Summary: This study aimed to identify predictors of good functional outcome after symptomatic intracerebral hemorrhage (SICH) following intravenous thrombolysis for acute ischemic stroke. The study analyzed patient data from the Safe Implementation of Treatment in Stroke-International Stroke Thrombolysis Register (SITS-ISTR) and found that higher baseline National Institutes of Health Stroke Scale (NIHSS) score and 24-hour NIHSS score were associated with lower likelihood of achieving good and excellent functional outcomes at 3 months. Age, baseline NIHSS score, 24-hour NIHSS score, admission glucose values, and hematoma location were identified as predictors of mortality and disability at 3 months.

Summary: We compared the efficacy and safety of endovascular therapy (EVT), intravenous (IV) thrombolysis and conservative treatment in patients with M2 segment occlusion stroke. The study found that EVT had better functional outcomes compared to IV thrombolysis and conservative treatment in patients with moderate to severe acute stroke with M2 occlusion. There was no significant difference in the incidence of symptomatic intracranial hemorrhage among the three groups.

Summary: The COVID-19 pandemic had a significant impact on stroke admissions, IV thrombolysis (IVT), and mechanical thrombectomy volumes, with a decline observed during the first year. However, mechanical thrombectomy volumes remained stable in high-volume centers. These findings suggest a preservation of stroke care for higher severity of disease during the first year of the pandemic.

Summary: This nationwide observational study evaluated the efficacy and safety of thrombolysis preceded by dabigatran-reversal in acute ischemic stroke patients. The results showed that there was no significant difference in symptomatic intracranial hemorrhage, death, and good functional outcome between the reversal group and the control group. However, reversal treatment seemed to be associated with a slight increase in the risk of symptomatic intracranial hemorrhage.

Summary: This international survey found that most stroke providers believed EVT to be superior to medical management in carefully selected patients, and indicated they would not randomize a BAO patient to medical treatment. Summary: This study aimed to evaluate the outcomes, safety, and time intervals of mechanical thrombectomy (MT) in patients with in-hospital stroke (IHS) compared with out-of-hospital stroke (OHS) patients. The results showed that IHS patients had worse functional outcomes and higher mortality at 3 months. However, the recanalization rates and risk of symptomatic intracranial hemorrhage were similar. Improvements in the management and shorter time intervals are needed for IHS patients.

Summary: Retrospective analysis of 185 cases of acute ischemic stroke patients showed that changes in the pattern of brain vasculature can predict the absence of reperfusion and failure of early neurological improvement after endovascular treatment. However, it is not able to predict poor 90-day outcome. In addition, these changes also predicted the risk of hemorrhagic transformation.

Summary: This study compared the differences between selective and non-selective treatments in mild acute ischemic stroke through endovascular thrombectomy. The results showed that there were no significant differences in the safety and efficacy of acute reperfusion treatments between disabling and nondisabling patients. Similar treatment approaches can be adopted for both groups. However, randomized data are needed to determine the best reperfusion treatment for mild acute ischemic stroke.

Summary: This study evaluated the efficiency of simulation training on door-to-needle time (DNT) for intravenous thrombolysis in stroke patients. It found that stroke centres that participated in simulation training shortened the DNT by 30 minutes in 2018 compared to 2015, while centres without training only shortened it by 20 minutes. The implementation of simulation training had a positive effect on reducing DNT.

Summary: This study found a significant association between blood pressure variability after endovascular thrombectomy (EVT) and mortality and disability. This association was independent of mean blood pressure levels within the first 24 hours post EVT. Therefore, blood pressure variability in the first 24 hours may be a new therapeutic target to improve outcomes after EVT for acute ischemic stroke.
