A novel signature for stratifying the molecular heterogeneity of the tissue-infiltrating T-cell receptor repertoire reflects gastric cancer prognosis

Many basic properties of the T-cell receptor (TCR) repertoire require clarification, and the changes occurring in the TCR repertoire during carcinogenesis, especially during precancerous stages, remain unclear. This study used deep sequencing analyses to examine 41 gastric tissue samples at different pathological stages, including low-grade intraepithelial neoplasia, high-grade intraepithelial neoplasia, early gastric cancer and matched adjacent tissues, to define the characteristics of the infiltrating TCR beta repertoire during gastric carcinogenesis. Moreover, to illustrate the relationship between the local molecular phenotype and TCR repertoire of the microenvironment, whole-genome gene expression microarray analysis of the corresponding gastric precancerous lesions and early gastric cancer tissues was conducted. Our results showed that the degree of variation in the TCR repertoire gradually increased during tumourigenesis. Integrative analysis of microarray data and the TCR repertoire variation index using the network-based Clique Percolation Method identified an 11-gene module related to the inflammatory response that can predict the overall survival of gastric cancer (GC) patients. In conclusion, our results revealed the multistage heterogeneity of tissue-infiltrating TCR repertoire during carcinogenesis. We report a novel way for identifying prognostic biomarkers for GC patients and improves our understanding of immune responses during gastric carcinogenesis.

Ethanol ablation is a promising non-surgical option for tumor shrinking, but there is insufficient evidence for its impact on symptom control and long-term efficacy.

MSHT: Multi-Stage Hybrid Transformer for the ROSE Image Analysis of Pancreatic Cancer

Summary: Pancreatic cancer is a highly lethal malignant tumor, but its diagnosis can be accelerated by using the rapid on-site evaluation (ROSE) technique. However, the shortage of experienced pathologists has hindered the wider use of ROSE diagnosis. Deep learning shows potential in automatically classifying ROSE images, but modeling the complex local and global image features is challenging. Therefore, a multi-stage hybrid Transformer (MSHT) is proposed to combine the advantages of both traditional convolutional neural networks (CNN) and Transformer structures, achieving improved accuracy and attention regions.

Distinct lipid profile in haemolytic anaemia-related gallstones compared with the general gallstone

Summary: This study described the clinical characteristics of gallstones following chronic haemolytic anaemia and compared them with the general gallstone population. The lipid profile of haemolytic anaemia patients with gallstones was distinct, with low total cholesterol and high-density lipoprotein, and increased or normal low-density lipoprotein. Elderly patients were more likely to develop gallstones in a shorter period after anaemia, and abdominal ultrasound and frequent follow-up visits were recommended for chronic haemolytic anaemia patients older than 50 years old.

Summary: This randomized controlled trial (RCT) aims to establish a state-of-the-art protocol for quantifying the multiroute exposure of medical staff to infectious particles exhaled by patients during real oesophagogastroduodenoscopy (OGD) procedures. The study will use an anthropomorphic model to simulate a medical staff member and will assess the dosage of exhaled infectious particles in the airway and facial mucosa of the model with or without a surgical mask. The results of this study will be disseminated through presentations and publications in peer-reviewed journals.

Mitochondria in innate immunity signaling and its therapeutic implications in autoimmune diseases

Summary: Autoimmune diseases involve complex immune response alterations, with the exact pathogenesis remaining unclear. Recent research has focused on the functions of mitochondria, which play crucial roles in cell differentiation, maturation, and death. This review summarizes the role of mitochondria in the innate immune response and its dysfunction in autoimmune diseases such as rheumatoid arthritis and systemic lupus erythematosus, with a focus on its impact on cellular metabolism and regulation of immune signaling pathways. Additionally, potential therapeutic targets in mitochondrial regulation for autoimmune diseases are discussed, highlighting the need for further research.

A Chinese prospective multicenter cohort study evaluating EUS-guided drainage of pancreatic fluid collections using the Hot AXIOS system

The study found that a previous pancreatitis attack > 6 months prior was associated with a greater chance of achieving 100% remission of PFCs within 1 month.

Gastroblastoma Treated by Endoscopic Submucosal Excavation with a Novel PTCH1::GLI2 Fusion: A Rare Case Report and Literature Review

Summary: Gastroblastoma is an extremely rare stomach tumor that primarily occurs in adolescents and young adults, with a biphasic cell morphology of epithelioid and spindle cells. Most patients survive after surgical treatment with no evidence of disease. A diagnosis of gastroblastoma should be considered in cases with characteristic histopathology and immunohistochemistry patterns, even without a MALAT1-GLI1 fusion.

Endoscopic characteristics in predicting prognosis of biopsy-diagnosed gastric low-grade intraepithelial neoplasia

Summary: This study aimed to evaluate predictive factors to identify malignancies misdiagnosed as low-grade intraepithelial neoplasia (LGIN) by biopsy and LGIN at high risk of progression. The study found that a single lesion, a large lesion size, and marked intestinal metaplasia (IM) were independent predictors of initially misdiagnosed malignancies; a whitish appearance, irregular margins, marked IM, and a histological diagnosis of LGIN more than twice within the first year were predictors for progression.
