Background: Falls are the leading cause of unintentional injury and injury-related death among older people. In addition to physical activity, vitamin D also may affect balance and neuromuscular function. Low serum 25-hydroksivitamin D level increases the risk of bone loss, falls and fractures. Thus, an appropriate exercise program and sufficient vitamin D intake may significantly improve not only functional balance, but also balance confidence. Balance represents a complex motor skill determined by reaction time, muscle strength, and speed and coordination of movement. Methods/Design: A 2-year randomized double-blind placebo-controlled vitamin D and open exercise trial of 409 home-dwelling women 70 to 80 years of age comprising four study arms: 1) exercise + vitamin D (800 IU/d), 2) exercise + placebo, 3) no exercise + vitamin D (800 IU/d), 4) no exercise + placebo. In addition to monthly fall diaries, general health status, life style, bone health, physical functioning, and vitamin D metabolism will be assessed. The primary outcomes are the rate of falls and fall-related injuries. Secondary outcomes include changes in neuromuscular functioning (e. g. body balance, muscle strength), ADL- and mobility functions, bone density and structure, cardiovascular risk factors, quality of life and fear of falling. Discussion: The successful completion of this trial will provide evidence on the effectiveness of exercise and vitamin D for falls reduction.

Vitamin D status and current policies to achieve adequate vitamin D intake in the Nordic countries

Suvi T. Itkonen, Rikke Andersen, Anne K. Bjoerk, Asa Brugard Konde, Hanna Eneroth, Maijaliisa Erkkola, Kristin Holvik, Ahmed A. Madar, Haakon E. Meyer, Inge Tetens, Johanna E. Torfadottir, Birna Thorisdottir, Christel J. E. Lamberg-Allardt

Summary: There are differences in the implementation of vitamin D policies and recommendations among Nordic countries, with better status and intake in the general adult population compared to at-risk groups. Collaboration and strategies are needed to improve vitamin D levels in all Nordic countries, particularly in high-risk populations.

Partial Replacement of Animal Proteins with Plant Proteins for 12 Weeks Accelerates Bone Turnover Among Healthy Adults: A Randomized Clinical Trial

Suvi T. Itkonen, Essi Paivarinta, Tiina Pellinen, Hanna Viitakangas, Juha Risteli, Maijaliisa Erkkola, Christel Lamberg-Allardt, Anne-Maria Pajari

Summary: The study suggests that partial replacement of animal proteins with plant-based proteins may increase markers of bone resorption and formation among healthy adults, indicating a possible risk for bone health.

Genetic Risk Score for Serum 25-Hydroxyvitamin D Concentration Helps to Guide Personalized Vitamin D Supplementation in Healthy Finnish Adults

Summary: Genetic factors can affect serum 25-hydroxyvitamin D concentration, influencing the optimal intake of vitamin D. By developing a population-matched genetic risk score, personalized vitamin D supplementation was tailored to individuals predisposed to low serum 25(OH)D concentration. Electronic return of individual genetic risk, serum 25(OH)D concentrations, and factors affecting vitamin D status were used to adjust supplementation, showing potential for application in other populations and countries.

Patterson, Inger Ohlund, Torbjorn Lind, Pia Karlsland Akeson, Christian Ritz

Summary: There is an urgent need to develop vitamin D dietary recommendations for dark-skinned populations at high latitudes, as they require higher vitamin D intake to maintain serum concentrations. The study found that the required vitamin D intake for dark-skinned individuals to maintain certain serum levels is substantially higher than that for White individuals.

Intake of vitamin B12 in relation to vitamin B12 status in groups susceptible to deficiency: a systematic review

Linnea Barebring, Christel Lamberg-Allardt, Birna Thorisdottir, Alfons Ramel, Fredrik Soderlund, Erik Kristoffer Arnesen, Bright I. Nwaru, Jutta Dierkes, Agneta Akesson

Summary: This systematic review aimed to assess the evidence for whether habitual or different levels of experimental intake of vitamin B12 from diet and supplements is sufficient to ensure adequate B12 status in groups most susceptible to vitamin B12 deficiency. The search yielded 18 included studies, and the conclusion was that the evidence is currently insufficient to determine if or which level of B12 intake is sufficient to maintain adequate status for all included populations.

Legume consumption in adults and risk of cardiovascular disease and type 2 diabetes: a systematic review and meta-analysis

Birna Thorisdottir, Erik Kristoffer Arnesen, Linnea Barebring, Jutta Dierkes, Christel Lamberg-Allardt, Alfons Ramel, Bright I. Nwaru, Fredrik Soderlund, Agneta Akesson

Summary: This study systematically reviewed the evidence for associations between legume consumption and cardiovascular disease, type 2 diabetes, and their risk factors in healthy adults. The findings suggest that while there is no clear association with the risk of cardiovascular disease and type 2 diabetes, protective effects were observed in randomized controlled trials, supporting the incorporation of legumes into diverse and healthy dietary patterns.

Animal versus plant-based protein and risk of cardiovascular disease and type 2 diabetes: a systematic review of randomized controlled trials and prospective cohort studies

Christel Lamberg-Allardt, Linnea Barebring, Erik Kristoffer Arnesen, Bright I. Nwaru, Birna Thorisdottir, Alfons Ramel, Fredrik Soderlund, Jutta Dierkes, Agneta Akesson

Summary: This study systematically reviewed the evidence on the effect of replacing animal protein with plant protein on cardiovascular disease (CVD) and type 2 diabetes (T2D). The results suggest that replacing animal protein with plant protein can reduce total cholesterol, low-density lipoprotein cholesterol, CVD mortality, and T2D incidence. Therefore, replacing animal protein with plant protein may be a public health strategy to lower the risk of CVD mortality and T2D.

Erik Kristoffer Arnesen, Birna Thorisdottir, Linnea Barebring, Fredrik Soderlund, Bright I. Nwaru, Ulrike Spielau, Jutta Dierkes, Alfons Ramel, Christel Lamberg-Allardt, Agneta Akesson

Summary: This study aimed to evaluate the relationship between nuts/seeds consumption and cardiometabolic diseases among adults. The results showed that high consumption of nuts/seeds was associated with a lower risk of cardiovascular diseases, particularly coronary heart disease. However, there was no significant association with type 2 diabetes. This study provides important insights into the effects of nuts/seeds on cardiovascular health.

Vitamin D deficiency in Bangladesh: A review of prevalence, causes and recommendations for mitigation

Md Zahirul Islam, Nizamul Hoque Bhuiyan, Mohammed Akhtaruzzaman, Christel Lamberg Allardt, Mikael Fogelholm

Summary: Vitamin D deficiency is a significant global public health problem, and there is a high prevalence of severe deficiency in different population groups in Bangladesh, especially among the elderly and women. Underlying factors for this include dark skin colour, homebound and sedentariness, insufficient sunlight exposure, atmospheric pollution, clothing style, obesity, use of sunscreen, and no supplementation. A comprehensive strategy is needed to address the health consequences of vitamin D deficiency.

Erik Kristoffer Arnesen, Birna Thorisdottir, Christel Lamberg-Allardt, Linnea Barebring, Bright Nwaru, Jutta Dierkes, Alfons Ramel, Agneta Akesson

Summary: The aim of this study was to examine the association between dietary protein intake in children and growth, as well as the risk of overweight or obesity. The study included 21 studies and found a positive association between total protein intake and BMI. In healthy Western children, early high protein intake may be causally related to higher BMI later in childhood.
