Lung transplantation is a well-established treatment option for selected patients with end-stage lung disease, leading to improved survival and improved quality of life. The last 20 years have seen a steady growth in number of lung transplantation procedures performed worldwide. The increase in clinical activity has been associated with tremendous progress in the understanding of cellular and molecular processes that limit both short- and long-term outcomes. This review gives a comprehensive overview of the current status of lung transplantation for the referring physician. It demonstrates that careful selection of potential recipients, optimisation of their condition prior to transplant, use of carefully assessed donor organs, excellent surgery and meticulous long-term follow-up are all essential ingredients in determining a successful outcome.

Summary: Impairment in efferocytosis of alveolar macrophages induced by treatment with ARDS BAL fluid in an in vitro model mimics the dysfunction observed in patients with ARDS, while phagocytosis remains preserved. Partial restoration of efferocytosis can be achieved by inhibition of ROCK, suggesting a potential therapeutic target for ARDS. This in vitro model provides a useful tool to study the mechanisms underlying AM dysfunction in the inflammatory alveolar microenvironment of ARDS.

CD14-positive extracellular vesicles in bronchoalveolar lavage fluid as a new biomarker of acute respiratory distress syndrome

Summary: Recent studies suggest that EVs play important roles in the pathogenesis of ARDS and serve as potential biomarkers for ARDS. The study found that patients with sepsis-related ARDS have more monocyte-derived BAL EVs compared to sepsis patients without ARDS. However, esophagectomy patients who later developed ARDS had lower BAL EV numbers. Esophagectomy patients who developed ARDS also had higher levels of CD31+/CD63+ and CD31+/CD81+ endothelial-derived BAL EVs. Further research is needed to determine if CD31+ BAL EVs can predict ARDS in esophagectomy patients. Additionally, CD14+/CD81+ BAL EVs were significantly higher in sepsis-related ARDS patients who died within 30 days of ICU admission.
