Purpose of reviewThe prevalence of obesity is on the increase, and consequently metabolic syndrome is also becoming a serious health problem in children and adolescents all over the world. This review attempts to summarize the recent literature on metabolic syndrome in children and adolescents.Recent findingsTo date, a standard definition of metabolic syndrome for the pediatric population is not available. Recently, the International Diabetes Federation has proposed a new set of criteria to define metabolic syndrome in children and adolescents aged 6-16 years. The relationships between obesity, insulin resistance and metabolic syndrome may be explained by the pattern of lipid partitioning. Fatty liver plays a central role in the insulin-resistant state in obese adolescents. Although insulin resistance has been proposed as the central factor leading to the abnormalities observed in metabolic syndrome, most definitions of metabolic syndrome use impaired fasting glucose as a marker. Nutrition impairment during both prenatal and early postnatal life can cause metabolic disturbances leading to insulin-resistance, type 2 diabetes, hypertension and cardiovascular disease.SummaryMetabolic syndrome prevalence in children and adolescents is on the increase. Therefore, the emphasis in all studies and programs related to metabolic syndrome should be focused on prevention, early detection of metabolic risk factors and interventions that will have a significant impact on future adult health.

Growth and Pubertal Features in a Cohort of 83 Patients with Osteogenesis Imperfecta

Summary: Osteogenesis imperfecta (OI) is a genetic disease characterized by fragile bones and variable short stature. In this retrospective cohort study, demographic data, clinical findings, growth and pubertal characteristics, and medical treatment of 83 OI patients were evaluated. The results showed that bisphosphonate therapy can improve bone mineral density and height in OI patients, but final adult height did not improve. Overweight and obesity were found to increase over time, and precocious and early puberty were common in OI patients. Effective interventions to improve mobility and functional parameters are necessary for these children.

Increased Carotid Intima-media Thickness and Its Association with Carbohydrate Metabolism and Adipocytokines in Children Treated with Recombinant Growth Hormone

Summary: This study aimed to evaluate carotid intima-media thickness (cIMT) in children treated with recombinant human growth hormone (rhGH) and assess the effects of rhGH therapy on cIMT, as well as the changes in serum carbohydrate metabolism, lipid profile, and adipocytokines. The results showed that cIMT was increased in children treated with rhGH and it was associated with higher ghrelin levels and lower HDL levels. The alterations in carbohydrate metabolism were not associated with cIMT in children treated with rhGH.

Evaluation of Children with Secondary Osteoporosis: A Single-center Experience

Summary: This study evaluated the clinical characteristics and treatment responses of children with chronic diseases, finding that they are at risk of inadequate bone mineralization. Dual-energy X-ray absorptiometry (DXA) was used to assess bone mineral density, and it was found that bisphosphonate treatment significantly improved bone density.

A novel RNPC3 gene variant expands the phenotype in patients with congenital hypopituitarism and neuropathy

Summary: This study evaluates the clinical and molecular characteristics of two patients with congenital hypopituitarism and neuropathy. The patients showed developmental delay, intellectual disability, and deficiencies in thyroid hormones, growth hormone, and prolactin. MRI revealed pituitary hypoplasia. A novel pathogenic variant in the RNPC3 gene was detected in both patients.

Long-term 10-year comparison of girls with congenital adrenal hyperplasia who underwent early and late feminizing genitoplasty

Summary: The study aimed to evaluate the relationship between the age at surgery and the long-term outcomes of feminizing genitoplasty in patients with CAH. It was found that patients who underwent surgery before the age of 2 years required more additional surgeries and had lower satisfaction rates. Therefore, it is recommended to postpone surgery until the patient's gender identity matures and they can autonomously decide whether to undergo surgery.

Body proportions in patients with Turner syndrome on growth hormone treatment

Summary: This study aimed to evaluate the auxological measurements and body proportions of Turner syndrome patients treated with recombinant human growth hormone (GH) and compare them with a group of healthy females. The results showed that TS patients had significantly shorter height, sitting height, and arm span, higher BMI, decreased extremity-trunk ratio, and shorter leg length compared to healthy controls. Despite GH treatment, TS patients still had disproportionate short stature.

Summary: This study evaluated the practice of assigning female gender to 46, XY infants with limited virilization, particularly in the case of partial androgen insensitivity syndrome (PAIS). The results showed that female assignment is becoming less common in PAIS and there is no evidence to support prophylactic prepubertal gonadectomy in reducing the risk of germ cell tumor.
