Summary: Positive findings on surgical margins in en bloc resection of bladder tumors were not associated with recurrence-free survival. Careful assessment is needed for residue at the original site. A second transurethral resection may be considered for patients with positive horizontal and vertical margins in pT1 bladder cancer.

Summary: En bloc resection for bladder tumors appears to be safer and provides superior histopathological information compared to conventional transurethral resection of bladder tumors. However, it does not seem to improve the recurrence rate conclusively.

Summary: Esophageal adenoid cystic carcinoma is a rare and challenging cancer. In this case, the tumor invaded a significant portion of the posterior trachea, making conventional surgical methods impossible. To achieve complete tumor resection, a laparoscopic esophagectomy and posterior tracheal resection with tracheoplasty using a rotational flap were performed, with the patient on venovenous extracorporeal membrane oxygenation.

Summary: The study found that en bloc esophagectomy after chemoradiation for esophageal adenocarcinoma resulted in a higher lymph node harvest and prolonged progression-free survival compared to standard esophagectomy. There were no significant differences in postoperative complications between the two surgical approaches.

Summary: The clinical significance of repeat transurethral resection (reTUR) and surgical margin status after en bloc resection of bladder tumor (ERBT) for pathological T1 (pT1) bladder cancer was assessed. It was found that reTUR did not improve recurrence or progression after ERBT for pT1 bladder cancer. Surgical margin status affected prognosis and reTUR outcomes.

Summary: Maximal cytoreductive surgery is crucial for prognosis in advanced epithelial ovarian cancer, with en bloc pelvic resection and rectosigmoid colectomy being considered an effective surgical strategy. Despite widespread adoption and demonstrated safety, limited research has led to ongoing debates regarding the efficacy of this surgical approach.

Summary: This retrospective study analyzed the clinical data of 21 patients who underwent arterial reconstruction surgery among 976 patients with soft tissue sarcoma. The results showed that arterial reconstruction can improve the success rate of sarcoma resection, but it is associated with a high incidence of postoperative complications and reoperation rate. This suggests that the characteristics of patients and treatment strategies should be taken into account when performing arterial reconstruction surgery.

Summary: In this study, clinical features, perioperative and long-term outcomes in patients who underwent vertebrectomy for cancer were analyzed. The results showed that primary vertebral tumors had the best 5-year overall survival rate, and surgery should be advised by a multidisciplinary team and only for selected cases. The use of a combined approach did not increase the risk of complications.

Summary: This article summarizes the history and development of en bloc resection of bladder tumour (ERBT) and discusses its future directions in managing bladder cancer. Since its first report in 1980, there have been significant advancements in the techniques, energy modalities, and tumor retrieval methods of ERBT. The international consensus statement on ERBT has been developed as a standard reference for urologists, and high-quality evidence on ERBT is emerging. Modified approaches of ERBT should be used for large bladder tumors, and the global ERBT registry has been launched to study its value in a real-world setting.

Summary: This study evaluated the combination of a novel AKT inhibitor, MK-4440, with imatinib mesylate (IM) in gastrointestinal stromal tumor (GIST) cells and preclinical models. The results demonstrated synergistic effects of dual inhibition of KIT and AKT in both IM-sensitive and -resistant GIST cell lines, with enhanced cell death through cell cycle arrest and increased PDCD4 expression. In vivo studies showed superior efficacy of the MK-4440/IM combination in IM-sensitive GIST models, suggesting potential clinical benefits in primary GIST treatment.

Summary: Tumor mutational burden (TMB) has been identified as a biomarker for predicting response to immune checkpoint inhibitors in various cancers. This study aimed to evaluate the potential of TMB and somatic mutations in predicting disease recurrence in melanoma. The results showed that TMB-L tumors and mutations in BRAF and NRAS were associated with higher risk of recurrence.

Summary: Amid the COVID-19 pandemic, there is a paradigm shift in the treatment of pancreatic cancer to adapt to the new health threat. Utilizing neoadjuvant treatment and safe surgery in a controlled environment can improve oncological outcomes, whereas oncologic procedures in hazardous settings might shorten patient survival if complicated by COVID-19 infection.
