A randomized, triple masked, placebo-controlled clinical trial for controlling childhood obesity

The efficacy of pharmacological treatment in controlling childhood obesity is controversial. We aimed to compare the effects of three types of drug regimens and placebo on generalized and abdominal obesity among obese children and adolescents who did not succeed to lose weight 3 months after lifestyle modification (diet and exercise). This triple-masked randomized clinical trial was conducted among 180 participants aged 10-16 years. They were assigned randomly to 4 groups of equal number to receive metformin, fluoxetine, a combination of the two drugs, or placebo. The trial lasted for 12 weeks and participants were followed up for an additional 12-week period. Overall, 91.1% (n=164) of the enrolled participants completed the trial. After the 12-week trial, the body mass index decreased significantly in all groups receiving medications [approximately -1.2 (0.2) kg/m(2), P < 0.05]. This decrease was not significant in the placebo group. Waist circumference decreased significantly in the groups receiving metformin [-2.1 (0.4) cm, P=0.03)] as well as in the group receiving a combination therapy of metformin and fluoxetine [-2.5 (0.4) cm, P=0.01)]. In the 24-week follow-up study, these anthropometric indexes were lower than the baseline in the group that had received a combination therapy of metformin and fluoxetine. No serious drug side-effects were reported. A limited period of such treatment may help weight control, and might be used to encourage those children who have been refractory to weight loss for continuing the non-pharmacological programs. Our findings should be confirmed in future studies with a longer follow-up period.

Summary: This systematic review suggests that individuals born in cold season may have higher BMI and weight in childhood, and birth month may have an impact on weight and BMI in boys. However, there is no clear association between season/month of birth and birth weight. Further research should focus on identifying the impact of confounding factors such as vitamin D status in the pathway of this association.

Investigating determinants of parabens concentration in maternal urine

Summary: The study evaluated urinary parabens concentration among 117 Iranian pregnant women in their first trimester. Methylparaben, ethylparaben, propylparaben, and butylparaben were detected with certain median concentrations, and significant associations were found with variables such as educational level, plastic packaging, physical activity, sampling season, and use of personal care products. This suggests that Iranian pregnant women are at high risk of paraben exposure.

The association between familial and environmental factors and prevalence of congenital hypothyroidism in center of Iran

Summary: This study aimed to investigate the association between familial and environmental factors and prevalence of congenital hypothyroidism in Isfahan province. The results showed a significant positive association between soil cadmium and air pollution with the level of screening serum TSH in CH patients. However, no significant association was found between soil lead and cadmium levels and air pollutants with the prevalence of CH in different cities of Isfahan province.

Simvastatin nanosuspensions prepared using a combination of pH-sensitive and timed-release approaches for potential treatment of colorectal cancer

Summary: A dual pH- and time-dependent polymeric coated capsule was developed for site-specific release of simvastatin in the colon, using nanosuspension and anti-solvent crystallization technique to improve drug solubility. The nanosizing technique transformed the crystalline drug into a more soluble amorphous form, while the coated capsules provided specific delivery of simvastatin in the colon with enhanced cytotoxic effects of the drug nanoparticles against HT29 cells. The findings suggest that the coated capsules using ethyl cellulose and Eudragit S100 polymers could be suitable for colon-targeted delivery of simvastatin.

Summary: In this study, Eudragit RL nanoparticles loaded with simvastatin were prepared using an emulsion solvent evaporation method for the treatment of ulcerative colitis. The optimized nanoparticles showed improved therapeutic effects and reduced inflammation in a rat model of colitis. This drug delivery system can be considered as an alternative treatment for ulcerative colitis.

Summary: This study aimed to examine the association between dietary acid load (DAL) and anthropometric indices in Iranian children and adolescents. The findings revealed a direct association between diet-induced acid load and neck circumference, as well as an inverse association between DAL indices and parental BMI. Further well-designed clinical studies are needed to confirm these results and explore the underlying mechanisms.

Technical factors such as navigation support, ease of searching, and finding needed information also played a crucial role, along with organizational factors like proximity to internet facilities and access during consultations.
