Oxidative stress and its downstream signaling in aging eyes

Background: Oxidative stress (OS) and its biomarkers are the biochemical end point of the imbalance between reactive oxygen species (ROS) production and the ability of the antioxidant (AOX) biological systems to fight against oxidative injury. Objective: We reviewed the role of OS and its downstream signaling in aging eyes. Methods: A search of the literature and current knowledge on the physiological and pathological mechanisms of OS were revisited in relation to the eyes and the aging process. Most prevalent ocular diseases have been analyzed herein in relation to OS and nutraceutic supplements, such as dry-eye disorders, glaucoma, age-related macular degeneration, and diabetic retinopathy. Results: Clinical, biochemical, and molecular data from anterior and posterior eye segment diseases point to OS as the common pathogenic mechanism in the majority of these ocular disorders, many of which are pathologies causing visual impairment, blindness, and subsequent loss of life quality. Studies with nutraceutic supplements in aging eye-related pathologies have also been reviewed. Conclusion: OS, nutritional status, and nutraceutic supplements have to be considered within the standards of care of older ophthalmologic patients. OS biomarkers and surrogate end points may help in managing the aging population with ocular diseases.

Understanding Neurodegeneration from a Clinical and Therapeutic Perspective in Early Diabetic Retinopathy

Summary: Recent evidence suggests that neurodegeneration plays a crucial role in the development of diabetic retinopathy. The apoptosis of neuronal cells contributes to microvascular impairment and blood-retinal barrier breakdown. Targeting neurodegeneration represents an early intervention strategy to prevent the progression of microvascular alterations. Multimodal imaging and functional assessment can help identify neuronal damage before the onset of retinopathy. Clinical features of neurodegeneration are important in identifying high-risk patients and assessing treatment efficacy. Topical compounds acting on different elements of the pathological cascade show promise in targeting neurodegeneration, with brimonidine, somatostatin, and citicoline being tested in human clinical trials.

Summary: The study concluded that after 5 years of treatment with an average of 10.6 injections, there was a mean gain in BCVA. Eyes with a better baseline BCVA and a shorter duration of symptoms were more likely to achieve better BCVA at 5 years, with more eyes achieving a BCVA of >= 20/40 and gaining >= 3 lines.

Non-arteritic anterior ischemic optic neuropathy after uneventful delivery in a healthy woman

Summary: This case report describes a healthy female who developed NAION following in vitro fertilisation and uneventful cesarean section. The only risk factor identified was a small cup-to-disc ratio. Hemodynamic and hormonal changes during late pregnancy and uneventful cesarean section may have triggered this condition.

Review for Diagnostics of the Year: Inflammatory Choroidal Neovascularization - Imaging Update

Summary: Inflammatory choroidal neovascularization (iCNV) is a rare complication of uveitis and can cause vision compromise. Fluorescein angiography (FA) is the gold standard for diagnosis, but it can be difficult to distinguish iCNV from inflammatory lesions. Optical coherence tomography (OCT) is a noninvasive imaging modality that can assist in diagnosing iCNV. OCT angiography (OCTA) uses motion contrast to visualize flow and is useful in distinguishing iCNV from inflammatory lesions. However, further studies are needed to determine its role in evaluating iCNV activity and treatment response.

Summary: Open-angle glaucoma (OAG) is the most common cause of irreversible blindness worldwide. This study aimed to investigate the relationship between specific lifestyle habits, anthropometric and endocrine-metabolic, cardiovascular, and respiratory events, and commonly used psychochemicals with the presence of ocular hypertension (OHT) or OAG in a Spanish and Portuguese population. The results showed that overweight/obesity, migraine, asthma, and smoking are major risk factors for conversion from OHT to OAG in this population.

ADVANTAGES OF OPTICAL COHERENCE TOMOGRAPHY AS A HIGH DYNAMIC RANGE IMAGING MODALITY IN SUBRETINAL HYPERREFLECTIVE MATERIAL

Summary: The purpose of this study was to use high dynamic range optical coherence tomography imaging to study subretinal hyperreflective material in patients with age-related macular degeneration. The study found that the disappearance rates, visual acuity improvement, and disruption of outer retinal layers varied among different types of subretinal hyperreflective material. This easily accessible tool may be helpful as a prognostic factor in age-related macular degeneration cases.

Oxidative Stress, Inflammatory, Angiogenic, and Apoptotic molecules in Proliferative Diabetic Retinopathy and Diabetic Macular Edema Patients

Summary: The study aimed to evaluate the molecules related to oxidative stress, inflammation, angiogenesis, and apoptosis in patients with proliferative diabetic retinopathy (PDR) and diabetic macular edema (DME). By investigating their correlation in the plasma and vitreous body, as well as examining ophthalmological examination data, the study found that oxidative stress and inflammation were increased in the plasma and vitreous samples from patients with PDR/DME, while antioxidant capacity was decreased. The study suggests that exploring the relationship between these potential biomarkers in the plasma and vitreous may help detect early molecular changes in PDR/DME and monitor therapeutic outcomes.

The study retrospectively observed the multimodal imaging features of eyes with active iMFC and chrysanthemum lesions.

Macular Anatomy Differs in Dyslexic Subjects

Summary: In this study, the thickness of different segments of the macula was compared between dyslexic subjects and normal controls. The dyslexic group was found to have significantly thicker parafoveal sectors in both eyes, while no differences were observed in the outer retina or other layers of the retina.

Retinal neurodegeneration in age-related macular degeneration

Summary: Age-related macular degeneration (AMD) is a complex and multifactorial disease that damages the unit comprising the photoreceptors, retinal pigment epithelium (RPE), Bruch's membrane, and choriocapillaris. While the outer retina is primarily affected, evidence suggests that the inner retina may also be impaired. Structural optical coherence tomography (OCT) technology has shown that both the inner and outer retina are impacted by AMD and that these impairments are associated. This review aims to describe the role of neurodegeneration in AMD to better understand the relationship between neuronal loss and outer retinal damage in this disease.

THE EFFECT OF INCREASING ACQUISITION SPEED ON OPTICAL COHERENCE TOMOGRAPHY ANGIOGRAPHY IMAGES

Summary: This study aimed to compare treatment decisions by ophthalmologists with reading center fluid identification from optical coherence tomography. The results showed that treatment and reading center fluid determination agreed in the majority of visits, with more pronounced treatment response and smaller fluid areas contributing to discrepancies. Positive fluid discrepancy did not have a substantial impact on subsequent visual acuity.
