Mental health status and risk of new cardiovascular events or death in patients with myocardial infarction: a population-based cohort study

Objective: To examine the association between mental health status after first-time myocardial infarction (MI) and new cardiovascular events or death, taking into account depression and anxiety as well as clinical, sociodemographic and behavioural risk factors. Design: Population-based cohort study based on questionnaires and nationwide registries. Mental health status was assessed 3 months after MI using the Mental Component Summary score from the Short-Form 12 V.2. Setting: Central Denmark Region. Participants: All patients hospitalised with first-time MI from 1 January 2009 through 31 December 2009 (n=880). The participants were categorised in quartiles according to the level of mental health status (first quartile=lowest mental health status). Main outcome measures: Composite endpoint of new cardiovascular events (MI, heart failure, stroke/transient ischaemic attack) and all-cause mortality. Results: During 1940 person-years of follow-up, 277 persons experienced a new cardiovascular event or died. The cumulative incidence following 3 years after MI increased consistently with decreasing mental health status and was 15% (95% CI 10.8% to 20.5%) for persons in the fourth quartile, 29.1% (23.5% to 35.6%) in the third quartile, 37.0% (30.9% to 43.9%) in the second quartile, and 47.5% (40.9% to 54.5%) in the first quartile. The HRs were high, even after adjustments for age, sociodemographic characteristics, cardiac disease severity, comorbidity, secondary prophylactic medication, smoking status, physical activity, depression and anxiety (HR3rd quartile 1.90 (95% CI 1.23 to 2.93), HR2nd quartile 2.14 (1.37 to 3.33), HR1st quartile 2.23 (1.35 to 3.68) when using the fourth quartile as reference). Conclusions: Low mental health status following first-time MI was independently associated with an increased risk of new cardiovascular events or death. Further research is needed to disentangle the pathways that link mental health status following MI to prognosis and to identify interventions that can improve mental health status and prognosis.

Functional somatic symptoms in youths in general practice: A cross-sectional study on prevalence, clinical management and perceived burden

Rasch analysis of the structural validity of the anxiety symptom scale in the lolland-falster health study

Summary: This study assessed the psychometric properties of the ten-item Anxiety Symptom Scale (ASS) using Rasch analysis. The results showed that, after modifying the scoring system, the ASS demonstrated effective measurement of anxiety symptoms. The study found no significant differential item functioning for sex, age group, and educational level. However, the total ASS was poorly targeted for the overall sample, indicating low reliability, and may be more suitable for use in clinical samples.

Cancer risk in persons with new-onset anaemia: a population-based cohort study in Denmark

Cross-cultural validity of the WHO-5 Well-Being Index and Euthymia Scale: A clinimetric analysis

Treatment indications and potential off-label use of antidepressants among older adults: A population-based descriptive study in Denmark

Summary: This study aimed to investigate the potential off-label use of antidepressants among adults aged 65 and older. The findings showed that nearly one-third of antidepressant prescriptions redeemed by older adults in Denmark had missing or unspecified treatment indications. Prevalence of anxiety disorders and association to socioeconomic position. Results from the Lolland Falster Health Study

Antidepressant treatment initiation and public sick-leave compensation in the following year: a register-based prospective cohort study in Denmark

Summary: This study investigated the association between the initiation of antidepressant treatment and public sick leave compensation (PLSC) in Denmark. The results showed that mirtazapine and mianserin were associated with the highest risk of PLSC in the year following treatment initiation, while citalopram, venlafaxine, and duloxetine were associated with increased PLSC. In contrast, paroxetine, fluoxetine, and vortioxetine were associated with a significantly lower risk of PLSC.

The Concept of Psychological Distress and Its Assessment: A Clinimetric Analysis of the SCL-90-R

Summary: Many studies have been conducted to investigate psychological distress, but there is still debate on how to define and measure this phenomenon. Clinimetrics, the science of clinical measurements, may offer insights on conceptualizing and assessing psychological distress. A study used Rasch analysis to validate the Symptom Checklist-90-Revised (SCL-90-R) as a measure of underlying dimensions of psychological distress. The findings suggest that the SCL-90-R total score doesn't fit the Rasch model but is reliable, and the subscales can assess specific symptoms of psychological distress.

Concurrent validity of the anxiety symptom-scale compared to two well-being scales: results from the Lolland-Falster health study

Summary: This study aimed to validate the Anxiety Symptom-scale against two well-being scales and identify cut-off points for the sum score. The results showed high concordance between the Anxiety Symptom-scale and the Cantril Ladder and WHO-5. A sum score of 3-4 on the scale indicated low well-being with a sensitivity of 0.85-0.99. The Anxiety Symptom-scale is considered a sensitive and valid tool for identifying anxiety symptoms in patients with low well-being.

Variation in Psychometric Testing in General Practice-A Nationwide Cohort Study

Summary: This study aimed to assess the use of psychometric tests in Danish general practice and estimate their impact on further treatment and death by suicide. The study found considerable variation in the use of psychometric tests among general practices, with a positive association between test use and talk therapy. The use of psychometric tests was also associated with prescriptions for anxiolytics, antidementia drugs, and antidepressants.

Do sociodemographic and clinical factors affect the selection of initial antidepressant treatment for depression in older adults? Results from a nationwide descriptive study in Denmark

Summary: This study investigated the first-choice antidepressant selection for older adults in Denmark. The study found that over two-thirds of older adults chose alternative antidepressants, primarily escitalopram/citalopram or mirtazapine, instead of the nationally recommended sertraline. Socially disadvantaged older adults and clinically vulnerable older adults were more likely to use alternative antidepressants.

Comparing the construct validity of the Patient Health Questionnaire (PHQ-9) and the Major Depression Inventory (MDI) using Rasch analysis
