Massive gene amplification drives paediatric hepatocellular carcinoma caused by bile salt export pump deficiency

Hepatocellular carcinoma (HCC) is almost invariably associated with an underlying inflammatory state, whose direct contribution to the acquisition of critical genomic changes is unclear. Here we map acquired genomic alterations in human and mouse HCCs induced by defects in hepatocyte biliary transporters, which expose hepatocytes to bile salts and cause chronic inflammation that develops into cancer. In both human and mouse cancer genomes, we find few somatic point mutations with no impairment of cancer genes, but massive gene amplification and rearrangements. This genomic landscape differs from that of virus-and alcohol-associated liver cancer. Copy-number gains preferentially occur at late stages of cancer development and frequently target the MAPK signalling pathway, and in particular direct regulators of JNK. The pharmacological inhibition of JNK retards cancer progression in the mouse. Our study demonstrates that intrahepatic cholestasis leading to hepatocyte exposure to bile acids and inflammation promotes cancer through genomic modifications that can be distinguished from those determined by other aetiological factors.

ATP7B Genotype and Chronic Liver Disease Treatment Outcomes in Wilson Disease: Worse Survival With Loss-of- Function Variants

Vitamin B12 deficiency presenting as intestinal pseudo-obstruction in short bowel syndrome: A case report

Summary: Bowel obstruction and nutrient deficiencies are common complications in follow-up of short bowel syndrome patients. Vitamin B12 deficiency is rare among micronutrient deficits. The onset of B12 deficiency can be insidious with autonomic features preceding other symptoms. This study reports a case in which vitamin B12 deficiency presented with intestinal dysmotility mimicking mechanical bowel obstruction.

Summary: The transition from locally confined epithelial malignancies to invasive cancers, known as unjamming, is accompanied by mechanical deformations of cells and nuclei due to large fluctuations in cell density. This triggers a cellular mechano-protective mechanism involving changes in nuclear size and rigidity, heterochromatin redistribution, and remodeling of actin architecture. The chronic strains and stresses associated with unjamming eventually lead to DNA damage and nuclear envelope ruptures, activating a cytosolic DNA response gene program. This transcriptional rewiring alters the cell state, promoting malignant traits in breast carcinoma.

Successful sequential liver and hematopoietic stem cell transplantation in a patient with Fanconi anemia

Summary: We report a unique case of a child with Fanconi anemia with FANCD2 mutation who developed neonatal cryptogenic liver cirrhosis and bone marrow failure. The child successfully underwent sequential liver transplantation and hematopoietic stem cell transplantation in the first 2 years of life. Nineteen months after hematopoietic stem cell transplantation and 30 months after liver transplantation, the patient is clinically well with normal hematopoietic function and excellent liver function.

Mechanistic insights into the interactions between cancer drivers and the tumour immune microenvironment

Summary: In this study, the significant interactions between cancer-specific genetic drivers and anti- and pro-tumour features of the tumour immune microenvironment (TIME) were computed using Lasso regularised ordinal regression in 32 cancer types. The functional networks linking specific TIME driver alterations to the associated TIME state were reconstructed for head and neck squamous cancer (HNSC). The study identified 477 TIME drivers that are multifunctional genes, predicted the immune profiles of HNSC molecular subtypes, and provided insights into the immune-regulatory role of TIME drivers.

Emerging Insights into Molecular Mechanisms of Inflammation in Myelodysplastic Syndromes

Summary: Inflammation has a significant impact on human hematopoiesis, particularly in regulating the homeostasis of hematopoietic stem cells. Dysregulated inflammation can lead to abnormal hematopoiesis and may be involved in the pathogenesis of diseases such as myelodysplastic syndromes. Recent studies have used single-cell sequencing and multi-omic approaches to investigate the cellular heterogeneity and gene expression of hematopoietic stem cells in normal and malignant conditions, shedding light on the mechanisms underlying inflammatory signaling and immune response activation in MDS progression.
