The use of medications directed against vascular endothelial growth factor (VEGF) signaling has revolutionized the treatment of age-related macular degeneration (AMD) and many other retinal diseases in the last decade. However, the rapidly increasing use of these agents has led to a rise in treatment-associated complications. One of the most feared by patients and ophthalmologists is post-injection endophthalmitis, which can result in severe vision loss and, in rare cases, loss of the eye. The aim of this article is to review the incidence, clinical findings, risk factors, management, and visual outcomes in cases of endophthalmitis following intravitreal injections of anti-VEGF medications.

An Examination of Positive and Negative Dementia Caregiving Experiences

Summary: This study examined the associations among caregiver experiences, including positive aspects of caring, caregiver burden, and mutuality, in dyads involving persons with dementia and their informal caregivers. The results showed that need characteristics were most frequently associated with the caregiving experience. Bivariate correlations between positive aspects of caring, caregiver burden, and mutuality were moderate to strong. Assessment of both positive and negative aspects of caregiving, as well as attention to depression and interventions targeting depressive symptoms, may be important in reducing caregiver burden and enhancing positive aspects of caregiving.

Facility-Level Variation in Reported Statin-Associated Side Effects Among Patients with Atherosclerotic Cardiovascular Disease-Perspective from the Veterans Affair Healthcare System

Summary: Significant variation in standardized reporting of statin-associated side effects (SASEs) was found across medical facilities within the Veterans Affairs healthcare system, which may affect optimal statin utilization among patients with atherosclerotic cardiovascular disease. The study of over 1.2 million patients revealed that 13.7% had at least one SASE reported, with a median facility rate of 140.5 cases per 1000 ASCVD patients. Hierarchical regression analysis showed a median rate ratio of 1.56 in the adjusted model, suggesting room for improvement in the documentation of SASEs among medical facilities.

Optic neuropathy after COVID-19 vaccination: a report of two cases

Summary: This report describes two cases of optic nerve pathology after COVID-19 vaccination and discusses the management implications for post-vaccination central nervous system (CNS) inflammation. The cases involve a 69-year-old woman and a 32-year-old woman who experienced optic nerve issues after receiving the Pfizer-BioNTech and AstraZeneca-Oxford vaccines, respectively. Both patients showed improvement and recovery following treatment.

Optical Coherence Tomography Angiography Biomarkers Predict Anatomical Response to Bevacizumab in Diabetic Macular Edema

Summary: This study aimed to identify biomarkers that can predict an early anatomical response to intravitreal bevacizumab (IVB) treatment in diabetic macular edema (DME) using optical coherence tomography angiography (OCTA). The study found that eyes with an early anatomical response had worse baseline visual acuity, larger central macular thickness, smaller foveal avascular zones, and higher vascular density in the foveal superficial and deep capillary plexuses compared to non-responding eyes. Starting central macular thickness, foveal avascular zone size, and vascular density were found to be predictive of early treatment response to IVB.

An initiative to improve follow-up of patients with diabetic retinopathy

Summary: This study successfully identified diabetic retinopathy patients lost to follow-up using an electronic medical record system and re-engaged them through provider intervention. Younger age, poorly controlled blood sugar and cholesterol, fewer ophthalmology appointments, and lack of retinal imaging were associated with loss to follow-up. Provider intervention and re-engagement significantly reduced the rate of loss to follow-up.

Multimodal image alignment aids in the evaluation and monitoring of sector retinitis pigmentosa

Summary: This study presents a semi-automated method of image alignment to monitor the progression of inherited retinal degenerations. The results show the case of a 22-year-old woman with inherited retinal degeneration, including the findings from fundus examination and multimodal imaging. Effectiveness and Financial Viability of Telehealth Physician Extenders for Re-Engagement of Patients with Diabetic Retinopathy

Summary: The study aims to evaluate the effectiveness and financial implications of employing a telehealth physician extender program in re-engaging lost-to-follow-up patients with diabetic retinopathy (DR). Randomized patients were assigned to a recall intervention or standard operating procedure (SOP), and the intervention group demonstrated higher rates of appointment scheduling and adherence. The use of a telehealth physician extender improves patient re-engagement at a reasonable cost.

Consequences of Real-World Surveillance of Fellow Eyes in Neovascular Age-Related Macular Degeneration

Summary: This study examined the relationship between the interval of monitoring the at-risk eyes of patients with unilateral neovascular age-related macular degeneration (nAMD) and the severity of the disease at diagnosis. The study compared the visual acuity and central macular thickness of patients who were actively receiving anti-VEGF treatment at the time of second eye diagnosis with patients who had discontinued treatment in their first eye. The results showed that the at-risk eyes of patients who had stopped treatment prior to fellow eye conversion were monitored less frequently than those who continued treatment. However, the visual acuity and central macular thickness were similar at the time of fellow eye diagnosis for both groups.

Second eyes to develop neovascular age-related macular degeneration have fewer symptoms and better one-year visual outcomes

Summary: This study compared visual and anatomical outcomes for patients with sequential neovascular age-related macular degeneration (nAMD), both at diagnosis and one year after treatment. Results showed that second eyes had better visual acuity and smaller pigment epithelial detachment (PED) heights at diagnosis and at one year after treatment compared to first eyes. This may be due to earlier diagnosis enabled by monitoring.

Assessing optometric care delivered by telehealth during the COVID-19 public health emergency
