Acute coronary syndrome (ACS) is a major health care and economic burden in the United States and accounts for more than 1 million hospitalizations annually. The morbidity and mortality due to ACS are substantialnearly half of all deaths due to coronary heart disease occur following an ACS. This review provides an up-to-date summary of the pathophysiology, diagnosis, and treatment of ACS. We also provide an overview of the management of common hemodynamic disturbances and myocardial infarction complications that physicians often encounter in an intensive care setting.

Community-Level Economic Distress, Race, and Risk of Adverse Outcomes After Heart Failure Hospitalization Among Medicare Beneficiaries

Amgad Mentias, Milind Y. Desai, Mary S. Vaughan-Sarrazin, Shreya Rao, Alanna A. Morris, Jennifer L. Hall, Venu Menon, Jason Hockenberry, Mario Sims, Gregg C. Fonarow, Saket Girotra, Ambarish Pandey

Summary: The association between community-level economic distress and adverse outcomes in heart failure patients differs across race and ethnic groups. White patients show a stronger association at short and long-term follow-up, while for Black patients, this association is less evident in the short term but becomes more prominent in long-term follow-up and rural areas.

Achieving Equity in Hospital Performance Assessments Using Composite Race-Specific Measures of Risk-Standardized Readmission and Mortality Rates for Heart Failure

Amgad Mentias, Eric D. Peterson, Neil Keshvani, Dharam J. Kumbhani, Clyde W. Yancy, Alanna A. Morris, Larry A. Allen, Saket Girotra, Gregg C. Fonarow, Randall C. Starling, Paulino Alvarez, Milind Y. Desai, Peter Cram, Ambarish Pandey

Summary: This study found that using race-specific risk adjustment models can more accurately represent hospital performance for patients of Black and other races in terms of heart failure hospitalization and 30-day risk-standardized readmission rate and risk-standardized mortality rate.

Cardiac Troponin I in Patients Undergoing Percutaneous and Surgical Myocardial Revascularization: Comparison of Analytical Methods

Summary: The study aimed to establish the cutoff values and the accuracy of three different cTnI kits in the diagnosis of post-procedural MI. Low accuracy was observed after applying the current guidelines recommendations, but the accuracies of the three cTnI assays improved when adjusted by the new ROC cutoffs.

Historical Redlining, Socioeconomic Distress, and Risk of Heart Failure Among Medicare Beneficiaries

Summary: This study reveals the association between historical redlining policies and contemporary heart failure risk, with racial discrimination. The investigation was conducted among White and Black Medicare beneficiaries, and it found that living in zip codes with a higher redlining proportion is closely associated with an increased risk of heart failure, especially in socioeconomically disadvantaged regions.

Summary: This community-based study investigated students' knowledge of stroke, emergency medical services activation, associated risk factors, warning signs and symptoms, and prior experience in the KIDS SAVE LIVES BRAZIL project. The study found varying levels of awareness among students, with factors such as gender, education level, and prior experience influencing knowledge of stroke. Schoolchildren and adolescents should be the main target for stroke awareness.
