Statins use is associated with poorer glycaemic control in a cohort of hypertensive patients with diabetes and without diabetes

Background: The US Federal and Drug Administration (FDA) recently revised statin drug labels to include the information that increases in fasting serum glucose and glycated haemoglobin levels have been reported with the use of statins. Yet in a survey, 87% of the doctors stated that they had never or infrequently observed increases in glucose or HbA1c levels in patients on statin. In this study we would like to determine the association between the use of statins and glycaemic control in a retrospective cohort of patients with hypertension. Methods: A retrospective review of 1060 medical records of patients with hypertension at a primary care clinic was conducted. These records were selected using systematic random sampling (1: 4). Data on patient socio-demographic factors; clinical profile; investigation results and prescribed medications were collected. Independent t-test was used for continuous variables while Pearson's chi 2 test was used for categorical variables. Logistic regression was done to adjust for confounders. Results: 810 (76.4%) patients with hypertension were on statins, out of which 792 (97.8%) were taking simvastatin 10 mg or 20 mg daily. Analysis of the whole group regardless of diabetes status showed that the statin user group had higher HbA1c and fasting blood glucose values. The difference in HbA1c levels remained significant (adjusted OR = 1.290, p = 0.044, 95% CI 1.006, 1.654) after adjustment for diabetes, diabetic medication and fasting blood glucose. In the study population who had diabetes, statin users again had significantly higher HbA1c level compared to statin non-users. This difference remained significant (adjusted OR 1.208, p = 0.037, 95% CI 1.012, 1.441) after adjustment for age and diabetic medications. Conclusions: Statins use is associated with increased HbA1c levels among hypertensive patients and hypertensive patients with diabetes. Clinicians managing hypertensive patients on statins should consider monitoring the HbA1c level and ensure that those with diabetes have their hyperglycaemia kept under control.

Summary: ESRD patients on RRT have a high risk of HCAIs, with catheter related bloodstream infection being the most common type of infection. Both Gram positive and Gram negative pathogens were found to be prevalent. The use of multiple accesses, elevated blood sugar levels, low serum sodium levels, and higher CRP concentration were associated with increased occurrence of HCAIs. Preventive strategies and optimal antibiotic therapy should be utilized to reduce the risk of these infections among ESRD patients.

Measurement accuracy and reliability of self-reported versus measured weight and height among adults in Malaysia: Findings from a nationwide blood pressure screening programme

Summary: Most studies on obesity prevalence rely on actual weight and height measurements, but self-reported measures have been shown to be reliable alternatives. This study investigates the accuracy and reliability of self-reported weight and height among adults in Malaysia. The results indicate substantial agreement between self-reported and measured values, with accurate BMI classification for 88.53% of participants. Therefore, self-reported measures may be useful for tracking and estimating population trends in Malaysian adults.

Summary: Discharge bundles aim to optimize patient outcomes for those hospitalized with exacerbation of COPD. Although previous studies suggest their effectiveness, inconsistencies in content and implementation have been observed. To ensure high-quality care and reduce readmission rates, a comprehensive discharge protocol is proposed, along with strategies to address implementation barriers. By utilizing successful implementation strategies from other disease areas, discharge bundles can be delivered more effectively to improve patient outcomes and ensure continuity of care.

A Systematic Review of Non-Seriously Ill Community-Dwelling Asians' Views on Advance Care Planning

Summary: This study systematically synthesized the views of community-dwelling Asians on advance care planning (ACP) and summarized the factors and reasons affecting their uptake of ACP. The findings showed that although Asian adults in the community were willing to engage in ACP, their awareness and uptake of ACP remained low. Background factors such as sociodemographic factors and health status, as well as underlying beliefs and attitudes towards ACP, were found to influence their uptake of ACP.

Innovation workshop using design thinking framework and involving stakeholders to co-create ideas for management of asthma

Summary: This paper uses the method of photovoice and design thinking to empathize with the challenges of asthma management and create innovative solutions. A one-day workshop was organized by SingHealth Polyclinics in Singapore, where healthcare providers discussed and ideated on the challenges and enablers of good asthma care. The study found that the use of photovoice and a multidisciplinary team format with patient involvement were highly favored. Participants' feedback indicated that the workshop effectively generated ideas and drove innovation in asthma care.

Experiences and influences of online health information-seeking about statin use in patients with high cardiovascular risk: a qualitative study

Summary: This study explores the views and experiences of patients with high cardiovascular risk on online health information-seeking about statins, and how this information influences their decisions. The results suggest that patients need different types of online health information throughout their disease trajectory, and unintentional passive exposure to online health information has an influence on patients' adherence to statins. The quality of patient-doctor communication in relation to online health information-seeking behavior remains critical for patient decision-making.
