Summary: The proportion of people suffering from age-related diseases is increasing worldwide. Recent research has shown that low protein diets, particularly those low in branched-chain amino acids (BCAAs), can reduce mortality and promote metabolic health and extended lifespan in both humans and rodents. This article discusses the emerging evidence that BCAAs play a crucial role in healthy metabolism and longevity, and explores the physiological and molecular mechanisms behind the benefits of restricting dietary BCAAs. The findings suggest that the quality of protein, specifically the composition of dietary protein, may be a previously overlooked factor in metabolic dysfunction and reducing dietary BCAAs could be a promising new approach to delaying and preventing age-related diseases.

Summary: Fibrosis, characterized by excessive production of extracellular matrix, is a common pathological feature of organ diseases. Metabolic alterations, especially in the regulation of branched-chain amino acids (BCAAs), have been found to play significant roles in fibrogenesis. Dysregulation of BCAAs metabolism has been implicated in various diseases. This review summarizes the metabolic regulation of fibrosis, changes in BCAAs metabolism associated with fibrosis, and the effects and mechanisms of BCAAs intervention in hepatic, renal, and cardiac fibrosis, with a focus on liver fibrosis and associated hepatocellular carcinoma.

Summary: Immunometabolism, especially in terms of augmenting anti-tumor immune responses, has been extensively researched. Regulatory T cells (Tregs) are known for their immunosuppressive roles in anti-tumor immune responses. Targeting Tregs specifically in the tumor microenvironment (TME) is crucial for successful and specific treatment. Recent studies have shown an association between Tregs and isoleucine, which may play a major role in affecting Tregs. Targeting the immunometabolic pathways of Tregs could be a rational approach for treatment.

Summary: Numerous studies have investigated the use of branched-chain amino acid (BCAA) supplements in patients with cirrhosis and liver cancer, with reported benefits including improved survival rates and hepatic functional reserve. However, the potential role of BCAA supplementation in conjunction with locoregional therapies for liver cancer remains unclear. A systematic review and meta-analysis of articles investigating BCAA supplementation in patients with hepatocellular carcinoma undergoing interventional radiology procedures revealed significantly higher post-treatment albumin levels, suggesting potential benefits in combination with locoregional treatments. However, current data are insufficient to support additional benefits.

Summary: Hajdu-Cheney syndrome (HCS) is a rare inherited skeletal disorder caused by mutations in the NOTCH2 gene. Current treatment options for HCS are limited, and previous reports have shown limited efficacy with other medications. This case report presents the first evidence of romosozumab treatment for HCS in Korea, showing significant improvements in bone mineral density and reduced markers of bone turnover. These findings suggest that romosozumab may be a promising treatment option for HCS patients with osteoporosis.

Summary: This study aimed to identify protein signatures for the diagnosis of parathyroid carcinoma (PC) through quantitative proteomic analyses. It identified 5 proteins that can discriminate PC from parathyroid adenoma (PA) and found that the nuclear percentage of carbonic anhydrase 4 and laminin subunit beta-2 was significantly lower in PC tissue. The findings of this study can contribute to the accurate diagnosis of PC and the discovery of potential therapeutic targets.

Summary: Middle-aged men living with human immunodeficiency virus (MLWH) have a significantly higher risk of hip fracture, almost a decade earlier than those without infection. Limited data on cortical and trabecular bone deficit in MLWH were observed. Summary: This study examined the risk of dementia in patients with primary aldosteronism (PA) compared to patients with essential hypertension (EH). Summary: Genomic and transcriptomic profiling of parathyroid tissues revealed multiple two-hit mutation patterns in the CDC73 gene, suggesting potential therapeutic targets. Transcriptomic analysis identified upregulated expression of E2F targets, KRAS and TNF-alpha signaling, and epithelial-mesenchymal transition pathways in parathyroid carcinomas. A molecular classification model based on carcinoma-specific genes could effectively distinguish between carcinomas, adenomas, and normal tissues, with potential clinical utility.
