State-of-the-art preoperative staging of gastric cancer by MDCT and magnetic resonance imaging

Gastric cancer is one of the most common and fatal cancers. The importance of accurate staging for gastric cancer has become more critical due to the recent introduction of less invasive treatment options, such as endoscopic mucosal resection or laparoscopic surgery. The tumor-node-metastasis staging system is the generally accepted staging system for predicting the prognosis of patients with gastric cancer. Multidetector row computed tomography (MDCT) is a widely accepted imaging modality for the preoperative staging of gastric cancer that can simultaneously assess locoregional staging, including the gastric mass, regional lymph nodes, and distant metastasis. The diagnostic performance of MDCT for T- and N-staging has been improved by the technical development of isotropic imaging and 3D reformation. Although magnetic resonance imaging (MRI) was not previously used to evaluate gastric cancer due to the modality's limitations, the development of high-speed sequences has made MRI a feasible tool for the staging of gastric cancer. Summary: The study aimed to evaluate the diagnostic and prognostic value of non-hyperemic Pd/Pa and its additional value when combined with hyperemic pressure ratio (FFR) to guide revascularization. A total of 2141 patients with physiological data in 2726 vessels were included in a retrospective registry. Abnormal pressure ratios in both non-hyperemic and hyperemic conditions were found to have important prognostic value. The combined use of FFR and non-hyperemic Pd/Pa efficiently identified vessels with concordant normal pressure ratios and excellent long-term clinical outcomes.

Intra-individual comparison of two-dimensional shear wave elastography techniques using plane wave imaging and the multi-beam technique: are they interchangeable in measuring liver fibrosis?

Summary: This study compared two different two-dimensional shear wave elastography techniques, PWI and MB imaging, for evaluating liver fibrosis. The results showed that the MB technique is more reliable for long-term follow-up, suggesting its preference in assessing liver fibrosis in chronic liver disease patients.

Post-neoadjuvant treatment pancreatic cancer resectability and outcome prediction using CT, 18F-FDG PET/MRI and CA 19-9

Summary: This study aimed to determine if the addition of PET/MRI and CA 19-9 to CECT can improve the accuracy of predicting resectability and prognosis in PDAC patients. The results showed that the combination of CECT, PET, and CA 19-9 increased the AUC and sensitivity for determining resectability without compromising specificity. Additionally, FDG avidity on post-NAT PET and pathologically confirmed vascular invasion predicted recurrence-free survival in PDAC patients.

Efficacy and Safety of Half-Dose Gadopiclenol versus Full-Dose Gadobutrol for Contrast-enhanced Body MRI

Summary: This study compared the effectiveness of Gadopiclenol and Gadobutrol for lesion visualization in body MRI. The results showed that Gadopiclenol at a dose of 0.05 mmol/kg was comparable to Gadobutrol at a dose of 0.1 mmol/kg in terms of lesion evaluation, and both had a similar safety profile.

Anatomical ablation for small hepatocellular carcinomas using multiple applicators: a preliminary study

Summary: The purpose of this study was to evaluate the technical feasibility and therapeutic outcomes of anatomical ablation using multiple radiofrequency applicators for peripherally-located small hepatocellular carcinomas (HCCs). The results showed a technical success rate of 98.2% and a low incidence of local tumor progression (3.7%) after treatment. Therefore, anatomical ablation using multiple RF applicators can provide excellent local tumor control in patients with peripherally-located small HCCs.

Sonazoid-enhanced ultrasonography for noninvasive imaging diagnosis of hepatocellular carcinoma: special emphasis on the 2022 KLCA-NCC guideline

Summary: Contrast-enhanced ultrasonography (CEUS) is a noninvasive imaging modality used for diagnosing hepatocellular carcinoma (HCC) without the need for pathological confirmation. Different contrast agents, such as SonoVue and Sonazoid, are available for CEUS imaging. Major guidelines recognize CEUS as a reliable method for HCC diagnosis, but the choice of contrast agent varies. This review provides a comparative overview of the pharmacokinetic features, examination protocols, diagnostic criteria, and potential applications of these contrast agents in the HCC diagnostic algorithm.

Summary: Coronary stent underexpansion is associated with restenosis and stent thrombosis. High wall shear stress (WSS) has been linked to prothrombotic pathways activation, matrix metalloproteinases upregulation, and future myocardial infarction in atherosclerosis studies. This study aimed to investigate the relationship between stent underexpansion and WSS. The results showed that frames with stent underexpansion were more likely to exhibit high WSS, while no significant association was found between underexpansion and low WSS or between underexpansion and stent platform.

Pancreas CT assessment for pancreatic ductal adenocarcinoma resectability: effect of tube voltage and slice thickness on image quality and diagnostic performance

Summary: The assessment of tumor vascular contact is crucial for evaluating the resectability of pancreatic ductal adenocarcinoma (PDAC). This study compared the image quality and diagnostic performance of high-resolution (HR) pancreas computed tomography (CT) using 80 kVp with 1 mm slice thickness (ST) with the standard protocol CT using 120 kVp. The results showed that HR-CT had better contrast and tumor visibility, but it was not significantly different from standard CT in predicting resectability.
