Efficacy of tricyclic antidepressants in irritable bowel syndrome: A meta-analysis

We aimed to evaluate the efficacy of tricyclic antidepressants (TCAs) as a therapeutic option for irritable bowel syndrome (IBS) through meta-analysis of randomized controlled trials. For the years 1966 until September 2008, PubMed, Scopus, Web of Science, and Cochrane Central Register of Controlled Trials were searched for double-blind, placebo-controlled trials investigating the efficacy of TCAs in the management of IBS. Seven randomized, placebo-controlled clinical trials met our criteria and were included in the meta-analysis. TCAs used in the treatment arm of these trials included amitriptyline, imipramine, desipramine, doxepin and trimipramine. The pooled relative risk for clinical improvement with TCA therapy was 1.93 (95% CI: 1.44 to 2.6, P < 0.0001). Effect size of TCAs versus placebo for mean change in abdominal pain score among the two studies was -44.15 (95% CI: -53.27 to -35.04, P < 0.0001). It is concluded that low dose TCAs exhibit clinically and statistically significant control of IBS symptoms. (C) 2009 The WIG Press and Baishideng. All rights reserved.

Epigenotoxicity: a danger to the future life

Summary: Environmental toxicants can regulate gene expression through epigenetic mechanisms. This article discusses the impact of three epigenetic modifications on seven important categories of diseases and the influence of eleven main environmental factors on epigenetic modifications. These epigenetic-related mechanisms play a crucial role in the development of common diseases, aiding in diagnosis and treatment, as well as predicting transgenerational and intergenerational effects. However, more well-designed research is needed to determine the precise role of environmentally-induced epigenetic alterations in disease etiology.

The Effects of Clove and Its Constituents on Reproductive System: a Comprehensive Review

Summary: The aim of this study is to investigate the contradictory effects of clove and its phytochemicals on the reproductive system of both males and females. A total of 76 articles were collected and analyzed, revealing the effects of clove and its constituents on sex hormones, fertility, sperm abnormalities, endometriosis, menstrual cycle, gynecological infections, and reproductive tumors. The main mechanism of clove's action remains unclear, but it appears that various factors, such as extraction method, dosage, and duration of administration, as well as the underlying cause of the disorder, can influence its pharmacological activity. More detailed studies are needed to explore the potential therapeutic effects of clove for reproductive disorders.

The effect of a topical formulation from Lawsonia inermis L. (henna) on pain intensity in patients with chronic sciatica: A randomized double-blind clinical trial

Summary: This study aimed to investigate the efficacy of a topical formulation from henna on pain intensity in patients with chronic sciatica. In a randomized, double-blind clinical study, 81 patients were divided into three groups to receive henna formulation, sesame oil, or placebo for four weeks. The results showed that the henna formulation significantly reduced pain intensity compared to sesame oil and placebo. It also improved quality of life and functional ability. Further randomized controlled trials with longer intervention periods are recommended to confirm its efficacy.

Glucosinolates in cancer prevention and treatment: experimental and clinical evidence

Summary: Glucosinolates, found in Brassicaceae family, can be hydrolyzed into isothiocyanates which have anti-cancer effects. Glucosinolate metabolites exert their cancer-preventive activity through various mechanisms and can also be effective in cancer treatment. However, some isothiocyanates may increase the risk of tumors, thus further studies are needed to determine the appropriate dosage for different types of tumors.

Alginate/gum arabic-based biomimetic hydrogel enriched with immobilized nerve growth factor and carnosine improves diabetic wound regeneration

Summary: The study proposes a novel approach using a multifunctional hydrogel to manage diabetic foot ulcers (DFUs). The hydrogel, made from a combination of alginate and gum arabic, is enhanced with nerve growth factor (NGF) immobilized in specially designed mesoporous silica nanoparticles (MSN). The hydrogel shows sustained release of NGF and significantly improves wound healing. The hydrogel also reduces inflammation, increases angiogenesis, and improves tissue pathology. This newly developed composite system holds promise as a potential treatment for non-healing diabetic wounds.
