Summary: This case presents a rare dual diagnosis of sarcoidosis and neuromyelitis optica with aquaporin-4 antibodies in a patient with atypical recurrent optic neuritis. Long-term immunosuppression is required for management, highlighting the importance of identifying the features and cause of atypical optic neuritis.

Summary: This study aimed to develop and validate revised classification criteria for EGPA. Through the identification, collection, and analysis of candidate criteria items, a risk score system was established for disease classification in EGPA patients and comparator diseases. The criteria demonstrated strong sensitivity and specificity after validation.

Summary: This study describes the clinical presentation, management, and outcome of endocarditis associated with antineutrophil cytoplasm antibody (ANCA)-associated vasculitides (AAV). The study found that endocarditis is a rare and potentially life-threatening manifestation of AAV, which may lead to acute valvular insufficiency and require urgent surgery.

Summary: This study compares the clinical features and laboratory profiles of ANCA-positive IE patients and primary AAV patients. The findings suggest that ANCA-positive IE patients and AAV patients have differences in age, disease duration, and organ involvement. Early differential diagnosis of IE in ANCA-positive patients is recommended, along with the use of echocardiography, serological profiles, and evaluation of multi-organ involvement to improve diagnostic accuracy.

Summary: Infective endocarditis is a heart disease caused by bacterial infection of heart valves. Antibiotics are the main treatment, but their efficacy is limited by bacterial drug resistance and biofilm formation. To overcome these issues, anti-biofilm treatments that enhance antibiotic action are urgently needed.

Summary: The objective of this study was to assess the diagnostic yield of sural nerve biopsy in patients with vasculitis and investigate factors associated with positive biopsy findings. The results showed that approximately half of the patients had histological evidence of vasculitis. Positive antineutrophil cytoplasmic antibody test, organ involvement other than the nervous system, and longer biopsy samples were associated with positive biopsy findings.

Summary: In the Japanese pediatric population, PR3-ANCA performed better as a serologic marker for the diagnosis of ulcerative colitis compared to MPO-ANCA. PR3-ANCA showed higher sensitivity and good specificity, making it a promising tool for diagnosing UC in children and adolescents.

Summary: This study investigated the clinical characteristics and outcomes of patients aged >= 65 years with ANCA-positive AAV in Korea. The results showed that patients in the elderly group had higher mortality rates, and older age, lower hemoglobin, higher serum creatinine level, and nontreatment were associated with mortality compared to oral CYC + steroids.

Summary: The study investigated the efficacy of tacrolimus (TAC) as maintenance therapy for antineutrophil cytoplasmic antibody-associated vasculitis (AAV) and compared it with azathioprine (AZA). The results showed no significant differences in efficacy between TAC and AZA as maintenance therapy for AAV, but a higher incidence of end-stage renal disease (ESRD) was observed in patients who received TAC after AZA.

Summary: In a single-center retrospective cohort study in Japan, it was found that low BMI was associated with a higher risk of severe infection in older adults with microscopic polyangiitis (MPA), highlighting the need for careful management to prevent this complication in this vulnerable group. Further research is needed to determine the optimal treatment strategy for these patients.

Summary: Anti-glomerular basement membrane (GBM) disease is a rare but aggressive form of glomerulonephritis, and the prognosis of patients has improved over the past few decades. Serum creatinine and crescent percentage at diagnosis are predictors for kidney outcomes, while positive antineutrophil cytoplasmic antibodies (ANCAs) are predictors for mortality.

Summary: This study evaluates the efficacy and safety of protein A immunoadsorption (PAIA) and therapeutic plasma exchange (TPE) for ANCA-associated vasculitis (AAV) patients with severe renal involvement. The results show that PAIA treatment effectively removes MPO-ANCA and IgG, and is superior to TPE in clearing MPO-ANCA. Furthermore, PAIA treatment has an advantage in reducing mortality. Higher serum creatinine, lower serum albumin or hemoglobin levels, age >60, and failure to achieve remission at 6 months independently predict the risk of end-stage renal disease (ESRD) or death in AAV patients.

Summary: This study aimed to identify determinants of adherence to oral hygiene guidelines on IE prophylaxis in patients with a history of IE. Among 100 IE patients enrolled in the study, only 40.8% were categorized as adherent to prophylaxis guidelines. Adherence was associated with non-smoking status, no symptoms of depression or cognitive decline, more valve surgery, searching for IE information, and considering oneself as adherent to IE prophylaxis.

Summary: The introduction of transcatheter pulmonary valve implantation (TPVI) has greatly improved the management of right ventricular outflow tract dysfunction. Infective endocarditis (IE) is a serious complication of TPVI that affects valve durability and patient outcomes. This article provides a comprehensive review of the current evidence on TPVI-IE, including its incidence, risk factors, causative organisms, diagnosis, and treatment.

Summary: Carcinoid heart disease (CHD) is a major complication of carcinoid syndrome (CS) associated with metastatic small intestine neuroendocrine tumors (NETs). The formation of fibrous plaques involving the right side of the heart is mainly caused by vasoactive hormones secreted by NETs, particularly serotonin. Diagnosis of CHD can be delayed as patients are often asymptomatic despite severe heart valve involvement. Treatment of CHD requires a multidisciplinary approach, including antitumor treatment, control of CS, and surgical valve replacement in severe cases.

Summary: In this study, the prognostic value of white blood cell (WBC) signal intensity on SPECT in patients with prosthetic valve endocarditis (PVE) treated conservatively was evaluated, and the evolution of the WBC signal under antibiotics was described. The results showed that intense WBC signal was associated with poor outcomes, and WBC-SPECT imaging could serve as a useful tool for risk stratification and monitoring the efficacy of antibiotic treatment.
