomously scored FFRangio for predicting pressure wire-derived FFR using a cutoff value of 0.80. The study was powered to meet prespecified performance goals for sensitivity and specificity. Results: Ten centers in the United States, Europe, and Israel enrolled a total of 301 subjects and 319 vessels meeting inclusion/exclusion criteria which were included in the final analysis. The mean FFR was 0.81 and 43% of vessels had an FFR0.80. The per-vessel sensitivity and specificity were 94% (95% CI, 88% to 97%) and 91% (86% to 95%), respectively, both of which exceeded the prespecified performance goals. The diagnostic accuracy of FFRangio was 92% overall and remained high when only considering FFR values between 0.75 to 0.85 (87%). FFRangio values correlated well with FFR measurements (r=0.80, P<0.001) and the Bland-Altman 95% confidence limits were between -0.14 and 0.12. The device success rate for FFRangio was 99%. Conclusions: FFRangio measured from the coronary angiogram alone has a high sensitivity, specificity, and accuracy compared with pressure wire-derived FFR. FFRangio has the promise to substantially increase physiological coronary lesion assessment in the catheterization laboratory, thereby potentially leading to improved patient outcomes. Summary: Noninvasive methods of estimating invasively measured fractional flow reserve (FFRinvasive) are actively being explored, aiming to avoid invasive procedures and drugs. This study evaluates the diagnostic performance of coronary angiography-derived FFR (FFRangio) in NSTEMI patients, demonstrating its sensitivity, specificity, and accuracy. FFRangio can precisely estimate FFRinvasive in NSTEMI patients and guide treatment decisions for coronary artery lesions without invasive procedures or drugs.

Summary: Using low-dose silicon photomultipliers (SiPM) technology with Rb-82 cardiac PET/CT, impaired global stress MBF, global MFR, and regional MFC were found to be powerful predictors of major adverse cardiovascular events (MACE), outperforming traditional risk factors. However, only reduced global stress MBF independently predicted MACE.

Reducing gap in pre-hospital delay between women and men presenting with ST-elevation myocardial infarction

Summary: This study aimed to analyze changes in pre-hospital delay over time in women and men with ST-elevation myocardial infarction (STEMI) in Switzerland. The study found that the pre-hospital delay decreased significantly for both sexes between 2002 and 2019, with a greater decrease observed in women. After adjustment for patient characteristics, the delay in 2019 was nearly the same for women and men.

Clinical outcomes after unprotected left main coronary artery occlusion: A retrospective multicentre cohort analysis

Summary: This study provides data on patients presenting with acute coronary syndrome due to unprotected left main coronary artery (ULMCA) occlusion. The study found that ULMCA occlusion has a high short-term mortality rate, with 15% of patients dying in the cath lab and 42% dying in hospital. Patients who survived the index hospitalization had similar mortality rates as other st elevation myocardial infarction patients.

Summary: Significant coronary artery disease (CAD) is common in patients with severe aortic stenosis undergoing transcatheter aortic valve implantation (TAVI), and the management of these two conditions is crucial, especially in younger and lower-risk patients. However, the preprocedural diagnostic evaluation and treatment indications for significant CAD in TAVI candidates are still debated. In this clinical consensus statement, experts aim to review the available evidence and propose a rationale for diagnostic evaluation and indications for percutaneous revascularisation of CAD in TAVI patients, as well as addressing other concerns related to TAVI.

Summary: We evaluated 20-year trends in modifiable cardiovascular risk factors in young adults with acute coronary syndromes in Switzerland. Young ACS patients in Switzerland exhibited a high prevalence of risk factors, with obesity and smoking being major concerns. These findings highlight the need for public health initiatives targeting young adults in Switzerland.

Summary: The prevalence of carotid artery stenosis (CAS) in patients undergoing transcatheter aortic valve implantation (TAVI) is not well-documented. This study aimed to investigate the impact of CAS, especially severe or bilateral CAS, on the prognosis after TAVI. The results showed that while CAS as a whole did not predict neurovascular complications, bilateral CAS was associated with an increased risk of stroke after TAVI.
