Preconception Screening for Cytomegalovirus: An Effective Preventive Approach

Congenital cytomegalovirus (CMV) is the leading infectious cause of sensorineural hearing loss and delayed psychomotor development. Viral transmission to the fetus is far more likely to occur following a primary than a secondary maternal infection. Primary prevention seems to be the best means to reduce the burden of congenital CMV due to the lack of treatment options during pregnancy. We evaluated this approach on a cohort of 500 women planning pregnancy who attended our fertility clinic. Of the 444 who underwent CMV screening, 18 (4.1%) had positive IgM serology for CMV; of these, IgG avidity was high in 12 (remote infection) and low in 6 (recent infection). The latter were advised to delay pregnancy. All women who were seroimmune for CMV (366/444, 82.4%), including the 12 with remote infection, continued fertility treatment. The remaining patients (72/444, 16.2%), who were not immune to CMV at the initial screen, were advised to minimize CMV exposure by improving personal hygiene and to continue fertility treatment. None of the 69/72 (95.8%) women who were followed for one year were infected with CMV. Cytomegalovirus testing and counselling at preconception seemed effective in reducing CMV exposure in pregnancy.

Induction of labor at second delivery subsequent to a primary cesarean: is stage of labor at previous cesarean a factor?

Summary: This study found no association between the stage of labor at previous cesarean delivery and successful induction of labor. However, augmentation of labor with oxytocin, epidural analgesia, and birth weight of the newborn less than 4000 g at P2 were associated with successful induction of labor.

Number of prior vaginal deliveries and trial of labor after cesarean success

Summary: The study found that prior VBAC can predict the success rate of TOLAC and the rate of uterine rupture, but as a single variable, its predictive value is low. Women with prior VBAC had a higher VBAC success rate compared to those with prior VD, but a lower rate of uterine rupture. The number of prior VDs did not improve predictive accuracy.

Sonographic growth curves versus neonatal birthweight growth curves for the identification of fetal growth restriction

Summary: The study aimed to compare the accuracy of sonographic versus birthweight curves in diagnosing fetal growth restriction and to compare maternal and neonatal outcomes based on these two approaches. The study found that pregnancies classified as appropriate for gestational age based on birthweight curves but classified as growth restricted based on sonographic curves had higher rates of small for gestational age and poor perinatal outcomes, similar to pregnancies classified as growth restricted based on birthweight curves. Furthermore, the use of sonographic curves increased the sensitivity for detecting small for gestational age neonates.

Summary: This study aimed to evaluate the association between midwives' annual birth workload and short-term adverse maternal and neonatal outcomes. The results showed that adverse perinatal outcomes are only mildly affected by midwives' annual volume after controlling for the midwife, parturient and neonate's characteristics.

Maternal and Neonatal Outcomes in Twin Deliveries with Prolonged Second-Stage Duration of the Presenting Twin: a Retrospective Cohort Study

Summary: This study examined the relationship between the duration of the second stage of labor in twin pregnancies and adverse maternal and neonatal outcomes. The results suggest that a prolonged second stage of labor may increase the risk of adverse maternal outcomes, especially in nulliparous women. Further large-scale studies are needed to confirm these findings.

Repeat low order caesarean delivery, risk factors for complications: A retrospective, longitudinal study

Summary: The study aimed to identify risk factors for complicated maternal cesarean delivery (CD) in women undergoing a repeat low-order CD and to develop a predictive model for at-risk women. The results showed that a complicated CD2, Sub-Saharan African origin, and preterm delivery were risk factors for complications in CD3. However, it is difficult to predict which specific women will experience these complications. The sensitivity, specificity, positive predictive value, and negative predictive value of a complicated CD2 for detecting complications in CD3 were 21%, 90%, 30%, and 85%, respectively.

The yield of procalcitonin and Interleukin-6 in predicting intraamniotic infection in the presence of intrapartum fever: A pilot study

Summary: Intrapartum fever accompanied by certain symptoms is classified as suspected triple 1, which is the hallmark of intraamniotic infection (IAI). However, the clinical diagnosis of IAI has poor specificity, leading to unnecessary treatment. This study evaluated the use of specific acute phase reactants (APRs) to detect bacterial IAI among suspected triple 1 cases, but their effectiveness was not sufficient, as indicated by the poor area under the receiver operating characteristic curve.
