Natural heme oxygenase-1 inducers in hepatobiliary function

Many physiological effects of natural antioxidants, their extracts or their major active components, have been reported in recent decades. Most of these compounds are characterized by a phenolic structure, similar to that of a-tocopherol, and present antioxidant properties that have been demonstrated both in vitro and in vivo. Polyphenols may increase the capacity of endogenous antioxidant defences and modulate the cellular redox state. Changes in the cellular redox state may have wide-ranging consequences for cellular growth and differentiation. The majority of in vitro and in vivo studies conducted so far have attributed the protective effect of bioactive polyphenols to their chemical reactivity toward free radicals and their capacity to prevent the oxidation of important intracellular components. However, in recent years a possible novel aspect in the mode of action of these compounds has been suggested; that is, the ultimate stimulation of the heme oxygenase-1 (HO-1) pathway is likely to account for the established and powerful antioxidant/anti-inflammatory properties of these polyphenols. The products of the HO-catalyzed reaction, particularly carbon monoxide (CO) and biliverclin/bilirubin have been shown to exert protective effects in several organs against oxidative and other noxious stimuli. In this context, it is interesting to note that induction of HO-1 expression by means of natural compounds contributes to protection against liver damage in various experimental models. The focus of this review is on the significance of targeted induction of HO-1 as a potential therapeutic strategy to protect the liver against various stressors in several pathological conditions. Oral and Parenteral vs. Parenteral Antibiotic Prophylaxis for Patients Undergoing Laparoscopic Colorectal Resection: An Intervention Review with Meta-Analysis

Summary: This study aims to assess the efficacy of parenteral and oral antibiotic prophylaxis compared to parenteral-only prophylaxis for preventing surgical site infection in laparoscopic surgery for colorectal cancer resection. The results showed that a combination of oral and intravenous antibiotics significantly reduces the incidence of surgical site infection in comparison to intravenous antibiotics alone.

Platelets in Non-alcoholic Fatty Liver Disease

Summary: Non alcoholic steatohepatitis (NASH) is an inflammatory reaction of the liver caused by excessive accumulation of lipids in hepatocytes. It can lead to cirrhosis and hepatocellular carcinoma (HCC). Fatty liver is a manifestation of metabolic syndrome, which is associated with subclinical inflammation. Platelets are involved in immune response and activation of stellate cells, and may play a key role in the development of NASH. Antiplatelet drugs have been shown to interrupt this process and improve NASH and metabolic alterations.

Subclinical and clinical atherosclerosis in non-alcoholic fatty liver disease is associated with the presence of hypertension

Summary: The study aimed to assess the differences in atherosclerosis, arterial stiffness, and cardiac morphology among patients with isolated NAFLD, isolated hypertension (HT), or a combination of the two conditions. The results showed that overt atherosclerosis was significantly present only in NAFLD + HT patients, indicating that the impact of NAFLD on vascular structure and function could depend on the coexistence of other major cardiovascular risk factors, such as HT.

Dietary Fats and Depressive Symptoms in Italian Adults

Summary: This study investigated the relationship between the intake of different types of dietary fats and depressive symptoms in Italian individuals living in the Mediterranean area. The results showed a negative association between moderate intake of polyunsaturated fatty acids (PUFA) and depressive symptoms, while moderate intake of saturated fatty acids (SFA) was positively associated with depressive symptoms. Moreover, individuals with lower to moderate intake of short-chain and medium-chain saturated fatty acids had a lower risk of depressive symptoms. These findings highlight the importance of distinguishing between different fat types.

Spur cells in liver cirrhosis are predictive of acute-on-chronic liver failure and liver-related mortality regardless of severe anaemia

Summary: In this study, the prevalence of spur cell anemia was found to be high in patients with liver cirrhosis, and it was significantly associated with disease decompensation, acute-on-chronic liver failure, and 1-year liver-related mortality. Even though most patients with high percentage of spur cells did not have severe anemia, the presence of spur cells should be evaluated for prognostic impact and to prioritize patients for intensive management and possible liver transplantation.

Summary: This study investigated the phenolic profile and antioxidant properties of the leaf aqueous extract of wild artichokes grown in Sicily, Italy. The extract showed promising antioxidant power and was able to counteract oxidative stress caused by fatty acid exposure. It also modulated gene expression related to antioxidant defense and inflammation.

The multi-drug resistant organisms infections decrease during the antimicrobial stewardship era in cirrhotic patients: An Italian cohort study

Summary: The aim of this study was to evaluate the incidence of bacterial infections, especially multidrug-resistant organisms (MDROs), before and after the introduction of the antimicrobial stewardship program. The study found a high incidence of bacterial infections in cirrhotic patients, especially MDROs, and a strong association with liver complications. The introduction of the antimicrobial stewardship program decreased the incidence of MDROs infections.

Dietary Fats and Cognitive Status in Italian Middle-Old Adults

Summary: The increase in life expectancy has led to a higher prevalence of age-related neurological diseases, such as cognitive impairment, dementia, and Alzheimer's disease. This study aimed to examine the association between specific categories and subcategories of dietary fats and cognitive status in older Italian adults. The results showed that moderate consumption of certain types of saturated fatty acids and monounsaturated fatty acids was associated with a lower risk of cognitive impairment, while moderate intake of linoleic acid was associated with a higher risk of cognitive impairment.

Interleukin-6: A New Marker of Advanced-Sarcopenic HCC Cirrhotic Patients

This study aimed to investigate the correlation between IL-6 and HCC stage, and its potential as a diagnostic marker for sarcopenia.

SerpinB3 as hepatic marker of post-resective shear stress

Summary: Post-resective liver failure is a common complication of liver surgery caused by portal hyperperfusion and arterial vasoconstriction. Splenectomy can reduce portal flow and increase survival chances. SerpinB3 is over-expressed in the liver under oxidative stress conditions, providing cell defense and survival. This study assessed SerpinB3 expression as a predictor of liver damage in liver resection models with or without splenectomy. Results showed that major hepatic resection led to higher transaminase values and ammonium levels. Splenectomy prevented an increase in portal flow and hepatic artery resistance. Only the group without splenectomy showed higher shear-stress conditions and elevated levels of HO-1, Nox1, Serpinb3, and IL-6. In conclusion, splenectomy controls inflammation and oxidative damage, preventing the expression of SerpinB3, making it a potential marker of post-resective shear stress.

Mediterranean diet and chronotype: Data from Italian adults and systematic review of observational studies

Summary: Scientific evidence suggests a relation between dietary factors and sleep, specifically higher adherence to the Mediterranean diet is associated with better sleep quality. This study found that individuals with intermediate and evening chronotypes were less likely to adhere to the Mediterranean diet compared to morning chronotype. However, this association was weakened in older participants.

Anthocyanin Effects on Vascular and Endothelial Health: Evidence from Clinical Trials and Role of Gut Microbiota Metabolites

Summary: Hypertension and cardiovascular disease are major global causes of death, and increased oxidative stress and inflammation are associated with endothelial dysfunction. Anthocyanins, with their antioxidant and cardioprotective effects, have garnered significant interest, but their detection in blood serum is limited due to metabolism by gut microbiota. This review summarizes the main findings from clinical studies on the cardioprotective effects of anthocyanins, as well as the role of gut microbiota in their metabolism, bioavailability, and impact on microbiota composition. Further research is needed to clarify which molecules and doses of anthocyanins offer vascular benefits, while considering the crucial role of the microbiota.
