Summary: Managing patients with multiple injuries, especially those with comorbidities such as diabetes mellitus, remains challenging and increases mortality. This study investigates the impact of major trauma centres in the UK on the outcomes of polytrauma patients with diabetes. The findings show that while overall mortality has decreased, patients with diabetes still had higher mortality compared to other groups, highlighting diabetes as an independent risk factor for mortality in polytrauma patients.

Summary: This study analyzed the correlation between body mass composition (BC) variables and outcomes in trauma patients. The results showed that low muscle mass and high adipose tissue index were associated with a poor outcome in multiple trauma (MT) patients. The body mass composition at the fourth thoracic vertebra level had comparable results to that at the third lumbar level. Therefore, body mass composition is clinically valuable for assessing the prognosis of trauma patients.

Summary: This study used multiple-cause-of-death analysis to investigate the mortality and associated causes of Hemophagocytic lymphohistiocytosis (HLH). Between 2000 and 2016, HLH was mentioned in 2072 death certificates, with 232 cases as the underlying cause of death and 1840 cases as a non-underlying cause of death. The age-standardized mortality rate of HLH was 1.93 per million person-years and showed an increasing trend over the study period. HLH decedents were more likely to have associated CMV infections or hematological diseases compared to the general population.

Summary: A new bioengineering strategy using oriented biocompatible 3D PLGA/aCaP nanocomposites has been developed to enhance osseointegration and treat large bone defects. Two types of scaffolds, decellularized or seeded with adipose-derived mesenchymal stromal cells, showed high osseointegration, improved bone quality, increased alignment of collagen fibers, and optimal alignment and size of hydroxyapatite minerals, as determined by micro-CT analysis and SAXS imaging.

Summary: This biomechanical study aimed to evaluate the stability of S1-S2 fixation of the sacroiliac (SI) joint using three different screw designs. The study found that using two cannulated compression headless screws or two partially threaded SI screws achieved better interfragmentary stability compared to fully threaded SI screws. Therefore, in the treatment of posterior pelvis ring injuries in young trauma patients, the former can be considered as a valid alternative to standard screw fixation.

Summary: The purpose of this study was to evaluate construct stiffness and failure load following posterior column fractures fixation with standard plate osteosynthesis, screw fixation, or using a screwable cup for total hip arthroplasty under full weight bearing conditions. The results showed that both standard plate osteosynthesis and screwable cup fixation demonstrated encouraging results for post-surgical treatment concept with full weight bearing.

Summary: This study evaluated the biomechanical competence of posterior column acetabulum fracture fixation using either a standard cannulated screw or a cannulated compression headless screw (CCHS) via a posterior approach. The results showed that CCHS fixation had higher initial stiffness and fatigue performance compared to the standard cannulated screw. Therefore, CCHS fixation could be a valuable alternative option for treating posterior column acetabulum fractures.

Summary: The purpose of this study was to evaluate patient outcomes after reverse shoulder arthroplasty for complex proximal humerus fractures and to explore the clinical implications of greater tuberosity malunions. The study included 56 patients who underwent reverse shoulder arthroplasty. The results showed that patients with complete tuberosity healing had better clinical outcomes compared to those with tuberosity malunions.

Summary: The aim of this biomechanical feasibility study was to develop and test a novel intramedullary splinting implant for the treatment of pubic ramus fractures and evaluate its biomechanical viability compared to established fixation methods. The results showed that the novel ramus intramedullary splint can be used as an alternative option and has the potential to decrease the rate of implant failures due to its minimally invasive implantation procedure.
