Role of DAMPs and cell death in autoimmune diseases: the example of multiple sclerosis

Summary: Multiple sclerosis is a chronic neuroinflammatory demyelinating disease of the central nervous system (CNS) with unknown etiology. Two pre-clinical models, the outside-in model and the inside-out paradigm, are discussed to examine the pathogenesis of the disease. The integrating role of damage-associated molecular patterns (DAMPs) in these models is examined, focusing on their origin and sources. Environmental triggers that induce regulated cell death (RCD) or act as exogenous DAMPs are examined, as they have been identified as prolific sources of DAMPs in autoimmune diseases including multiple sclerosis. A model of a DAMP-driven positive feed-forward loop of chronic inflammatory demyelinating processes is proposed to reconcile the competing inside-out and outside-in paradigms.
