Summary: Infectious diseases cause significant human suffering and socioeconomic burden worldwide. Understanding the interactions between human pathogens and hosts is crucial for developing effective treatment strategies. However, there is a lack of model systems that accurately replicate these interactions. The tropism of hepatitis B (HBV) and C (HCV) viruses for the human liver poses challenges in studying their interactions during the natural progression of these infections. This review summarizes the efforts to advance our knowledge of virus-host interactions in chronic HCV and HBV infection through the analysis of surplus liver biopsy tissue donated by patients.

Summary: Severe alcoholic hepatitis has limited treatment options and poor outcomes with current supportive medical care and corticosteroids. While new treatment options like granulocyte colony-stimulating factor and stem cell transplantation are being explored, evidence for their efficacy remains scarce and further research is needed. Insights into the pathogenesis of AH and liver regeneration processes are crucial for the development of new treatment options, but high-quality studies and reliable experimental models are lacking.

The results showed that allocating corneas from HBsAg(+) or anti-HBc(+) donors to HBsAg(+) or hepatitis B surface antibody recipients maximizes cornea usage from hepatitis B donors without compromising transplant safety.

Summary: Hepatitis B virus (HBV) infection is a serious public health problem and studies on the pathogenesis of chronic hepatitis B (CHB) combined with nonalcoholic fatty liver disease (NAFLD) are needed. This study explored the correlation between HBV-related autophagy and the progression of NAFLD. The results showed that HBV promoted the occurrence of NAFLD and lipid droplet accumulation in hepatocytes. Furthermore, HBV-related autophagy was found to inhibit the decomposition of lipid droplets, leading to their accumulation in hepatocytes and promoting the progression of NAFLD.

Post-LT HCV reinfection patterns vary, and quasispecies complexity is not predictive of liver damage progression. Higher VL values post-LT, particularly at day 15, may predict fast liver fibrosis progression, independent of fibrosis stage.

This study identified distinct patterns of alcohol use post-transplantation and found that early alcohol use was associated with higher risk of mortality at 3 and 5 years. Pre-transplant characteristics were associated with post-transplant alcohol use patterns, which can inform candidate selection and addiction care after transplantation.

Summary: Alcohol-associated liver disease (ALD) is a common liver disease and a reason for liver transplantation (LT). Patients with ALD often have alcohol use disorder (AUD) and psychiatric comorbidities. Early identification of ALD and treatment of AUD can prevent progression to advanced stages of the disease. However, there are barriers to screening and treatment for AUD in ALD patients. This review discusses the multidisciplinary care model for managing the dual pathology of ALD and AUD, with a focus on the roles of hepatologists, psychiatrists, addiction counselors, and social workers.

Summary: This study compared the waitlist and post-transplant outcomes of liver transplantation alone (LTA) and simultaneous liver kidney (SLK) listings. Summary: Alcohol-associated liver disease (ALD) is a complex and rapidly increasing disease without approved therapeutic targets. Challenges in study designs and clinical trials need to be overcome to drive drug development in this field. ALD management should focus on achieving and maintaining alcohol abstinence, while early liver transplantation protocols and noninvasive biomarkers require improvement. Integrated multidisciplinary care models are urgently needed to improve long-term outcomes for ALD patients with alcohol use disorder and liver disease.

A survey found that 11.3% of patients reported an impact on their liver disease, with 73% experiencing delays in follow-up care. Furthermore, 81% of patients with a history of COVID-19 and 69% without a history experienced worsening in at least one area.

Integrated care models involving hepatologists and addiction specialists are considered to be the optimal approach for managing patients with dual pathology. However, the implementation of these models is challenging due to various barriers at the patient, clinician, and system levels.
