In Vitro Antimicrobial Activity of Fibrin Sealants Containing Antimicrobial Agents

Background: Antimicrobial prophylaxis is required to prevent surgical site infections (SSIs). However, it is difficult to maintain prolonged antimicrobial activity in the surgical incision. Fibrin sealants are used primarily for hemostasis, suture support, or the adhesion of tissues. The aim of this study was to develop fibrin sealants that exhibit antimicrobial activity due to the addition of an antimicrobial agent. Methods: We determined the potencies and activities of cefazolin, ampicillin-sulbactam, vancomycin, teicoplanin, and arbekacin eluted from fibrin sealants. The antimicrobial activity was evaluated using methicillin-resistant Staphylococcus aureus (MRSA), multi-drug-resistant Pseudomonas aeruginosa (MDRP), and multi-drug-resistant Acinetobacter baumannii (MDRAB), which are the major pathogens responsible for hospital-acquired infections. Results: Each antimicrobial agent eluted continuously from the fibrin clots for approximately two weeks. All eluates from fibrin clots containing antimicrobial agents maintained antimicrobial activity against MRSA for at least 7d. The eluates from fibrin clots containing ampicillin-sulbactam inhibited the growth of both MDRP and MDRAB for at least 5d. Conclusion: Our data suggest that the addition of an antimicrobial agent to fibrin sealants is a useful method to prevent SSIs caused by bacteria, including antimicrobial-resistant strains.

Multidrug-resistant Cutibacterium avidum isolated from patients with acne vulgaris and other infections

Summary: This study investigated the epidemiological features and antimicrobial susceptibility of Cutibacterium avidum isolated from patients with acne vulgaris and other infections. The results showed that C. avidum strains exhibited resistance to clarithromycin, clindamycin, and ciprofloxacin, with the prevalence of the macrolide-clindamycin resistance gene erm(X). Pulsed-field gel electrophoresis analysis did not find clustering associated with antimicrobial resistance, biofilm-forming ability, or type of infection.

Increased Prevalence of Minocycline-Resistant Staphylococcus epidermidis with tet(M) by Tetracycline Use for Acne Treatment

Clinical features and antimicrobial treatment of skin infections caused by Panton-Valentine leukocidin-positive methicillin-resistant Staphylococcus aureus

Summary: Skin infections caused by PVL-positive MRSA, specifically the USA300 clone, have been increasing in Japan. This study reports 10 cases of PVL-positive MRSA skin infections over a two-year period, with 90% of the infections being caused by the USA300 clone. Atopic dermatitis (AD) was found to be a common underlying disease, and longer durations of antimicrobial therapy were required for AD patients compared to non-AD patients. However, all cases improved with long-term treatment using fosfomycin, minocycline, doxycycline, and/or rifampicin.

A case of Shewanella algae-induced bacteremia in Japan: Case report and literature review

Summary: This article presents a case of an 84-year-old man with S. algae-induced bacteremia and reviews 12 similar cases identified through literature search. Previous studies in Japan found that 69.2% of S. algae-induced bacteremia patients had a history of contact with fresh fish. Proper patient interviews, especially in hot seasons, and accurate identification techniques such as MALDI-TOF-MS and genetic testing are necessary if S. algae or other bacteria from the Shewanella genus are detected in blood culture tests.

Prevalence of antimicrobial-resistant Cutibacterium isolates and development of multiplex PCR method for Cutibacterium species identification

Summary: This study investigated the antimicrobial resistance of Cutibacterium species and developed a multiplex PCR method for their identification. The results showed higher antimicrobial resistance rates in strains isolated from acne vulgaris patients compared to those from patients with infectious diseases. The multiplex PCR method demonstrated 100% sensitivity and specificity in identifying the three Cutibacterium species.

Avidumicin, a novel cyclic bacteriocin, produced by Cutibacterium avidum shows anti-Cutibacterium acnes activity

Summary: The prevalence of antimicrobial-resistant Cutibacterium acnes in acne patients has increased due to inappropriate use of antimicrobial drugs. Commensal skin bacteria can produce antimicrobial substances to maintain the balance of the skin microbiome. In this study, skin bacteria with anti-Cu. acnes activity were evaluated. Cu. avidum, a commensal skin bacterium, produces avidumicin, a novel cyclic bacteriocin with a narrow antimicrobial spectrum, which has been found to have anti-Cu. acnes activity. These findings suggest that Cu. avidum and avidumicin may serve as potential alternative agents in antimicrobial therapy for acne vulgaris.

Multidrug Resistance Plasmid pTZC1 Could Be Pooled among Cutibacterium Strains on the Skin Surface

Summary: The emergence of antimicrobial resistance in bacteria causing acne vulgaris is a major concern. The prevalence of macrolide and clindamycin resistance in Cutibacterium acnes is due to the acquisition of exogenous antimicrobial resistance genes harbored by the multidrug resistance plasmid pTZC1. The transfer of pTZC1 between different Cutibacterium species has been identified, indicating the potential spread of antimicrobial resistance.

Summary: The Cutibacterium avidum isolate TP-CV302 from an acne vulgaris patient in Japan carries the macrolide-clindamycin resistance gene erm(X) located on Tn5432. While this mobile genetic element has been extensively identified in Cutibacterium acnes, it has not been found in Cutibacterium avidum.

Surveillance of Antimicrobial Prescriptions in Community Pharmacies Located in Tokyo, Japan

Summary: An AMR Action Plan was implemented in Japan in 2016 to combat the spread of antimicrobial-resistant bacteria in the community. Additional support for appropriate use of pediatric antimicrobial agents was introduced in 2018. The effectiveness of the action plan was evaluated by analyzing antimicrobial prescriptions in 42 community pharmacies in Tokyo. The study found that the number of prescriptions for certain antimicrobial agents decreased, indicating the positive impact of the AMR Action Plan in promoting appropriate use of antimicrobial agents in Japan.
