Summary: This study aimed to evaluate the ability of neutrophil gelatinase-associated lipocalin (NGAL) in differentiating acute kidney injury (AKI) from chronic kidney disease (CKD). The results showed that urinary NGAL (uNGAL) levels were significantly higher in the AKI group compared to the CKD group, while serum NGAL (sNGAL) levels were not significantly different. Furthermore, the u/s NGAL ratio and fractional excretion of NGAL (Fe NGAL) were able to effectively distinguish AKI from CKD. Further confirmatory studies are still necessary.

Summary: This study aimed to assess the early predictive values of hepcidin, NGAL, and their combination for secondary AKI in patients with sepsis. Results indicated that higher serum hepcidin and urine NGAL levels at admission were independent predictors of AKI development in critically septic patients. Serum hepcidin and urine NGAL were comparable predictive indicators of AKI occurrence, with their combination showing improved accuracy in prediction.

Summary: This study found that plasma calprotectin and NGAL levels were significantly higher in ACS patients with AKI compared to those without AKI. These two biomarkers could early discriminate AKI in ACS patients and a combination of them showed even better diagnostic performance.

Summary: This study compared the predictive performance of different biomarkers for acute kidney injury (AKI). The biomarkers containing NGAL showed the best predictive accuracy for the occurrence of AKI, especially in medically treated patients. However, the predictive performance of urinary NGAL was limited in surgical patients, and urinary NGAL/creatinine seemed to be the most accurate biomarkers in these patients. All of the biomarkers had similar predictive performance in critically ill patients.

Summary: This study found that baseline serum NGAL levels in CKD patients can effectively identify a high-risk population for CI-AKI following PCI, and NGAL levels performed better than eGFR in predicting CI-AKI. NGAL levels greater than 127 ng/mL were independently associated with CI-AKI.

Summary: This study investigated the usefulness of urinary neutrophil gelatinase-associated lipocalin (uNGAL) in predicting the short- and long-term effects of tolvaptan (TVP) and the incidence of acute kidney injury (AKI) post-TVP administration. The results showed that uNGAL can be a useful predictor of the short- and long-term efficacy of TVP and can be used to predict the occurrence of AKI after TVP administration.

Summary: Elevated NGAL levels have prognostic utility for renal and clinical outcomes among STEMI patients undergoing primary PCI, independent of traditional inflammatory markers. High NGAL is associated with increased short-term adverse outcomes, such as acute kidney injury, lower left ventricular ejection fraction, higher 30-day mortality, and higher incidence of MACE.

Summary: This article reviews the literature on commonly evaluated biomarkers of acute kidney injury (AKI) in the last 2 years and highlights the findings of these biomarkers. Among several studied AKI biomarkers, urine neutrophil gelatinase-associated lipocalin (NGAL) and the combination of urine tissue inhibitor of metalloproteinases-2 (TIMP-2) and insulin-like growth factor binding protein 7 (IGFBP7) have been most frequently studied as diagnostic biomarkers of AKI and for AKI risk stratification. Urine NGAL continues to show good discriminative value in predicting and diagnosing AKI in children, while urine TIMP-2*IGFBP7 provides modest improvement to clinical models of AKI.

Summary: Circulating levels of NGAL are associated with acute kidney injury, severity and progression of CKD. This study investigated NGAL as a potential biomarker for new-onset CKD and found that higher plasma NGAL concentrations are associated with an increased risk of developing CKD in the general population, mainly driven by renal function decline.

Summary: This study aimed to evaluate urinary NGAL as a predictor of changes in kidney transplant function, finding that high urinary NGAL levels may indicate graft dysfunction or urinary tract infections in clinically stable kidney transplant recipients beyond the first year post-transplantation.

Summary: This study assessed the diagnostic value of NGAL in early diagnosis of AKI among patients with suspected sepsis through a meta-analysis. The results showed that urine NGAL outperformed serum and plasma NGAL in diagnosing AKI. However, further research is needed to clarify the controversial issue regarding the utility of serum and plasma NGAL.

Summary: The high prevalence of inadequate hydration is concerning and may have negative health impacts. This study found that prolonged mild hypohydration increased a urinary biomarker associated with kidney injury. Urine osmolality and urine specific gravity showed high accuracy in identifying individuals at risk of acute kidney injury.

Summary: This study compared the diagnostic accuracy of urine neutrophil gelatinase-associated lipocalin (UNGAL) and renal resistive index (RRI) in predicting the type of acute kidney injury (AKI) in decompensated cirrhosis (DC) patients. The results showed that UNGAL has an excellent diagnostic accuracy in predicting ATN-AKI.
