Liver disease is one of the main contributors to the increased levels of morbidity and mortality seen in the HIV-1-infected, ART-treated population. Circulating miRNAs, particularly those located inside extracellular vesicles, are seen as promising biomarkers for a number of human disease conditions, including liver-related diseases. Here, we show that serum levels of miR-122 and miR-200a are greater in HIV/HCV co-infected individuals compared to HIV-1 mono-infected individuals. We also show that miR-122 and miR-200a are elevated in ART-treated, HIV-1-infected individuals prior to the development of fatal liver disease, suggesting that these miRNA may have some potential clinical utility as biomarkers. While this study is hypothesis generating, it shows clearly that both miR-122 and miR-200a are promising novel biomarkers for liver disease in the ART-treated, HIV-1-infected population.

Summary: This study presents a multi-stage clinical trial protocol for treating COVID-19 inpatients globally, aiming to evaluate the safety and efficacy of various novel antiviral drugs. Through multi-network partnerships, the protocol successfully achieved rapid assessment of candidate drugs and the generation of efficacy data. Implementation faced challenges such as ensuring drug supply, reliable recruitment, and regulatory approvals across global sites.

Early prediction of decompensation (EPOD) score: Non-invasive determination of cirrhosis decompensation risk

Annika R. P. Schneider, Carolin Schneider, Kai Markus Schneider, Vanessa Baier, Steffen Schaper, Christian Diedrich, Katrin Coboeken, Hannah Mayer, Wenyi Gu, Jonel Trebicka, Lars M. Blank, Rolf Burghaus, Joerg Lippert, Daniel J. Rader, Christoph A. Thaiss, Jan-Frederik Schlender, Christian Trautwein, Lars Kuepfer

Summary: The study aims to develop a prognostic score for predicting decompensation risk in patients with compensated cirrhosis. The EPOD score, based on platelet count, albumin, and bilirubin concentration, outperforms existing cirrhosis scores and accurately predicts the probability of decompensation within a 3-year follow-up.

TIPS for the management of stomal variceal bleeding due to cirrhotic and non-cirrhotic portal hypertension

Summary: In this case series, patients with recurrent stomal bleedings caused by parastomal varices due to portal hypertension were successfully managed with TIPS implantation, leading to sustainable hemostasis in the majority of cases. Two patients experienced bleeding relapse due to TIPS dysfunction, but sustainable cessation of bleeding was achieved after TIPS revision with coil embolization of the varices.

Closing spontaneous portosystemic shunts in cirrhosis: Does it make sense? Does it work? What does it take?

Wim Laleman, Michael Praktiknjo, Mette Munk Lauridsen, Lawrence Bonne, Emma Vanderschueren, Chris Verslype, Aleksander Krag, Jonel Trebicka, Geert Maleux

Summary: Spontaneous portosystemic shunts are often overlooked as a cause of hepatic encephalopathy associated with cirrhosis. They are considered as radiological biomarkers of clinically significant portal hypertension and have a clinical impact on complications related to portal hypertension. Interventional radiology embolization is a direct method to target these shunts.

Colon Cancer Microbiome Landscaping: Differences in Right- and Left-Sided Colon Cancer and a Tumor Microbiome-Ileal Microbiome Association

Summary: In the current era of precision oncology, it is recognized that colorectal cancer (CRC) is a heterogeneous disease. The tumor location is crucial in determining disease progression and prognosis. The microbiome has been identified as an important factor in CRC, but previous studies have shown inconsistent results due to the heterogeneous nature of microbiomes and the lack of separate analysis of colon cancer (CC) and rectal cancer (RC). Our prospective study aimed to map the colon cancer landscape using 16S rRNA amplicon sequencing, and we found distinct microbiomes in right- and left-sided colon cancers, as well as a significant increase in potentially pathogenic bacteria in stool samples after surgery.

Endoscopic Advances in Hepatology

Emma Vanderschueren, Jonel Trebicka, Wim Laleman

Summary: Endoscopy is an indispensable tool in diagnosing and managing liver disease and its complications. It has become an alternative route for interventions and is increasingly being used as a first-line choice. Endoscopic ultrasound is valuable for evaluating liver parenchyma, lesions, and surrounding tissues, as well as guiding portal pressure measurement and managing complications of portal hypertension. Hepatologists should be aware of the full spectrum of diagnostic and therapeutic tools available in this field. Future directions for endoscopy in hepatology are also discussed in this comprehensive review.

A Novel Insertion in the Hepatitis B Virus Surface Protein Leading to Hyperglycosylation Causes Diagnostic and Immune Escape

Felix Lehmann, Heiko Slanina, Martin Roderfeld, Elke Roeb, Jonel Trebicka, John Ziebuhr, Wolfram H. Gerlich, Christian G. Schuettler, Bernhard Schlevogt, Dieter Glebe

Summary: Chronic hepatitis B virus (HBV) infection is a global health threat. Mutations in the surface antigen of HBV (HBsAg) may alter its antigenicity, infectivity, and transmissibility. A patient positive for HBV DNA and detectable but low-level HBsAg in parallel with anti-HBs suggested the presence of immune and/or diagnostic escape variants. Secreted HBsAg was also subjected to four widely used, state-of-the-art diagnostic assays, which all failed to detect the hyperglycosylated insertion variant. Additionally, the recognition of mutant HBsAg by vaccine- and natural infection-induced anti-HBs antibodies was severely impaired.

Limited access to liver transplantation and TIPS despite high mortality, healthcare resource use and costs of cirrhosis in Germany

Christian M. M. Lange, Jonel Trebicka, Alexander Gerbes, Ali Canbay, Andreas Geier, Uta Merle, Markus Peck-Radosavljevic, Frank Tacke, Tobias Vogelmann, Sina Theis, Hartmut Heinze, Alexander Zipprich

Summary: Limited data is available on patients with cirrhosis in Germany. This study aimed to estimate the prevalence, comorbidities, mortality, healthcare resource utilization, and costs of these patients, as well as the incidence of decompensation of cirrhosis in Germany. The study found that the prevalence of cirrhosis in 2015 was 250/100,000, with an estimated 201,747 patients in the German population. Among patients with compensated cirrhosis, 16.0% developed decompensation within 3 years. Patients with decompensated cirrhosis had a higher burden of comorbidities and higher costs compared to patients with compensated cirrhosis. The 1-year mortality rate after decompensation was significantly higher than in patients with compensated cirrhosis. Only a small percentage of patients with decompensated cirrhosis received liver transplantation or transjugular intrahepatic portosystemic shunts (TIPS). Overall, patients with cirrhosis in Germany face a high healthcare burden, particularly in the decompensated stage.
