The role of the placenta in thyroid hormone delivery to the fetus

The transplacental passage of thyroid hormones from the maternal circulation to the fetal circulation within the human hemochorial placenta is important for normal fetal development, particularly the development of the central nervous system. The role of maternal thyroid hormones is particularly important in the first half of pregnancy, before the onset of endogenous thyroid hormone production in the fetus. The human placenta regulates the quantity and composition of different forms of transported thyroid hormones to ensure that the requisite levels are present in the fetus for each stage of development. Transplacental thyroid hormone supply to the fetus is modulated by several factors, including the following proteins: plasma membrane transporters, which regulate the passage of thyroid hormones in and out of cells; iodothyronine deiodinases, which metabolize thyroid hormones; and proteins within trophoblast cells, which bind thyroid hormones. In pathological situations of either maternal or fetal thyroid hormone deficiency during pregnancy, the placenta seems to lack the full compensatory mechanisms necessary to optimize maternal-fetal transfer of thyroid hormones. Inadequate passage of thyroid hormones can lead to suboptimal fetal thyroid hormone levels, which might contribute to the neurodevelopmental delay associated with such conditions. Thus, maintaining normal maternal thyroid hormone status is likely to be the primary factor in ensuring adequate transplacental thyroid hormone passage and appropriate iodide supply to the fetus.

Impact of maternal gestational weight gain in twin pregnancies on early childhood obesity risk: A longitudinal birth cohort study

Summary: This study investigates the impact of gestational weight gain (GWG) on the body mass index-for-age z score (BAZ) and obesity risk among twin offspring. The results show that total GWG is positively correlated with offspring birthweight and BAZ at 6, 12, and 24 months. Excessive total GWG is related to an increased relative risk of large for gestational age (LGA) and overweight at 6 and 12 months. Only the second trimester gestational weight gain rate (GWGR) is positively correlated with birthweight, and is associated with an increased risk of LGA and overweight at 12 months.

Summary: Pre-eclampsia (PE) is a metabolic disorder of the placenta that is caused by defective invasion of trophoblasts. This study investigated the role of AMPK in PE development and found that AMPK activation exacerbates PE manifestations but reduces cell death in the placenta. AMPK activation promotes glucose metabolism and glycogen deposition, enhancing cell viability but compromising trophoblast invasion. In contrast, ablation of AMPK leads to decreased glycogen deposition and structural malformation in the placenta. These findings highlight the importance of balancing invasiveness and viability in trophoblasts for proper placental function.

Evaluating antenatal risk in twin pregnancies-A feasibility study to identify modifiable factors associated with adverse pregnancy outcomes

The results showed that twin pregnancies have different frequencies of exposures compared to singleton pregnancies, highlighting the need for twin-specific studies to identify modifiable factors and develop preventative strategies for morbidity and mortality in twin pregnancies.

Summary: This study examined the impact of a combined myo-inositol, probiotics, and micronutrient nutritional supplement on time-to-natural-conception and clinical pregnancy rates. The results showed that there was no significant difference in time-to-natural-conception and clinical pregnancy rates within a year between women receiving the supplement and those in the control group. Overweight women had a longer time-to-conception, but the supplement may shorten their time-to-conception to that comparable to nonoverweight/nonobese women.

This study identified epigenetic variations in cord blood of discordant MZ twins that are associated with CHD. Improving or preventing a decline in diet quality postpregnancy may improve lipid profile and insulin resistance.

Cord blood adiponectin and leptin concentrations in monochorionic twins with selective intrauterine growth restriction and their associations with childhood growth trajectories

Summary: This study measured the concentrations of adiponectin and leptin in the cord blood of monochorionic twins with selective intrauterine growth restriction (IUGR). The findings showed that cord blood adiponectin concentrations were negatively associated with IUGR, while cord blood leptin concentrations were significantly lower in IUGR twins. Furthermore, adiponectin was negatively associated with height increments in the first 6 months, and leptin was inversely associated with weight and height increments in the first 6 months, as well as weight and height at 6 and 24 months. These results indicate the influence of adiponectin and leptin in cord blood on IUGR and childhood growth trajectories.

Perinatal Plasma Carotenoids and Vitamin E Concentrations with Glycemia and Insulin Resistance in Women during and after Pregnancy

Summary: This study examined the associations between perinatal plasma carotenoids and E vitamers concentrations and glycemia, insulin resistance, and gestational and type 2 diabetes mellitus in GUSTO women. The findings suggest that higher concentrations of alpha- and beta-carotene and lutein may be beneficial for gestational fasting glycemia, while higher concentrations of vitamin E may increase gestational and post-pregnancy glycemia.
