Incidence, predictors, management and outcomes of coronary perforations

Objectives We examined the contemporary incidence, types, predictors, angiographic characteristics, management and outcomes of coronary perforation. Background Coronary perforation is a rare, but important, complication of percutaneous coronary intervention (PCI). There is lack of data on perforations stratified as large and distal vessel perforations. Methods Retrospective, observational cohort study of all patients who underwent PCI at a high volume, tertiary hospital between the years 2009 and 2016. Angiograms of all coronary perforation cases were reviewed to determine the mechanism, type, and management of perforation. Risk-adjusted periprocedural complication rates were compared between patients with and without coronary perforation. One-year mortality outcomes of patients with large vessel vs. distal vessel perforation were also examined. Results Coronary perforation occurred in 68 of 13,339 PCIs (0.51%) performed during the study period: 51 (75%) were large vessel perforations and 17 (25%) distal vessel perforations. Most (67%) large vessel perforations were due to balloon/stent inflation, whereas most (94%) distal vessel perforations were due to guidewire exit. Patients with coronary perforations had significantly higher risk for periprocedural complications (adjusted odds ratio 7.57; 95% CI: 4.22-13.50; P < 0.001). Only one patient with large vessel perforation required emergency cardiac surgery, yet in-hospital mortality was high with both large vessel (7.8%) and distal vessel (11.8%) perforations. Conclusions Coronary perforation is an infrequent, but potentially severe PCI complication. Most coronary perforations are large vessel perforations. Although coronary perforations rarely lead to emergency cardiac surgery, both distal vessel and large vessel perforations are associated with high in-hospital mortality, highlighting the importance of prevention.

Summary: This study analyzed the use of mechanical circulatory support (MCS) in chronic total occlusion (CTO) percutaneous coronary intervention (PCI) and found that MCS was used in 4.5% of CTO PCIs, mostly for prophylactic purposes. Prophylactic MCS patients had a higher risk of complications, while urgent MCS cases had lower success rates and higher complications compared to non-MCS cases.

Summary: A systematic review and meta-analysis of randomized controlled trials showed that very short (<= 3 months) duration of dual antiplatelet therapy (VSDAPT) after percutaneous coronary intervention with drug-eluting stent significantly reduced net adverse clinical events (NACE) and major bleeding without increasing ischemic events. VSDAPT could be a well-tolerated and feasible treatment option in this patient population.

Summary: This study analyzed data from 9296 chronic total occlusion percutaneous coronary intervention cases and found that 15% of the procedures were performed by multiple operators. Despite the increased complexity, cases performed by multiple operators had similar technical and procedural success rates, as well as risk of complications, compared to cases performed by a single operator.

Summary: This study investigated 9718 cases of CTO PCI conducted at 41 centers between 2012 and 2022. Proximal cap ambiguity was found in 35% of CTO lesions. Patients with proximal cap ambiguity were more likely to have had previous coronary artery bypass graft surgery, had more complex lesions, and had lower success rates. The incidence of in-hospital major adverse cardiovascular events was higher in cases with proximal cap ambiguity.

Power knuckle for facilitating the move the cap technique

Summary: Chronic total occlusions with proximal cap ambiguity are difficult to treat and have a higher failure rate. We introduce the power knuckle technique, which involves safely entering the subintimal space proximal to the proximal cap using a knuckle wire supported by a microcatheter and an inflated balloon. The power knuckle allows for easier access to the extraplaque space for subsequent procedures.

Summary: Placement of drug eluting stents is the current standard in chronic total occlusion percutaneous coronary intervention after successful lesion crossing. However, using a drug-coated balloon only strategy may be feasible, particularly with intraplaque crossing. Further studies are required to determine the optimal timing and technique for using drug-coated balloons in chronic total occlusion percutaneous coronary intervention.

Coils embolization use for coronary procedures: Basics, indications, and techniques

Summary: The use of coils is essential in interventional cardiology and can save lives in specific situations. Coils are classified by materials (bare metal, fiber coated, and hydrogel coated) and deliverability method (pushable or detachable). The size of coils is important to ensure compatibility with the delivery catheter's inner diameter and to avoid complications. Clinically, coils are used in acute or elective settings, such as sealing coronary perforation or treating congenital abnormalities, aneurysms, fistulas, or arterial side branch steal syndrome after CABG. Fluoroscopy guidance is necessary for coil delivery, and regular follow-up is required due to associated complications. Education, training, and hands-on workshops are needed to gain experience in using coils in uncommon situations.
