Calcium oxalate nephrolithiasis and expression of matrix GLA protein in the kidneys

Polymorphism of the gene for matrix GLA protein (MGP), a calcification inhibitor, is associated with nephrolithiasis. However, experimental investigations of MGP role in stone pathogenesis are limited. We determined the effect of renal epithelial exposure to oxalate (Ox), calcium oxalate (CaOx) monohydrate (COM) or hydroxyapatite (HA) crystal on the expression of MGP. MDCK cells in culture were exposed to 0.3, 0.5 or 1 mM Ox and 33, 66 or 133-150 mu g/cm(2) of COM/HA for 3-72 h. MGP expression and production were determined by Western blotting and densitometric analysis. Enzyme-linked immunosorbent assay was performed to determine MGP release into the medium. Hyperoxaluria was induced in male Sprague-Dawley rats by feeding hydroxyl-l-proline. Immunohistochemistry was performed to detect renal MGP expression. Exposure to Ox and crystals led to time- and concentration-dependent increase in expression of MGP in MDCK cells. Cellular response was quicker to crystal exposure than to the Ox, expression being significantly higher after 3-h exposure to COM or HA crystals and more than 6 h of exposure to Ox. MGP expression was increased in kidneys of hyperoxaluric rats particularly in renal peritubular vessels. We demonstrate increased expression of MGP in renal tubular epithelial cells exposed to Ox or CaOx crystals as well as the HA crystals. The most significant finding of this study is the increased staining seen in renal peritubular vessels of the hyperoxaluric rats, indicating involvement of renal endothelial cells in the synthesis of MGP.

Summary: The study aimed to enhance the oral bioavailability of the poorly water-soluble drug tacrolimus by developing niosomes using polyoxyethylene lauryl ether (Brij-35) and poloxamer 407 surfactants. The niosomes were prepared using a thin film hydration technique and evaluated using various techniques including FTIR, ultra-centrifugation, DLS experiments, SEM microscopy, and DSC technique. The results showed that the newly developed niosomes can improve the dissolution profile of tacrolimus and can be considered as potential oral carriers.

Toxicity profiling of Burgmansia aurea Lagerh. Leaves using acute and sub-acute toxicity studies in rats

Summary: This study aimed to detect any hazardous effects of B. aurea on animals and find out its LD50. The results revealed no mortality and morbidity in both acute oral and sub-acute toxicity studies. However, oxidative stress markers were significantly increased in the treatment groups, and animals showed decreased food and water intake in the acute oral toxicity study. A slight difference in renal function tests was observed, but no significant change in histopathology was found in selected organs. This study concluded that B. aurea can be safely used for pharmacological purposes.

An insight into the hepatoprotective role of Velpatasvir and Sofosbuvir per se and in combination against carbon tetrachloride-induced hepatic fibrosis in rats

Summary: This study aimed to examine the antifibrotic effect of direct-acting antiviral drugs Sofosbuvir and Velpatasvir in a rat model of carbon tetrachloride-induced fibrosis. The results showed that Sofosbuvir and Velpatasvir, alone or in combination, significantly inhibited fibrosis and mediated this effect by reducing the levels of TNF-a/NF-& kappa;B and the IL-6 signaling pathway. Therefore, Sofosbuvir and Velpatasvir may have antifibrotic effects independent of their antiviral activity.

An insight investigation to the antiurolithic activity of Trachyspermum ammi using the in vitro and in vivo experiments

Summary: The crude extract of Trachyspermum ammi seeds (Ta.Cr) has shown antiurolithic activity through diuretic effects, inhibition of calcium oxalate crystal aggregation, antioxidant effects, renal epithelial cell protection, and antispasmodic effects. This suggests that Ta.Cr has therapeutic potential in urolithiasis.

Use of Complementary and Alternative Medicine in the General Public of Western Saudi Arabia: A Cross-Sectional Survey

Summary: This study investigated the awareness, self-use, perceptions, beliefs, and attitudes towards complementary and alternative medicine (CAM) in the general public of Jeddah, Saudi Arabia. The majority of the population was aware of various CAM modalities and considered prayers and spirituality to be the most effective. Information about CAM was mainly obtained from friends and relatives, while lack of knowledge and lack of trained professionals were perceived as barriers to CAM implementation. Gender, educational level, and relationship to the medical field were found to be associated with awareness and self-use of CAM modalities.

Despite the belief that herbal medicines are safe and do not require consultation, there is a need to increase awareness and establish strategies to evaluate their safety and efficacy.
