Molecular Diagnosis of Shrimp Allergy: Efficiency of Several Allergens to Predict Clinical Reactivity

BACKGROUND: The diagnosis of shellfish allergy remains a challenge for clinicians. Several shellfish allergens have been characterized and their IgE epitopes identified. However, the clinical relevance of this sensitization is still not clear. OBJECTIVE: The objective of this study was to identify allergens and epitopes associated with clinical reactivity to shrimp. METHODS: Shrimp-sensitized subjects were recruited and grouped based on the history of shrimp-allergic reactions and challenge outcome. IgE reactivity to recombinant crustacean allergens, and IgE and IgG4 reactivity to peptides were determined. Subjects sensitized to dust mites and/or cockroach without shrimp sensitization or reported allergic reactions, as well as nonatopic individuals, were used as controls. RESULTS: A total of 86 subjects were recruited with a skin prick test to shrimp; 74 reported shrimp-allergic reactions, 58 were allergic (38 positive double-blind placebo-controlled food challenge and 20 recent anaphylaxis), and 16 were tolerant. All subjects without a history of reactions had negative challenges. The individuals with a positive challenge more frequently recognized tropomyosin and sarcoplasmic calcium-binding proteins than those found tolerant by the challenge. Especially a sarcoplasmic-calcium-binding-protein positive test is very likely to result in a positive challenge, though the frequency of recognition is low. Subjects with dust mite and/or cockroach allergy not sensitized to shrimp recognized arginine kinase and hemocyanin. Several epitopes of these allergens may be important in predicting clinical reactivity. CONCLUSION: Tropomyosin and sarcoplasmic-calciumbinding- protein sensitization is associated with clinical reactivity to shrimp. Myosin light chain testing may help in the diagnosis of clinical reactivity. Arginine kinase and hemocyanin appear to be cross-reacting allergens between shrimp and arthropods. Detection of IgE to these allergens and some of their epitopes may be better diagnostic tools in the routine workup of shrimp allergy. (C) 2015 American Academy of Allergy, Asthma & Immunology

Summary: This real-world study found that biological treatments, including omalizumab, mepolizumab, reslizumab, and benralizumab, can improve olfaction in patients with severe asthma and associated CRSwNP. There were no significant differences in olfaction improvement between different biologic drugs, with factors such as atopy, steroid use, and polyp size influencing the improvement. The study also showed no differences in olfaction improvement between N-ERD and non-N-ERD groups.

Summary: MicroRNAs (miRNAs) are increasingly being used as biomarkers for diagnosis and treatment of various diseases. This study analyzed miRNA expression profiles in serum and lung tissue from severe asthma patients with and without oral corticosteroids (OCS) treatment. MiRNA-Seq and RT-qPCR were used for evaluation and validation of miRNA levels. Altered expression of miRNAs in serum and lung tissue was observed, with hsa-miR-221-5p and hsa-miR-769-5p showing the best differentiation between OCS-treated and untreated individuals. These miRNAs were found to be associated with important signaling pathways for asthma and regulated the expression of genes involved in these pathways. The findings suggest that miRNA profiles can serve as biomarkers for OCS treatment in asthma patients.

Summary: This study aimed to calculate cutoff values for visual analog scale (VAS) of global, nasal, ocular, and asthma symptoms in the MASK-air system. The percentile-based approach resulted in lower cutoff values compared to the data-driven approach. Using a data-driven approach, we were able to define cutoff values for MASK-air VASs on allergy and asthma symptoms, allowing for better classification and improved disease management.

Summary: This study aimed to investigate the impact of pollen exposure on asthma exacerbations and found that outdoor pollen exposure may have an effect on asthma exacerbation, particularly in children under 18 years of age. Further research is needed to evaluate the effects of pollen on asthma exacerbation, considering factors such as pollen sensitization, hay fever, air pollution, green spaces, and pre-existing medications, in order to improve preventive measures.

Effect of Obesity on the Expression of Genes Associated with Severe Asthma-A Pilot Study

Summary: Asthma is a complex condition influenced by genes and environment, and obesity is a risk factor that can worsen asthma therapy and severity. This study aimed to determine the impact of obesity on the expression of genes associated with severe asthma. Three groups of subjects were examined: non-obese asthmatics, obese asthma patients, and non-asthmatic obese individuals. The findings showed that obesity decreased the expression of MSR1 and PHLDA1, and significantly reduced CPA3 expression, while having no effect on CD86, IL-10, SERPINB2, PI3, CHI3L1, and IL-8 expression. These results suggest that obesity can modify the expression of genes related to asthma, particularly severe asthma.
