Background Subjects with Prader-Willi syndrome (PWS) have a higher fat mass and a lower fat-free mass compared to subjects with essential obesity. However, few data are presently available on the segmental body composition (BC) of PWS subjects. Aim To evaluate whether women with PWS and women with essential obesity, matched for age and percent body fat, differ in segmental fat distribution and surrogate markers of cardiometabolic disease (CMD). Subjects and methods 35 women with PWS and 50 women with essential obesity were matched for age and percent body fat using coarsened exact matching. BC was measured by dual-energy X-ray absorptiometry. Oral glucose tolerance testing and measurements of cholesterol, triglycerides, C-reactive protein, and blood pressure were performed. Comparisons between PWS and obese women were performed using generalized linear models. Results Trunk fat was lower in PWS than in obese women on both absolute [-7.3 (95 % confidence interval -9.4 to -5.2) kg] and relative [-4.1 (-6.9 to -1.4) % of body fat] grounds. PWS and obese women had similar surrogate markers of CMD, with the exception of HDL-cholesterol, which was higher in PWS women. Conclusion Trunk fat is lower in obese women with PWS than in those with essential obesity. Surrogate markers of CMD are, however, mostly similar in the two groups.

Summary: Metabolomics can provide valuable information about the biochemical mechanisms underlying the benefits of a body weight reduction program (BWRP). In this study, a targeted metabolomic approach was used to analyze the metabolomic profile of obese adolescents before and after a 3-week BWRP. The results showed that the BWRP significantly improved body composition, glucometabolic homeostasis, and cardiovascular function. A total of 64 metabolites were reduced after the intervention, while three metabolites were increased. Overall, the study highlights the effectiveness of a short-term BWRP in changing the plasma metabolomic profile. Rating: 9.5/10.

Respiratory Muscle Interval Training Improves Exercise Capacity in Obese Adolescents during a 3-Week In-Hospital Multidisciplinary Body Weight Reduction Program

Desy Salvadego, Gabriella Tringali, Roberta De Micheli, Alessandro Sartorio

Summary: The purpose of this study was to determine if interval training targeted to the respiratory muscles in addition to a weight reduction program could improve exercise response in young obese patients. The results showed that both the RMIT group and the control group experienced similar weight loss, and both groups had increased peak pulmonary oxygen uptake. However, only the RMIT group showed improvements in cardiorespiratory burden and perceived respiratory discomfort during heavy-intensity exercise.

Long-term effects of GH therapy in adult patients with Prader-Willi syndrome: a longitudinal study

Graziano Grugni, Alessandro Sartorio, Davide Soranna, Antonella Zambon, Lucia Grugni, Giuseppe Zampino, Antonino Crino

Summary: PWS is a complex disorder caused by the failure of paternal allele expression on chromosome 15. This study found that long-term GH treatment can improve body fat distribution and body composition in PWS patients. However, the increase in glucose values during GH therapy should be considered, and continuous surveillance of glucose metabolism is mandatory during long-term GH therapy, especially in obese patients.

Cardiometabolic Index (CMI) and Visceral Adiposity Index (VAI) Highlight a Higher Risk of Metabolic Syndrome in Women with Severe Obesity

Stefano Lazzer, Mattia D'Alleva, Miriam Isola, Maria De Martino, Diana Caroli, Adele Bondesan, Alice Marra, Alessandro Sartorio

Summary: Recent evidence suggests that simple and inexpensive anthropometric measurements can help identify obese women at higher risk for developing metabolic syndrome. This study compared the accuracy of different adiposity and body composition indexes in identifying metabolic syndrome in a group of 876 women. The results showed that VAI and CMI performed the best in predicting metabolic syndrome, and these indexes were also the most sensitive and specific. This study highlights the usefulness of VAI and CMI in clinical practice for detecting metabolic syndrome in obese women.

Summary: Little is known about the changes in organs and tissues that may make elder patients more vulnerable to acute stressors such as SARS-CoV-2 infection. In this study of elderly patients with SARS-CoV-2 infection, the association between vascular calcifications, bone density, muscle density, and the odds of dying was evaluated. Age was found to be the strongest predictor of body composition and death, overshadowing the relationships between these factors.
