The missing role of pharmacists in public hospitals in Pakistan has led to poor management of COPD, but they can help by educating patients, implementing management protocols, and following guidelines.

Summary: This study aims to evaluate the clinical symptoms that determine changes in both pharmacological and non-pharmacological treatment for COPD patients. The study provides suggestions for optimizing COPD management based on the insights gained. The observational cohort study involved 24 general practitioners who screened 260 COPD patients for eligibility. General practitioners collected general information and symptoms using a standardized questionnaire. Follow-up visits were conducted to assess symptoms and changes in therapy, which were recorded in a central electronic database. The results showed that treatment adjustments were made during 28% of visits. The severity of the disease and the presence of comorbidities influenced the likelihood of therapy adjustment. Exacerbations and the severity of obstruction were associated with changes in therapy. The study provides valuable insights into the management of COPD patients in a primary care setting.
