Summary: Although testosterone replacement therapy is commonly used for men with classical hypogonadism, its role in age-related decline of testosterone remains unclear due to limited evidence from long-term clinical trials. However, men aged 50 years and above with comorbidities and higher body mass index often exhibit symptoms of androgen deficiency and low testosterone levels. Clinicians face a challenging dilemma in initiating testosterone therapy and must carefully assess the benefits and risks based on limited clinical trial data. In this article, a practical approach to the assessment and management of such men is presented using a case scenario.

Summary: The study developed and validated a nomogram named TRACE for predicting the need of testosterone therapy in patients with low-normal total testosterone levels. The nomogram accurately predicted the probability of clinical impairment related to testosterone deficiency and provided a simple and reliable method for selecting hypogonadal patients who will benefit from testosterone therapy.

Summary: This study evaluated the frequency and type of androgen receptor (AR) gene variants in a large cohort of infertile males. The study found that patients with AR gene variants had lower sperm count, higher testosterone concentration, and higher androgen sensitivity index compared to patients without variants. Two novel potentially pathogenic AR variants were identified. Based on these findings, AR sequencing is suggested as a routine genetic test in cases of idiopathic oligozoospermia with high testosterone levels.

Summary: The T4DM study shows that testosterone treatment for men with impaired glucose tolerance or newly diagnosed diabetes reduces the risk of diabetes and improves sleep apnea diagnosis during the study period, but these effects disappear after the study. There is also evidence of androgen dependence in some men receiving prolonged injectable testosterone treatment.

Summary: This study aimed to investigate the impact of testosterone therapy on luteinizing hormone (LH) levels and analyze predictors of LH suppression. The study found that while 73% of patients had at least one LH level below 1 IU/ml during treatment, only 22% of patients maintained LH suppression throughout the treatment. Intramuscular route, baseline LH level, and estradiol were significant predictors of LH suppression.

Summary: This study investigated the current diagnosis and treatment trends of hypogonadism in adult men in the United States using a commercial insurance database. Results revealed higher incidence rates among men aged 35-44 and those living in the Southern United States, with an increasing prevalence of HG and decreasing treatment rates. Further research is needed to understand the factors driving the observed decline in healthcare utilization among men with hypogonadism.

Summary: Quarterly intramuscular injections with long-acting testosterone undecanoate (TU) provide stable serum testosterone concentrations over time and are preferred by many testosterone-deficient patients. However, the use of long-acting TU in elderly patients is limited due to lack of safety and feasibility studies. The study found that elderly patients require more adjustments in TU dose to reach treatment targets, but the risk of polycythemia in elderly patients does not increase compared to younger patients when age-adjusted treatment targets are achieved.

Summary: This study demonstrated that co-treatment with annatto products, such as purified annatto oil or its granules, had a more significant improvement on blood lipid profile and bone integrity compared to testosterone alone. The co-treatment also improved some endpoints not affected by testosterone reposition alone, suggesting a synergistic effect between annatto products and testosterone in promoting bone health. Overall, the granules had better results than purified annatto oil.

Limited evidence suggests that subcutaneous administration results in comparable pharmacokinetics and testosterone levels as intramuscular injection. The subcutaneous route is easier for self-administration and has the potential to improve patient adherence.

Summary: The study aimed to define the optimized inter-injection interval of injectable testosterone undecanoate (TU) treatment for hypogonadal and transmen based on individual dose titration in routine clinical practice. Results show that the optimal injection interval after individual titration resulted in the approved 12-week interval in 70% of patients, with only minor influence for clinical application of body surface area (BSA) and not of trough serum LH, FSH, and SHBG. Individually optimized inter-injection interval did not differ between men with primary or secondary hypogonadism or transmen.

Summary: The study aimed to explore the causal attributions of breast cancer among survivors and their relationship to quality of life. Findings indicated stress and worry, as well as diet or eating habits, were believed to be the main causes of breast cancer. Additionally, being diagnosed at an older age, receiving chemotherapy, and holding nonbehavioral causal attributes were significantly associated with lower quality of life.

Summary: The purpose of this study was to investigate and compare the pregnancy stress, impact, and fear of COVID-19 among working and non-working pregnant women in Korea. The study explored the influencing factors of pregnancy stress considering the pandemic situation in each group. The findings showed that working pregnant women had more pregnancy stress, particularly in concerns about labor and delivery and caring for infant. Non-working pregnant women experienced more negative impact of COVID-19 on their pregnancy experience, especially in concerns related to prenatal care, obtaining living supplies and giving birth during the pandemic situation. The study suggests the need for tailored prenatal management, education, and social supports to address the psychological distress in each group.

Summary: The objective of this study was to evaluate the effectiveness of a mobile-based intervention called "Self-care for Pregnant Women at Work (SPWW)" in enhancing the self-care practices of pregnant women who work. The results of this randomized repeated measures design showed that the intervention had a positive effect in reducing pregnancy stress and promoting health practices. Therefore, developing educational content and methods specifically targeted at pregnant women in the workplace would be beneficial.
