High SARS-CoV-2 Viral Load and Low CCL5 Expression Levels in the Upper Respiratory Tract Are Associated With COVID-19 Severity

Summary: Mucosal immune response in the upper respiratory tract plays a crucial role in controlling SARS-CoV-2 replication, clearing the virus, and affecting the progression of COVID-19. Higher viral load and lower CCL5 expression levels are associated with severe COVID-19, with CCL5 being the best predictor of disease severity.

Summary: The study found that low levels of anti-SARS-CoV-2 S antibodies are associated with higher mortality in critical COVID-19 patients, while higher antibody levels are linked to reduced viral RNA-load in plasma and lower mortality risk.

Summary: The use of corticosteroids in ICU-admitted patients with COVID-19 can reduce 90-day mortality, but early administration may be harmful. Different patient subgroups may have varied efficacy of corticosteroids based on factors such as age, severity of illness, baseline inflammation, and mechanical ventilation.

Summary: This study identified the molecular basis associated with severe lung diffusion involvement in survivors of SARS-CoV-2-induced ARDS using transcriptomic analysis. The findings provide insights into the biological pathways and immune cells related to post-acute pulmonary sequelae, and suggest potential targets for intervention and biomarker development.

Summary: This study evaluates the association between viral RNA load in plasma and host response, complications, and deaths in critically ill patients with COVID-19. The presence of a so-called viral storm is associated with increased all-cause death in patients admitted to the intensive care unit with severe COVID-19.

Digital PCR applications for the diagnosis and management of infection in critical care medicine

Irene Merino, Amanda de la Fuente, Marta Dominguez-Gil, Jose Maria Eiros, Ana P. Tedim, Jesus F. Bermejo-Martin

Summary: Infection is a major cause of morbidity and mortality in critical care medicine. Digital PCR (dPCR) offers technical advantages for the management of infection, such as higher sensitivity, reproducibility, and absolute quantification. This review examines the applications of dPCR in critical care medicine, including microbial gene amplification, quantification of microbial burden, and analysis of host responses.
