Birth Weight and Prematurity in Infants with Single Ventricle Physiology: Pediatric Heart Network Infant Single Ventricle Trial Screened Population

Objectives. Although congenital heart disease is associated with low birth weight and prematurity, there is little information about these birth outcomes in infants with single ventricle physiology. We describe the birth outcomes (i.e., gestational age and birth weight) in neonates with single ventricle physiology screened for enrollment in the Pediatric Heart Network's Infant Single Ventricle Trial, compare these outcomes with US norms, and examine the association of birth outcomes with anatomic diagnosis and race. Patients and Methods. All neonates with single ventricle physiology presenting to Infant Single Ventricle Trial centers were screened for enrollment. Demographic data and anatomic diagnoses were obtained from medical records. Results. A total of 1245 neonates with single ventricle physiology were screened at 10 centers (63 to 266 per center). Diagnoses included hypoplastic left heart syndrome in 49%, unbalanced atrioventricular septal defect in 12%, and tricuspid atresia in 9%. Preterm birth occurred in 16% of neonates with single ventricle physiology vs. 12% in normal neonates (P < .001), low birth weight (< 2.5 kg) in 18% vs. 8% in normals (P < .001), and small for gestational age (< 10th percentile by definition) in 22% vs. 10% in normals (P < .001). A genetic syndrome was reported in 8%. The percentage of preterm birth, low birth weight, and small for gestational age was similar between screened neonates with and without hypoplastic left heart syndrome. Conclusions. In this large, contemporary cohort of neonates with single ventricle physiology, rates of preterm birth, low birth weight, and small for gestational age were higher than in the general population, but similar between screened neonates with and without hypoplastic left heart syndrome.

Summary: This study aimed to investigate the impact of intraoperative methylprednisolone during neonatal cardiac surgery on neurodevelopmental outcomes at 12 months of age and to identify early prognostic variables. The results showed that intraoperative methylprednisolone and other variables during surgery were not associated with neurodevelopmental outcomes. Participants who underwent a palliative procedure had worse neurodevelopmental outcomes and longer cardiac intensive care unit stays.

Use of Real-World Data and Physiologically-Based Pharmacokinetic Modeling to Characterize Enoxaparin Disposition in Children With Obesity

Summary: Dosing guidance for children with obesity is often unknown and it remains unclear whether children with and without obesity experience equivalent enoxaparin exposure. A physiologically-based pharmacokinetic model was developed to explore the differences in anti-Xa concentration between children with and without obesity. The model showed that weight-normalized clearance was reduced in children with obesity, leading to higher anti-Xa concentrations under weight-based dosing. Fat-free mass weight-based dosing improved enoxaparin exposure across different age groups and obesity status.

Summary: Obesity is a growing concern in public health, especially among children. Due to the lack of pharmacokinetic studies, there is limited information to guide the dosing of opioids in obese children. In order to fill this knowledge gap, researchers developed physiologically-based pharmacokinetic models to explore the interaction between obesity, age, and pharmacogenomics in children. The models accurately predicted drug concentrations and dosing requirements in obese children, highlighting the importance of considering both age and obesity status when prescribing medications.

Summary: This retrospective observational study analyzed data from 12,891 COVID-19 patients and found that age, severe COVID-19, severe acute kidney injury (AKI), and ischemic heart disease were associated with worse mortality outcomes. The severity of AKI was associated with poorer kidney function recovery, while the use of remdesivir was associated with better recovery. In patients without chronic kidney disease, age, male sex, severe AKI, and hypertension were associated with post-AKI kidney function impairment. COVID-19-associated AKI was also linked to higher mortality and worse long-term kidney function recovery.
