摘要

Acute coronary syndrome (ACS) is a major health care and economic burden in the United States and accounts for more than 1 million hospitalizations annually. The morbidity and mortality due to ACS are substantialnearly half of all deaths due to coronary heart disease occur following an ACS. This review provides an up-to-date summary of the pathophysiology, diagnosis, and treatment of ACS. We also provide an overview of the management of common hemodynamic disturbances and myocardial infarction complications that physicians often encounter in an intensive care setting.

Serum Concentrations of Ischaemia-Modified Albumin in Acute Coronary Syndrome: A Systematic Review and Meta-Analysis

Summary: This study conducted a systematic review and meta-analysis to explore the potential of ischaemia-modified albumin (IMA) as a biomarker for acute coronary syndrome (ACS). The findings suggest that IMA concentrations are significantly higher in ACS patients, indicating its potential usefulness in diagnosing ACS. Further research is needed to compare the diagnostic performance of IMA with established markers and to understand its potential in distinguishing between different types of ACS.

Summary: This study aimed to examine the 1-year prognosis of patients surviving acute coronary syndrome (ACS) with concomitant first-time detected atrial fibrillation (AF). The results showed that first-time detected AF in ACS patients was strongly associated with ischaemic stroke, mortality, and bleeding rates comparable to patients with a history of AF.

Summary: Although progress has been made in diagnosing and treating acute coronary syndromes, cardiovascular disease remains a leading cause of death globally. High-sensitivity troponin assays have allowed for rapid diagnosis in suspected cases. Dual antiplatelet therapy and additional preventive measures are recommended. The diagnosis and management of acute coronary syndromes continue to evolve, especially in response to the COVID-19 pandemic.

Factors that influence whether patients with acute coronary syndromes undergo cardiac catheterisation

Summary: The study found that ACS patients presenting to hospitals with invasive coronary angiography availability were more likely to undergo catheterisation procedures. However, patients with similar baseline characteristics and major adverse cardiovascular events, as well as all-cause death rates, were observed regardless of the hospital's capability for catheterisation. This suggests equitable management of ACS patients across Australia.

No Association of Hair Zinc Concentration with Coronary Artery Disease Severity and No Relation with Acute Coronary Syndromes

Summary: This study assessed the zinc nutritional status in patients with coronary artery disease using inductively coupled plasma optical emission spectrometry. The results showed no association between zinc and the severity of coronary artery disease, and there were no significant differences in zinc levels between patients with acute coronary syndrome and stable coronary artery disease. However, a significant negative correlation was observed between zinc content and serum triglyceride concentration, and patients with type 2 diabetes mellitus had lower hair zinc content.

Summary: Patients with left main coronary artery disease presenting with ACS have higher rates of early death, but the long-term all-cause mortality rates were similar between PCI and CABG in this high-risk subgroup. The advantages and disadvantages of PCI and CABG in terms of early stroke and long-term spontaneous MI and repeat revascularization were consistent regardless of ACS status.

Summary: This study aimed to report the demographics, angiographic findings, and in-hospital outcomes of COVID-19 ACS patients and compare them with pre-COVID-19 cohorts. The study found that COVID-19-positive ACS patients experienced delayed presentation and had higher in-hospital mortality rates compared to the control subjects before the COVID-19 pandemic. Excessive rates of and mortality from cardiogenic shock were major contributors to the worse outcomes in COVID-19 positive STEMI patients.

Results showed a high 30-day all-cause mortality rate, emphasizing the urgent need for a nationwide program to reduce pre-hospital delay, promote the use of guideline-directed medications, and increase access to reperfusion therapy.

Summary: This study aimed to evaluate whether double-dose influenza vaccination during hospitalization for acute coronary syndrome (ACS) would further reduce the risk of major cardiopulmonary events. The results showed that double-dose influenza vaccination before discharge did not reduce cardiopulmonary outcomes compared with standard-dose vaccination in the outpatient setting.

Summary: This study reveals the association between historical redlining policies and contemporary heart failure risk, with racial discrimination. The investigation was conducted among White and Black Medicare beneficiaries, and it found that living in zip codes with a higher redlining proportion is closely associated with an increased risk of heart failure, especially in socioeconomically disadvantaged regions.

CPR Quality Assessment in Schoolchildren Training

Summary: This study evaluated the use of a checklist as a tool for assessing chest compression quality in schoolchildren. The results showed moderate to high agreement between the checklist and real-time software in evaluating CPR performance. Older schoolchildren performed CPR better regardless of sex and BMI.

Summary: This community-based study investigated students' knowledge of stroke, emergency medical services activation, associated risk factors, warning signs and symptoms, and prior experience in the KIDS SAVE LIVES BRAZIL project. The study found varying levels of awareness among students, with factors such as gender, education level, and prior experience influencing knowledge of stroke. Schoolchildren and adolescents should be the main target for stroke awareness.
