In October 2000 the German Federal Committee of Physicians and Health Insurers recommended that special Model Projects on Acupuncture (Modellvorhaben Akupunktur'') be developed in order to determine the evidence-based role of acupuncture in the treatment of certain illnesses. This paper presents a summary of the main randomised controlled trials performed as part of these projects, and the associated economic analyses. Overall the results show that acupuncture is effective in practice for a range of chronic conditions, and it seems likely to have acceptable cost utility (at least at a rate of (sic)35 per session). Sham acupuncture, in the form of minimal off-point needling in a therapeutic context, also appears to be effective, being no different to prophylactic medication in migraine, and superior to guideline-based standard care in chronic low back pain. In patients recruited to acupuncture trials, the response to treatment does not differ between those that agree to be randomised and those that do not. This suggests that the results of the pragmatic Acupuncture in Routine Care studies are applicable to patients from the general population who express a preference for acupuncture. In conclusion, acupuncture appears to be effective in a range of chronic conditions and it seems to have acceptable cost-effectiveness in Western health economic terms. These programmes of research do not confirm the hypothesis that needling at specific points is essential to achieve satisfactory clinical effects of acupuncture. Sham acupuncture, in the form of minimal off-point needling in a therapeutic context, is unlikely to be an inactive placebo. In April 2006, the German health authorities decided that acupuncture would be included into routine reimbursement by social health insurance funds for chronic low back pain and chronic osteoarthritis of the knee.

Effects of manual acupuncture and electroacupuncture for lateral epicondylalgia of musculoskeletal origin: a systematic review and meta-analysis

Marcos J. Navarro-Santana, Jorge Sanchez-Infante, Guido F. Gomez-Chiguano, Mike Cummings, Cesar Fernandez-de-las-Penas, Gustavo Plaza-Manzano

Summary: The study showed that acupuncture has a moderate effect on reducing elbow pain and improving related-disability in lateral epicondylalgia of musculoskeletal origin. However, acupuncture and electroacupuncture had small effects on strength, with significant effects mostly observed in the short term.

A feasibility trial of acupuncture in cancer patients undergoing radiotherapy treatment

Summary: The feasibility trial showed that providing cancer patients with the opportunity to receive acupuncture within their routine NHS care is acceptable and valued, with participants perceiving positive impacts of the treatment. However, quantitative analysis revealed no significant differences between those receiving acupuncture and those not in terms of outcome measures.

Needling Interventions for Sciatica: Choosing Methods Based on Neuropathic Pain Mechanisms-A Scoping Review

Thomas Perreault, Cesar Fernandez-de-las-Penas, Mike Cummings, Barry C. Gendron

Summary: Acupuncture is supported as an effective treatment for sciatica, but it is unclear whether neuropathic pain mechanisms are considered in needling interventions. Further research is needed to explore if considering NP mechanisms could enhance the efficacy of needling interventions for sciatica.

Periosteal Needling to the Cervical Articular Pillars as an Adjunct Intervention for Treatment of Chronic Neck Pain and Headache: A Case Report

Thomas Perreault, Mike Cummings, Jan Dommerholt, Deanna Hayes, Jonathan Hobbs

Summary: This case report presents the use of periosteal dry needling (PDN) to treat chronic neck pain and headache in an 85-year-old female patient. The application of PDN to the cervical articular pillars (CAPs), following trigger point dry needling (TrPDN) and manual therapy, led to significant improvement in pain and function.
