Probiotics are defined as live micro-organisms that when administered in adequate amounts confer a health benefit on the host. Among their pleiotropic effects, inhibition of pathogen colonization at the mucosal surface as well as modulation of immune responses are widely recognized as the principal biological activities of probiotic bacteria. In recent times, the immune effects of probiotics have led to their application as vaccine adjuvants, offering a novel strategy for enhancing the efficacy of current vaccines. Such an approach is particularly relevant in regions where infectious disease burden is greatest and where access to complete vaccination programs is limited. In this study, we report the effects of the probiotic, Lactobacillus rhamnosus GG (LGG) on immune responses to tetanus, Haemophilus influenzae type b (Hib) and pneumococcal conjugate (PCV7) vaccines in infants. This study was conducted as part of a larger clinical trial assessing the impact of maternal LGG supplementation in preventing the development of atopic eczema in infants at high-risk for developing allergic disease. Maternal LGG supplementation was associated with reduced antibody responses against tetanus, Hib, and pneumococcal serotypes contained in PCV7 (N = 31) compared to placebo treatment (N = 30) but not total IgG levels. Maternal LGG supplementation was also associated with a trend to increased number of tetanus toxoid-specific T regulatory in the peripheral blood compared to placebo-treated infants. These findings suggest that maternal LGG supplementation may not be beneficial in terms of improving vaccine-specific immunity in infants. Further clinical studies are needed to confirm these findings. As probiotic immune effects can be species/strain specific, our findings do not exclude the potential use of other probiotic bacteria to modulate infant immune responses to vaccines.

Biomarkers of asthma relapse and lung function decline in adults with spontaneous asthma remission: A population-based cohort study

Daniel J. Tan, Caroline J. Lodge, Eugene Haydn Walters, Adrian J. Lowe, Dinh S. Bui, Gayan Bowatte, Rangi Kandane-Rathnayake, Fahad M. Aldakheel, Bircan Erbas, Garun S. Hamilton, Paul S. Thomas, Mark Hew, Mimi L. K. Tang, Michael J. Abramson, Jennifer L. Perret, Shyamali C. Dharmage

Summary: This study found that subclinical inflammation exists in adults with spontaneous asthma remission, and these inflammatory biomarkers are associated with future asthma relapse and lung function decline. Bronchial hyperresponsiveness (BHR) and serum inflammatory cytokines have prognostic value in predicting outcomes in adults with spontaneous asthma remission, suggesting the need for closer monitoring and follow-up in high-risk individuals.

Summary: PET scan can be a valuable tool in accurate staging, especially for ambiguous or equivocal lesions on CT. The study showed that PET provided additional information and resulted in an altered management plan in 18.2% of the patients.

Endpoints and Outcomes After Immunotherapy for Food Allergy: What Is Meaningful for Patients?

Melanie Lloyd, Nandinee Patel, Daniel Munblit, Mimi L. K. Tang

Summary: Multiple novel interventions for food allergy are currently in development, aiming to reduce or eliminate allergic reactions. However, the success of these therapies in improving patients' lives is difficult to determine due to limited understanding of the alignment between clinical trial efficacy endpoints and patient-centered outcomes. This review assists clinicians in interpreting clinical trial outcomes and effectively communicating risks and outcomes to patients during shared decision-making. Associations between coastal proximity and children's mental health in Australia

Laura H. Oostenbach, Jennifer Noall, Karen E. Lamb, Amber L. Pearson, Suzanne Mavoa, Lukar E. Thornton

Summary: Limited research has explored the associations between blue spaces (coastal environments) and mental health in children. This study found that living closer to the coast was associated with lower levels of depression in childhood, but not in adolescence. No associations were found with anxiety. The duration of residency at the current address appeared to have a weak effect on associations. Further research is needed to understand the specific characteristics of coastal environments that may benefit children's mental health.

Age-dependent associations of human milk oligosaccharides with body size and composition up to 4 years of age

Summary: The concentration of human milk oligosaccharides (HMOs) at 6 weeks postpartum is associated with infant anthropometry, particularly within the first 6 months. For secretor mothers, higher levels of 2'FL are associated with increased weight and length in infants, while higher levels of lacto-N-tetraose are associated with increased weight and length in nonsecretor mothers.

Modelling count, bounded and skewed continuous outcomes in physical activity research: beyond linear regression models

Muhammad Akram, Ester Cerin, Karen E. Lamb, Simon R. White

Summary: Inference using standard linear regression models (LMs) relies on assumptions that are rarely satisfied in practice. Count, bounded, and skewed outcomes can substantially violate LM assumptions. Instead of relying on transformations, generalized linear models (GLMs) should be considered as more suitable approaches for managing these types of outcomes. Physical activity researchers should add GLMs to their statistical toolboxes and be aware of situations when GLMs are a better method than traditional approaches.

Do 20-minute neighbourhoods moderate associations between work and commute hours with food consumption?

Laura Helena Oostenbach, Karen Elaine Lamb, David Crawford, Anna Timperio, Lukar Ezra Thornton

Summary: This study aimed to examine the associations between work and commute hours with food consumption and test the moderating effect of neighbourhood type. The findings suggest that non-workers and overtime workers have less healthy food behaviours compared to full-time workers. Among those employed, longer work and commute hours were positively associated with takeaway consumption. Although behaviours appeared healthier for individuals in the 20-minute neighbourhood, the differences in associations between work and commute hours with food consumption across neighbourhood type were negligible.

Summary: This study conducted a comprehensive assessment of Clinical Practice Guidelines (CPGs) for primary prevention of food allergy and atopic dermatitis. It found that although some guidelines had high quality, they performed poorly in terms of global applicability. The study calls for guideline developers to consider stakeholders' preferences, local applicability, and adapt existing recommendations to different geographical settings.

Childhood body mass index trajectories and asthma and allergies: A systematic review

Chia-Lun Chang, Gulshan Bano Ali, Jonathan Pham, Shyamali C. C. Dharmage, Caroline J. J. Lodge, Mimi L. K. Tang, Adrian J. J. Lowe

Summary: This study systematically synthesized the association between BMI trajectories in childhood and allergic diseases. The findings suggest that a persistently high BMI between 6 and 10 years of age may be associated with an increased risk of asthma at 18 years, and a rapid increase in BMI in the first 2 years of life may be associated with subsequent asthma. Maintaining a normal BMI trajectory during childhood may reduce the risk of asthma.

Summary: The TransformUs programme is being disseminated to all primary schools in Victoria, Australia, aiming to increase physical activity and reduce sedentary behavior among school children. The study protocol involves a 5-year effectiveness-implementation trial to assess the scalability and effectiveness of the programme, using mixed methods and a quasi-experimental design. Findings will be used to inform education policy and practice on promoting physical activity and reducing sedentary behaviors on a population-wide scale.

Summary: This study synthesized the best available evidence from randomized controlled trials to evaluate the efficacy and safety of oral immunotherapy (OIT) for desensitization (DS) and remission (SU) in peanut, hen's egg, and cow's milk allergies. The results showed that OIT had a positive effect on desensitization, but it also increased the risk of allergic reactions.
