Serum Parathyroid Hormone and 25-Hydroxyvitamin D Concentrations and Risk of Incident Heart Failure: The Multi-Ethnic Study of Atherosclerosis

Background--Heart failure (HF) is common and is associated with high mortality. We aimed to determine associations of serum parathyroid hormone (PTH) and 25-hydroxyvitamin D (25[ OH] D) with incident HF and left ventricular mass. Methods and Results--Among 6459 participants in the community-based Multi-Ethnic Study of Atherosclerosis, all of whom were free of prevalent clinical cardiovascular disease, we measured serum concentrations of PTH and 25(OH) D at the baseline examination. In longitudinal analyses, we tested associations of PTH and 25(OH) D with incident HF events, adjudicated by a panel of physicians. In cross-sectional analyses of a subset of 4763 participants, we tested associations of PTH and 25(OH) D with left ventricular mass, measured by cardiac magnetic resonance imaging at baseline. Multivariable Cox proportional hazard and linear regression models were adjusted for demographics, physical examination measures, comorbidity, kidney function, and other mineral metabolism markers. Mean age was 62 years and 53% of participants were female. There were 180 incident HF events over a median (interquartile range) follow-up time of 8.46 (7.67 to 8.63) years. Compared with participants with PTH < 65 pg/mL, PTH = 65 pg/mL was associated with a 50% greater risk of incident HF (95% CI: 3% to 210%) and a 5.3 g higher left ventricular mass (95% CI: 2.6, 7.9 g). In contrast, there was no association of 25(OH) D with risk of incident HF or elevated left ventricular mass. Conclusions--In a racially/ethnically diverse population without prevalent cardiovascular disease, higher serum PTH concentration was associated with increased left ventricular mass and increased risk of incident HF. Further studies should be pursued to determine whether PTH excess may be a modifiable risk factor for

However, osteocalcin carboxylation status does not significantly differ between individuals with and without CKD. The data also do not support the hypothesis that differences in osteocalcin carboxylation may explain reduced insulin sensitivity in individuals with CKD.

Summary: This study found that supplementation with vitamin D-3 or n-3 fatty acids did not reduce inflammation levels over 5 years in adults with type 2 diabetes. While vitamin D-3 supplementation resulted in an increase in NT-proBNP, there was no significant difference compared to n-3 fatty acid supplementation.

Association of the Estimated Glomerular Filtration Rate With vs Without a Coefficient for Race With Time to Eligibility for Kidney Transplant

Summary: In a cohort study of 1658 self-identified Black adults with chronic kidney disease, commonly used estimates of kidney function did not correspond well with directly measured kidney function. Estimating kidney function not including a coefficient for race (vs including a race coefficient) was significantly associated with a shorter time to achieving an estimated glomerular filtration rate less than 20 mL/min/1.73 m(2), a key threshold of kidney function for referral and listing for kidney transplant. The findings suggest that biases in race-based glomerular filtration rate estimates may be associated with delays in potential kidney transplant eligibility.

Summary: This study evaluated the prognostic value of cardiac biomarkers and echocardiographic variables in predicting heart failure (HF) in patients with chronic kidney disease (CKD). The results showed that NT-proBNP and hsTnT had better discrimination than the current HF prediction model. HF clinical prediction models tailored to CKD patients are needed.

Association of Proximal Tubular Secretory Clearance with Long-Term Decline in Cognitive Function

Summary: Lower kidney clearances of secreted solutes are associated with incident global cognitive decline in a prospective study of CKD, independent of eGFR. However, serum concentrations of the solutes were not associated with cognitive decline. Further research is needed to determine the specific domains of cognition affected by decreased secretory clearance and the underlying mechanisms of these associations.

Summary: This study aimed to evaluate the risks of adverse effects in chronic kidney disease (CKD) patients after initiating atrial fibrillation (AF) therapies. The results showed a significant association between lower estimated glomerular filtration rate (eGFR) and higher risks of adverse effects after initiation of commonly used AF therapies. These findings can help guide therapeutic decisions for patients with CKD and AF.

Summary: The study investigated the associations of the vitamin D metabolite ratio (VDMR), 25-hydroxyvitamin D (25[OH]D), and 1,25-dihydroxyvitamin D (1,25[OH](2)D) with cardiovascular disease (CVD) in chronic kidney disease patients. The results showed that lower VDMR and 1,25(OH)(2)D were associated with incident CVD, while only 25(OH)D was associated with left ventricular mass index. However, none of these metabolites were found to be associated with incident CVD in chronic kidney disease after adjustment for relevant factors.
