Feasibility of α-fetoprotein as a diagnostic tool for hepatocellular carcinoma in Korea

Background/Aims: The aim of this study was to evaluate the feasibility of a-fetoprotein (AFP) as a diagnostic tool for hepatocellular carcinoma (HCC) in Korean patients. Methods: We retrospectively reviewed the medical records of HCC and cirrhosis patients at three hospitals. For each HCC patient, a cirrhosis patient matched for age, sex, etiology, and Child-Pugh classification was selected by simple random sampling. The performance of AFP in the diagnosis of HCC was determined using receiver operating characteristic curve analysis. Results: A total of 732 patients with HCC or cirrhosis were selected for each case and the control groups. The mean age was 54 years, and 72.4% of patients were male. The mean serum AFP levels in the HCC group and cirrhosis group were 3,315.6 and 117.2 ng/mL, respectively (p < 0.001). The area under the receiver operating characteristic curve for all HCC patients was 0.757. The sensitivity, specificity, and positive predictive value of AFP was 50.55%, 87.70%, and 80.43%, respectively, at a cut-off of 20 ng/mL; 37.70%, 95.90%, and 90.20%, respectively, at a cutoff of 100 ng/mL, and 30.05%, 97.27%, and 91.67%, respectively, at a cut-off of 200 ng/mL. A cut-off of 100 ng/mL was more sensitive than one of 200 ng/mL with equivalent specificity and positive predictive value. Conclusions: The cut-off AFP value for early-stage HCC was 17.4 ng/mL. Our study cautiously suggests that AFP has a role in the diagnosis of HCC, and that the appropriate value of AFP for the diagnosis of HCC may be 100 ng/mL rather than 200 ng/mL.

Summary: This study used network meta-analysis to compare the efficacy of different prophylactic modalities for preventing post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP). The results showed that rectal NSAIDs-based combination regimens with aggressive hydration or nitrate are more effective in preventing PEP than single modalities.

Aggressive hydration for preventing post-endoscopic retrograde cholangiopancreatography pancreatitis: trial sequential analysis

Summary: This study analyzed 12 studies and found that aggressive hydration can significantly reduce the incidence of pancreatitis after endoscopic retrograde cholangiopancreatography (ERCP). However, the effect of aggressive hydration is not significant in high-risk populations and patients with moderate-severe pancreatitis, suggesting the need for further research.

Comparison of the Effectiveness of Palonosetron and Ramosetron in Preventing Postoperative Nausea and Vomiting: Updated Systematic Review and Meta-Analysis with Trial Sequential Analysis

Summary: This study compared the efficacy of perioperative administration of palonosetron and ramosetron in preventing postoperative nausea and vomiting (PONV). A total of 17 randomized controlled trials were included in the analysis, and the results showed that palonosetron was more effective than ramosetron in reducing vomiting and late vomiting. However, there were no significant differences in nausea, PONV, complete response, use of antiemetics, and adverse effects between the two drugs. Subgroup analysis showed that palonosetron was superior to ramosetron in early PON, late PON, overall POV, and use of rescue antiemetics when administered early, while ramosetron was superior in early PON when administered late. The prophylactic administration of palonosetron was more effective than ramosetron in preventing retching and late POV. No significant differences in PONV prevention were found between the two drugs in this meta-analysis. Further studies are needed to confirm these findings.

Postoperative Segmental Motion up to 1 Year Following Single-Level Anterior Cervical! Discectomy and Fusion: Plate versus Non-plate

Summary: This retrospective observational study investigated the impact of plating on postoperative serial segmental motion and its correlation with clinical outcomes in single-level anterior cervical discectomy and fusion (ACDF) for up to 1 year. The results showed that using cervical plating significantly decreases postoperative serial segmental motion and is correlated with decreased neck pain, particularly at 3 and 6 months postoperatively. Therefore, the authors recommend the use of plating in single-level ACDF for better early clinical outcomes.

Effect of active airway warming with a heated-humidified breathing circuit on core body temperature in patients under general anesthesia: a systematic review and meta-analysis with trial sequential analysis

Summary: This study investigated the effectiveness of heated-humidified breathing circuit (HHBC) in maintaining the core temperature of patients receiving mechanical ventilation under general anesthesia. The analysis of 18 randomized controlled trials revealed that patients with HHBC had significantly higher core temperature at the end of surgery compared to those without any device or with heat and moisture exchanger. This suggests that HHBC can be used as an effective supplemental device to maintain intraoperative core temperature.

Effect of Immature Rubus occidentalis on Postoperative Pain in a Rat Model

Summary: This study aimed to compare the analgesic properties of immature and mature Rubus occidentalis extracts (iROE and mROE) using a rat model of postoperative pain. The results showed that iROE had better analgesic and anti-inflammatory effects compared to mROE. The analgesic activity of iROE may be associated with α2-adrenergic and opioid receptors.
