Toll-like receptors (TLRs) are key receptors of the innate immune system which are expressed on immune and nonimmune cells. They are activated by both pathogen-associated molecular patterns and endogenous ligands. Activation of TLRs culminates in the release of proinflammatory cytokines, chemokines, and apoptosis. Ischaemia and ischaemia/reperfusion (I/R) injury are associated with significant inflammation and tissue damage. There is emerging evidence to suggest that TLRs are involved in mediating ischaemia-induced damage in several organs. Critical limb ischaemia (CLI) is the most severe form of peripheral arterial disease (PAD) and is associated with skeletalmuscle damage and tissue loss; however its pathophysiology is poorly understood. This paper will underline the evidence implicating TLRs in the pathophysiology of cerebral, renal, hepatic, myocardial, and skeletal muscle ischaemia and I/R injury and discuss preliminary data that alludes to the potential role of TLRs in the pathophysiology of skeletal muscle damage in CLI.

Limb Salvage in Patients With Severe Critical Limb Ischemia (CLI) After Referral for a Second Opinion to a Dedicated CLI Center

Summary: The complexity of critical limb ischemia (CLI) requires a dedicated multidisciplinary team for treatment. This study aimed to assess the effectiveness of referring CLI patients scheduled for major amputation elsewhere to a dedicated CLI center. The analysis showed that patients in severe stages of CLI who received a second opinion from a dedicated multidisciplinary team had high limb salvage and survival rates after major amputation.

Summary: This systematic review compares the effectiveness and safety of class Ic agents for cardioversion of paroxysmal AF and finds that intravenous flecainide is the most effective option. Class Ic agents appear to be safe for pharmacologic cardioversion of AF regardless of the presence of SHD.

Burnout Among Surgeons in the UK During the COVID-19 Pandemic: A Cohort Study

Summary: The study found that during the COVID-19 pandemic, there is a high prevalence of burnout among surgeons in the UK, particularly in the domains of emotional exhaustion and depersonalisation. Burnout prevalence was not related to COVID-19 infection status, but was associated with the perceived impact of the pandemic on work. Concerns about contracting COVID-19 oneself and about family and friends getting infected were positively associated with burnout across all three domains.

Summary: This article examines tippy network markets that allow price discrimination and finds that under certain conditions, network competition may lead to a unique subgame-perfect equilibrium where the winner's identity is fully determined by its product features. The concept of the dominant value margin is introduced as a metric for the effectiveness of divide-and-conquer strategies. However, the outcome of a market where suppliers with larger dominant value margins can sell to all customers does not necessarily result in social efficiency.

Infective Native Extracranial Carotid Artery Aneurysms: A Systematic Review

Summary: Infective native extracranial carotid artery aneurysms are rare and there is a lack of evidence assessing outcomes. This systematic literature review identified 193 cases of infective native extracranial carotid artery aneurysms, predominantly in male patients. The most common presenting features were a neck mass and fever, and the internal carotid artery was the most common location. Treatment strategies involved open surgical methods and endovascular approaches, with fewer complications in the endovascular group.

Best Practice Guide for Cryoballoon Ablation in Atrial Fibrillation: The Compilation Experience of More than 1000 Procedures

Summary: Nowadays, cryoballoon (CB) has become an established alternative to radio frequency (RF) ablation for pulmonary vein isolation (PVI), with the ability to isolate PVs in a single application. In a study on over 1000 consecutive patients, our center optimized the CB PVI procedure. It is expected that future guidelines will recommend CB as the first-line PVI for patients with paroxysmal AF and a class I indication. In a long-term follow-up of the EARLY-AF trial, CB showed a lower incidence of persistent atrial fibrillation compared to anti-arrhythmic drugs.
