Summary: Hepatocellular carcinoma (HCC) is a malignant tumor with a grim prognosis, and early diagnosis is difficult. Alpha-fetoprotein (AFP) has been used as a surveillance tool, but its specificity and sensitivity are suboptimal. Studies show no clear correlation between AFP levels and HCC tumor size, stage, or survival. The presence of persistently rising AFP does not offer diagnostic benefit, with low sensitivity and specificity. Rising AFP levels alone do not support a role in the diagnosis of HCC.

Summary: This article reports a case of rare sarcomatoid hepatocellular carcinoma, which was initially misdiagnosed as hepatocellular carcinoma, leading to inappropriate treatment. Sarcomatoid hepatocellular carcinoma is more likely to recur and has a worse prognosis. Aggressive surgical resection seems to be the most appropriate treatment for this rare cancer.

Summary: This study investigated the relationship between basal parameters and survival characteristics in patients with hepatocellular carcinoma (HCC). The results showed that the number and diameter of tumors, platelet count, platelet-to-lymphocyte ratio, level of aspartate aminotransferase, alanine aminotransferase, gamma-glutamyl transferase, portal and hepatic vein involvement, and alpha-fetoprotein level were independently related to survival. The findings of this study may have important implications for treatment decisions in patients with HCC.

Summary: This study compared the efficacy of perioperative administration of palonosetron and ramosetron in preventing postoperative nausea and vomiting (PONV). A total of 17 randomized controlled trials were included in the analysis, and the results showed that palonosetron was more effective than ramosetron in reducing vomiting and late vomiting. However, there were no significant differences in nausea, PONV, complete response, use of antiemetics, and adverse effects between the two drugs. Subgroup analysis showed that palonosetron was superior to ramosetron in early PON, late PON, overall POV, and use of rescue antiemetics when administered early, while ramosetron was superior in early PON when administered late. The prophylactic administration of palonosetron was more effective than ramosetron in preventing retching and late POV. No significant differences in PONV prevention were found between the two drugs in this meta-analysis. Further studies are needed to confirm these findings.

Summary: This retrospective observational study investigated the impact of plating on postoperative serial segmental motion and its correlation with clinical outcomes in single-level anterior cervical discectomy and fusion (ACDF) for up to 1 year. The results showed that using cervical plating significantly decreases postoperative serial segmental motion and is correlated with decreased neck pain, particularly at 3 and 6 months postoperatively. Therefore, the authors recommend the use of plating in single-level ACDF for better early clinical outcomes.

Summary: This study investigated the effectiveness of heated-humidified breathing circuit (HHBC) in maintaining the core temperature of patients receiving mechanical ventilation under general anesthesia. The analysis of 18 randomized controlled trials revealed that patients with HHBC had significantly higher core temperature at the end of surgery compared to those without any device or with heat and moisture exchanger. This suggests that HHBC can be used as an effective supplemental device to maintain intraoperative core temperature.

Summary: This study aimed to compare the analgesic properties of immature and mature Rubus occidentalis extracts (iROE and mROE) using a rat model of postoperative pain. The results showed that iROE had better analgesic and anti-inflammatory effects compared to mROE. The analgesic activity of iROE may be associated with α2-adrenergic and opioid receptors.
