Prevention of Primary Cytomegalovirus Infection in Pregnancy

Background: Cytomegalovirus (CMV) is the leading infectious agent causing congenital sensorineural hearing loss and psychomotor retardation. CMV vaccine is currently unavailable and treatment options in pregnancy are limited. Susceptible pregnantwomen caring for children are at high risk for primary infection. CMV educational and hygienic measures have the potential to prevent primary maternal infection. Methods: A mixed interventional and observational controlled study was conducted to investigate the effectiveness of hygiene information among pregnantwomen at risk for primary CMV infection for personal/occupational reasons. In the intervention arm, CMV-seronegative women, identified at the time of maternal serum screening for fetal aneuploidy at 11-12 weeks of gestation, were given hygiene information and prospectively tested for CMV until delivery. The comparison arm consisted of women enrolled at delivery who were neither tested for nor informed about CMV during pregnancy, and who had a serum sample stored at the screening for fetal aneuploidy. By design, groups were homogeneous for age, parity, education, and exposure to at least one risk factor. The primary outcome was CMV seroconversion. Acceptance of hygiene recommendations was a secondary objective and was measured by a self-report. Findings: Four out of 331 (1.2%) women seroconverted in the intervention group compared to 24/315 (7.6%) in the comparison group (delta = 6.4%; 95% CI 3.2-9.6; P < 0.001). There were 3 newborns with congenital infection in the intervention group and 8 in the comparison group (1 with cerebral ultrasound abnormalities at birth). Ninety-three percent of women felt hygiene recommendations were worth suggesting to all pregnant women at risk for infection. Interpretation: This controlled study provides evidence that an intervention based on the identification and hygiene counseling of CMV-seronegative pregnantwomen significantly prevents maternal infection. While waiting for CMV vaccine to become available, the intervention described may represent a responsible and acceptable primary prevention strategy to reduce congenital CMV. (C) 2015 The Authors. SARS-CoV-2 infection testing at delivery: a clinical and epidemiological priority

Summary: Universal testing for pregnant women during the COVID-19 pandemic is suggested as a useful strategy. During the study period, 3.1% of pregnant women tested positive for SARS-Cov-2, a significantly higher proportion than estimated by health authorities. Among the positive cases, 44.9% had symptoms or reported close contact with positive patients.

Summary: This retrospective study demonstrated improved survival outcomes for patients undergoing neoadjuvant chemotherapy followed by radical surgery compared to those undergoing concurrent chemo-radiotherapy. Therefore, our findings support the use of neoadjuvant chemotherapy as an effective alternative to standard therapy before radical surgery.

Summary: There are differences in the severity and mortality rate of COVID-19 between the sexes, with women having better outcomes. The role of sex steroid hormones and menopause in causing this gender disparity has not been proven. Summary: This study investigated the association between perinatal leptin concentrations and growth trajectories of newborns during their first year. The results showed an inverse correlation between leptin levels and maternal plasma and milk. Premature infants seemed to be most affected by leptin, and low leptin levels appeared to hinder their ability to regain a normal body weight within one year.

The results showed that catheter ablation by radiofrequency or cryoablation is feasible and safe for these patients, with similar efficacy observed in both males and females over a median follow up of 29.7 months.

The results will provide evidence on the optimal screening imaging for cardiotoxicity.

Summary: A study found that ECG features can predict the 30-day survival rate of patients after out-of-hospital cardiac arrest. Age >62 years, female sex, QRS wider than 120 ms, the presence of a Brugada pattern, and ST-segment elevation in >1 segment were independently associated with 30-day mortality. By dividing the population into three risk classes based on a score, significant differences in survival rates at 30 days were observed.

Mid-Term Clinical Outcomes and Hemodynamic Performances of Trifecta and Perimount Bioprostheses following Aortic Valve Replacement

Summary: The study evaluated the clinical outcomes and hemodynamic performance of two standard aortic bioprostheses. The results showed that the postoperative outcomes and mid-term follow-up were similar between the Trifecta and Perimount bioprostheses. Although the Trifecta valve showed better hemodynamic performance in the early postoperative period, this advantage did not persist over time. There was no difference in the reoperation rate for structural valve degeneration between the two valves.

Summary: This study compared the outcomes of transcatheter aortic valve replacement (TAVR) procedures performed through alternative access routes. The results showed that both transapical and direct transaortic TAVR procedures provided satisfactory clinical results and had similar thirty-day outcomes in high-risk patients with severe aortic valve stenosis and diseased peripheral arteries.
