Early Experience with Targeted Therapy and Dendritic Cell Vaccine in Metastatic Renal Cell Carcinoma after Nephrectomy

Purpose: Metastatic renal cell carcinoma (RCC) is one of the most treatment-resistant malignancies and nephrectomy, isolated or combined with systemic chemotherapy typically has limited or no effectiveness. We report our initial results in patients treated with the association of molecular targeted therapy, nephrectomy, and hybrid dendritic-tumor cell (DC) vaccine. Materials and Methods: Two male patients diagnosed with metastatic RCC were selected for the study. They were treated with the triple strategy, in which sunitinib (50 mg per day) was given for 4 weeks, followed by radical nephrectomy after two weeks. DC vaccine was initiated immediately after surgery and repeated monthly. Sunitinib was restarted daily after 2 to 3 weeks of surgery with a 7-day interval every 4 weeks. Results: Both patients had complete adherence to the proposed treatment with DC vaccine therapy combined with sunitinib. Follow-up in these patients at 9 and 10 months demonstrated a stable disease in both, as shown by imaging and clinical findings, with no further treatment required. Conclusion: The immune response obtained with DC vaccine combined with the antiangiogenic effect of sunitinib and the potential benefits of cytoreductive nephrectomy in advanced disease could represent a new option in the treatment of metastatic RCC. Further prospective trials are needed not only to elucidate the ideal dosing and schedule, but also to better define the proof-of-concept proposed in this report and its role in clinical practice.

Summary: The classification of malignant tumors is influenced by immunohistochemical and molecular genetic findings. Renal oncocytoma, a benign tumor, can occasionally exhibit malignant behavior as a variant called hybrid oncocytic chromophobe tumor. Two new entities, low-grade oncocytic tumor (LOT) and eosinophilic vacuolated tumor (EVT) with varying nuclear pleomorphism, have been proposed. The diagnosis of these apparently benign tumors is based on morphological, immunohistochemical features, and mutations in the mTOR pathway. However, the importance and specificity of these tumors are still debatable.

Summary: There is evidence that cribriform morphology is associated with a worse prognosis for prostatic adenocarcinoma. This study aimed to investigate the reproducibility of cribriform cancer diagnosis in prostate needle biopsies. A panel of nine prostate pathology experts independently reviewed 304 digitised biopsies and achieved a 2/3 consensus diagnosis of cribriform and noncribriform cancer in 90% of the cases. Strict diagnostic criteria were used to identify cribriform patterns and generate a set of consensus cases for standardization.

Summary: This study aimed to evaluate the suppressive effect of miR-137 in prostate cancer, regardless of diet-induced hypercholesterolemia. The results showed that miR-137 could inhibit the transcription and translation of the p160 family, SRC-1, SRC-2, and SRC-3, and indirectly reduce the expression of AR. It also inhibited cell migration and invasion, reduced proliferation, and increased apoptosis rates. In vivo experiments demonstrated that tumor growth was arrested after intratumoral restoration of miR-137, with a more significant response in the hypercholesterolemic group.

LncRNA JHDM1D-AS1 Is a Key Biomarker for Progression and Modulation of Gemcitabine Sensitivity in Bladder Cancer Cells

Summary: Long non-coding RNAs are frequently dysregulated and have been associated with carcinogenesis, aggressiveness, and chemoresistance in tumors. In this study, we investigated the combined expression of JHDM1D and JHDM1D-AS1 in bladder tumors, and found that it can distinguish between low-grade and high-grade tumors. We also discovered that silencing JHDM1D-AS1 can reduce the growth and proliferation of high-grade bladder tumor cells and increase their sensitivity to gemcitabine treatment.
