Global Inequities in Precision Medicine and Molecular Cancer Research

Precision medicine based upon molecular testing is heralded as a revolution in how cancer is prevented, diagnosed, and treated. Large efforts across the world aim to conduct comprehensive molecular profiling of disease to inform preclinical models, translational research studies and clinical trials. However, most studies have only been performed in patients from high-income countries. As the burden on non-communicable diseases increases, cancer will become a pressing burden across the world, disproportionately affecting low-middle income settings. There is emerging evidence that the molecular landscape of disease differs geographically and by genetic ancestry, which cannot be explained by environmental factors alone. There is a lack of good quality evidence that characterises the molecular landscape of cancers found in low-middle income countries. As cancer medicine becomes increasingly driven by molecular alterations in high-income settings, low-income settings may become left behind. Further efforts on an international scale must be made by researchers, funders, and policymakers to ensure cancer research addresses disease across the world, so models are not limited to subtypes of disease found in high-income countries. In this review, we discuss differences found in the molecular profiles of tumours worldwide and the implication this has for the future of global cancer care. Finally, we identify several barriers currently limiting progress in this field and innovative solutions, which may address these shortcomings.

Summary: This study aimed to investigate the clinical characteristics of hospitalized children and young people with SARS-CoV-2 during the UK's second wave. The results showed that patients in the second wave had lower PEWS, less antibiotic use, and fewer respiratory and cardiovascular support compared to the first wave. However, there was no change in the proportion of patients admitted to critical care. At least 20% of patients in this cohort had asymptomatic/incidental SARS-CoV-2 infection.

Summary: This study aimed to determine if remote wound reviews using telemedicine can be safely upscaled and whether standardized assessment tools are needed. The findings showed that the rate of surgical site infection reported using telemedicine was slightly lower than with in-person review, indicating the need for standardized tools for remote assessment of surgical site infection.

Summary: This study examines the educational impact of clinical trial recruitment for medical students. The findings show that involving medical students as recruiters can accelerate the recruitment rate of clinical trials and improve their clinical and academic competencies. Adequate training, support, and suitable trial selection are essential for future student involvement in randomised trials.

Summary: Immunocompromised patients have a higher risk of in-hospital death from COVID-19 compared to immunocompetent patients. The risk of death for immunocompromised patients decreases at a slower rate as the pandemic progresses, especially for older age groups. Targeted measures should be taken to protect and treat this vulnerable patient group.

Summary: This study examined the nasal and plasma antibody responses in COVID-19 hospitalized patients one year after discharge and vaccination. The findings showed sustained elevated antibody responses in both nasal and plasma samples for at least 12 months, but the nasal antibody response was minimally influenced by vaccination. These findings highlight the importance of developing vaccines that enhance nasal immunity.

Summary: This study aimed to characterize and identify risk factors for patients with persistent breathlessness following COVID-19 hospitalization and develop a prediction model. The results showed that lower socioeconomic status, preexisting depression/anxiety, female sex, and longer hospital stay were risk factors for post-COVID-19 breathlessness. In contrast, Black ethnicity and older age groups were less likely to experience post-COVID-19 breathlessness. Breathlessness was associated with decreased exercise capacity and lung function, but not with obstructive airflow limitation.

Summary: This study investigated the prevalence and nature of sleep disturbance after discharge following hospital admission for COVID-19 and found that sleep disturbance is associated with dyspnoea, anxiety, and muscle weakness. Summary: This international study examined the impact of global nutritional state on early postoperative outcomes in patients undergoing surgery for colorectal or gastric cancer. Severe malnutrition was found to be a risk factor for 30-day mortality and major complications following elective surgery, with a greater burden in low-income and lower-middle-income countries. Therefore, there is an urgent need for global nutritional interventions to improve early outcomes in patients with gastrointestinal cancer.
