Summary: The study aimed to evaluate the changes in circulating microRNA (c-miR) in the blood of liver transplant recipients before and after transplantation, and to correlate their levels with gold standard biomarkers and outcomes such as rejection or complications. The results showed that c-miR changes were associated with follow-up time, and miR-122-5p, miR-92a-3p, and miR-18a-5p exhibited the same trend after transplantation, with an increase in their levels in patients with complications. Compared to changes in standard haemato-biochemical parameters, the importance of c-miRs as potential noninvasive biomarkers for monitoring patient outcomes was confirmed.

Summary: This study found that inhibiting miR-122-5p can activate p53 and hinder the progression of the MVA pathway in non-small cell lung cancer (NSCLC). This leads to a decrease in cell proliferation and migration, and an increase in apoptosis. miR-122-5p interacts with p53 and is negatively correlated with its expression. Thus, miR-122-5p regulates NSCLC by targeting p53, offering potential molecular targets for targeted drug development.

Summary: This study investigated the plasma levels of vascular endothelial growth factor (VEGF) in children and adolescents with major depressive disorder (MDD) and examined the impact of antidepressant treatment on VEGF levels. The results showed that the levels of VEGF were significantly increased in depressed individuals and were decreased after a short course of antidepressant treatment. Additionally, certain microRNAs were found to be associated with the regulation of VEGF. This study supports the use of VEGF as a biomarker for MDD in children and adolescents and highlights the importance of understanding the underlying regulatory mechanisms.

Summary: This study aimed to identify common targets of miR-223-3p, miR-122-5p, and miR-93-5p in PCOS and assess their transcript levels in PCOS rat ovaries. Transcriptome profiles of granulosa cells from PCOS patients were analyzed to identify DEGs, followed by miRNA target prediction and functional analysis. The study identified several potential genes, including JMJD1C, PLCG2, SMAD3, FOSL2, TGFB1, TRIB1, GAS7, TRIM25, NFYA, and CALCRL, which may play a role in the development of PCOS. These findings contribute to the identification of potential biomarkers for the prevention and treatment of PCOS in the future.

Summary: In this case series, patients with recurrent stomal bleedings caused by parastomal varices due to portal hypertension were successfully managed with TIPS implantation, leading to sustainable hemostasis in the majority of cases. Two patients experienced bleeding relapse due to TIPS dysfunction, but sustainable cessation of bleeding was achieved after TIPS revision with coil embolization of the varices.

Summary: In the current era of precision oncology, it is recognized that colorectal cancer (CRC) is a heterogeneous disease. The tumor location is crucial in determining disease progression and prognosis. The microbiome has been identified as an important factor in CRC, but previous studies have shown inconsistent results due to the heterogeneous nature of microbiomes and the lack of separate analysis of colon cancer (CC) and rectal cancer (RC). Our prospective study aimed to map the colon cancer landscape using 16S rRNA amplicon sequencing, and we found distinct microbiomes in right- and left-sided colon cancers, as well as a significant increase in potentially pathogenic bacteria in stool samples after surgery.

Summary: Endoscopy is an indispensable tool in diagnosing and managing liver disease and its complications. It has become an alternative route for interventions and is increasingly being used as a first-line choice. Endoscopic ultrasound is valuable for evaluating liver parenchyma, lesions, and surrounding tissues, as well as guiding portal pressure measurement and managing complications of portal hypertension. Hepatologists should be aware of the full spectrum of diagnostic and therapeutic tools available in this field. Future directions for endoscopy in hepatology are also discussed in this comprehensive review.

Summary: Chronic hepatitis B virus (HBV) infection is a global health threat. Mutations in the surface antigen of HBV (HBsAg) may alter its antigenicity, infectivity, and transmissibility. A patient positive for HBV DNA and detectable but low-level HBsAg in parallel with anti-HBs suggested the presence of immune and/or diagnostic escape variants. Secreted HBsAg was also subjected to four widely used, state-of-the-art diagnostic assays, which all failed to detect the hyperglycosylated insertion variant. Additionally, the recognition of mutant HBsAg by vaccine- and natural infection-induced anti-HBs antibodies was severely impaired.

Summary: Limited data is available on patients with cirrhosis in Germany. This study aimed to estimate the prevalence, comorbidities, mortality, healthcare resource utilization, and costs of these patients, as well as the incidence of decompensation of cirrhosis in Germany. The study found that the prevalence of cirrhosis in 2015 was 250/100,000, with an estimated 201,747 patients in the German population. Among patients with compensated cirrhosis, 16.0% developed decompensation within 3 years. Patients with decompensated cirrhosis had a higher burden of comorbidities and higher costs compared to patients with compensated cirrhosis. The 1-year mortality rate after decompensation was significantly higher than in patients with compensated cirrhosis. Only a small percentage of patients with decompensated cirrhosis received liver transplantation or transjugular intrahepatic portosystemic shunts (TIPS). Overall, patients with cirrhosis in Germany face a high healthcare burden, particularly in the decompensated stage.
