Summary: This study explores how stroke survivors' expectations and understanding of Early Supported Discharge (ESD) influenced their engagement with the service. The findings show that stroke survivors were uncertain about what to expect initially, but their experience exceeded expectations and increased their engagement. They valued the goal-oriented approach adopted by the team and the practicality of rehabilitation at home. Psychosocial support played a crucial role in their recovery.

Summary: The purpose of this study was to investigate the interaction between cognitive and physical functions and activities of daily living (ADL) in stroke patients in the rehabilitation ward and their ability to return home. A total of 231 first stroke patients were included in the study, and relevant information such as age, gender, affected side, stroke impairment assessment set, ADL, and discharge destination were collected. The study found that there was a significant interaction between age and stroke-related dysfunction, indicating that stroke patients aged over 76.5 years with a stroke impairment assessment set score of 27 or less at admission may have difficulty returning home.

Summary: This study aimed to explore healthcare professionals' experiences of the development and delivery of Early Supported Discharge for people after stroke, including experiences of the COVID-19 pandemic. The findings identified key issues such as the uncoordinated development of services, staff shortages, and limited utilization of telerehabilitation. The study highlights the importance of collaboration with stroke patients and their families in Early Supported Discharge.

Summary: This study aimed to understand how the adoption of core components of Early Supported Discharge (ESD) drives the delivery of responsive and intensive services in real-world settings. The findings showed that adhering to evidence-based selection criteria, promoting interdisciplinary skillsets, and supporting the role of rehabilitation assistants helped teams manage pressures and deliver services that met evidence-based standards. However, there are still variations in service provision in England and a more integrated and comprehensive stroke service provision is needed. Transferable lessons could be drawn to inform improvement interventions in different settings.

Summary: This study investigated the impact of early supported discharge (ESD) on length of hospital stay for stroke patients, and found that receiving ESD led to a significant increase in hospital stay, particularly in the 2015-2016 analysis with an increase of 15.8%. However, further research is needed to explore the long-term benefits of ESD.

Summary: This study evaluated different stroke early supported discharge (ESD) services in various geographical settings using cost-consequence analysis (CCA). It found that rural services had higher costs per patient, mainly due to staff costs, but these costs were associated with greater adherence to evidence-based ESD service specifications agreed by experts. More resources and costs may be needed for rural services to meet evidence-based criteria.

Summary: The study aims to examine the impact of a comprehensive transitional care model on the utilization of skilled nursing facility (SNF) and inpatient rehabilitation facility (IRF) care for stroke patients in the 12 months following discharge home, as well as identify predictors for SNF or IRF admission after discharge home.

Summary: This systematic review and meta-analysis aimed to identify the physical functioning factors associated with home discharge after inpatient stroke rehabilitation. The review included 45 studies with 204,787 participants and found that higher independence in activities of daily living on admission is associated with home discharge.

Summary: This study aimed to explore the feasibility and efficacy of a body awareness program after stroke, comparing class-based lessons to home-based lessons. The results demonstrated that the class-based group showed improvements in body awareness, quality of life, and motor impairment compared to the home-based group. Qualitative data also indicated a stronger sense of awareness, achievement, and connection in the class-based group.

Summary: The HOME Rehab trial is a national, multicentre, phase III randomized controlled trial in Australia comparing the enhanced occupational therapy discharge planning intervention with standard in-hospital predischarge planning intervention for stroke survivors aged >= 45 years. The trial aims to determine the effectiveness and cost-effectiveness of the HOME programme, with primary outcome being participation and secondary outcomes including hospital readmission, disability, and quality of life. Ethical approval has been obtained for the study, and results will be disseminated through publication in peer-reviewed journals.

Summary: This study tracks the recovery of body awareness after stroke for the first time and investigates its potential relationship with recovery of sensation, motor impairment and function, self-efficacy, and quality of life. The research found that most recovery in sensation and body awareness occurred within the first month post-stroke, and they were correlated with other clinical outcomes, demographics, and stroke-specific clinical characteristics. Further research is needed to continue exploring body awareness and developing effective stroke-specific assessment and intervention strategies.

Summary: This pilot study aims to compare the feasibility and preliminary effects of task-oriented home-exercises in patients in the subacute stage after stroke. The study will use video and paper-based training methods and assess the patient's goals using Goal Attainment Scaling. The primary outcome will be the feasibility of the methods and patient satisfaction with the exercise program.

Summary: The study aimed to develop and validate a decision tree model for stroke patients to determine if they can be discharged within 48 hours post-stroke. The results showed that using the patient's activities of daily living performance within the first 48 hours post-stroke can effectively predict safe discharge.
