Estimating the Burden of Irritable Bowel Syndrome: Analysis of a Nationwide Korean Database

Background/Aims Management of irritable bowel syndrome (IBS) imposes a heavy economic burden. This study was to estimate the epidemiologic features of IBS and to report the IBS burden for the first time in the Korean population. Methods A cross-sectional study was conducted using the National Health Insurance (NHI) system database, which covers the entire population of Korea. IBS was defined as diagnostic code -10 in adults with any outpatient clinic visits or hospitalization related to IBS. We excluded diseases that mimic IBS symptoms. Results A total of 2.42 million (58.2% female) individuals were identified as patients with IBS, yielding an age- and gender-adjusted prevalence of 5.1% in males and 6.9% in females. The prevalence of IBS increased proportionally with age, with higher medical costs in middle-aged patients. Outpatient clinics were visited by 98.6% of IBS patients, and 1.9% were treated upon admission. Of these patients, 87.6% were given a prescription. Co-morbidities that commonly accompanied IBS included upper gastrointestinal (36.1%), respiratory (12.3%), musculoskeletal (8.0%) disease, somatoform (4.3%) and depression/anxiety disorders (3.1%). The NHI costs of IBS, which include the NHI covered cost and beneficiary copayment charges, were estimated to be 155 million USD, which accounts for 0.46% of the total NHI costs for the entire Korean population. Conclusions According to the Korean national claims database, about 6% of the Korean population seeks medical care for IBS at least once per year. This high prevalence places a large economic burden on the Korean healthcare system, accounting for 0.46% of overall national medical expenditure.

Summary: The study found that the use of CSI-EMR does not increase the complete or en bloc resection rate of colorectal lesions with SSL endoscopic features, but does increase the R0 resection rate. Although the procedure time is longer for CSI-EMR, there is no significant difference in adverse events or recurrence rate compared to EMR. Further evaluation is needed to assess the association of CSI-EMR with R0 resection and non-recurrence.

Infectious events after endoscopic procedures in patients with neutropenia and hematologic diseases

Summary: This study investigated the development of infectious events after endoscopic procedures in neutropenic patients with hematologic diseases and assessed the risk factors. The results showed that procedural invasiveness and severity of neutropenia did not affect the occurrence of infectious events, whereas poor performance status was the strongest factor for the development of infectious events.

Efficacy of bougie dilation for normal diet in benign esophageal stricture

Summary: This study aimed to identify the clinical and endoscopic factors affecting the achievement of a normal diet with only bougie dilation in patients with benign esophageal stricture (BES). Results showed that approximately one-third of BES patients can achieve a normal diet through fewer than four dilation sessions, short length of stricture, pre-procedural dysphagia status, and larger dilator diameter.

Tumor heterogeneity and carcinoma in resected specimens of gastric low-grade dysplasia: A retrospective single center study

Summary: This study retrospectively analyzed endoscopic resection data from January 1999 to December 2019 to investigate the risk factors for pathological upgrades of gastric low-grade dysplasia (LGD) to early gastric cancer (EGC) or high-grade dysplasia (HGD). The results showed that lesion size exceeding 1.5 cm, mucosal nodularity, heterogeneous color, presence of erosion, and open-type gastric atrophy were significantly associated with upgraded pathology to EGC. Among EGC cases, 89.2% had pre-existing dysplasia. Therefore, endoscopic evaluations should be performed in LGDs, especially those exhibiting surface nodularity, erosion, heterogeneous color, and large size, to prevent possible pathological upgrades and co-existence of carcinomas.

Prognostic value of soluble PD-L1 and exosomal PD-L1 in advanced gastric cancer patients receiving systemic chemotherapy

Summary: The study investigated the prognostic significance of soluble PD-L1 (sPD-L1) and exosomal PD-L1 (exoPD-L1) in patients with gastric cancer receiving chemotherapy. It was found that low levels of sPD-L1 and exoPD-L1 were associated with better overall survival and progression-free survival. Pretreatment sPD-L1 could be used as an independent prognostic factor for overall survival, while exoPD-L1 may reflect the immunosuppressive state of patients.

Accuracy of administrative claim data for gastric adenoma after endoscopic resection

Summary: This study evaluated the diagnostic accuracy of an administrative database for resected gastric adenomas. The study used International Classification of Diseases (ICD) 10-codes to identify gastric adenoma cases. The results showed that the overall diagnostic accuracy of ICD-10 codes for gastric adenoma was 98.7%, with a specificity of 88.5%, positive predictive value of 95.2%, and negative predictive value of 96.8%.
