Oral mucosal manifestations of autoimmune skin diseases

A group of autoimmune diseases is characterised by autoantibodies against epithelial adhesion structures and/or tissue-tropic lymphocytes driving inflammatory processes resulting in specific pathology at the mucosal surfaces and the skin. The most frequent site of mucosal involvement in autoimmune diseases is the oral cavity. Broadly, these diseases include conditions affecting the cell-cell adhesion causing intra-epithelial blistering and those where autoantibodies or infiltration lymphocytes cause a loss of cell-matrix adhesion or interface inflammation. Clinically, patients present with blistering, erosions and ulcers that may affect the skin as well as further mucosal surfaces of the eyes, nose and genitalia. While the autoimmune disease may be suspected based on clinical manifestations, demonstration of tissue-bound and circulating autoantibodies, or lymphocytic infiltrates, by various methods including histological examination, direct and indirect immunofluorescence microscopy, immunoblotting and quantitative immunoassay is a prerequisite for definitive diagnosis. Given the frequency of oral involvement and the fact that oral mucosa is the initially affected site in many cases, the informed practitioner should be well acquainted with diagnostic and therapeutic aspects of autoimmune dermatosis with oral involvement. This paper reviews the pathogenesis and clinical presentation of these conditions in the oral cavity with a specific emphasis on their differential diagnosis and current management approaches. Lack of PRAME Expression in Cutaneous T-Cell Lymphomas

Summary: PRAME is not expressed in cutaneous T-cell lymphomas (CTCLs), suggesting that the pathogenesis of CTCL is not mediated by PRAME. Further study is required to identify biomarkers that might aid in the diagnosis and prognostication of CTCLs.

Absence of TFE3 Immunoexpression in a Spectrum of Cutaneous Mixed Tumors: A Retrospective Pilot Study

Summary: Our study failed to reveal possible TFE3 gene fusion by IHC staining in benign, atypical, and malignant chondroid syringomas. Although the negative staining in MCS suggests a genetic heterogeneity in this entity, further studies with larger case groups are needed for a more definitive conclusion.

FLI-1/Melan-A dual stain is an alternative to PRAME in differentiating metastatic melanoma from nodal nevus: A monocentric retrospective study

Summary: Melanocytic nevi in lymph nodes pose a diagnostic challenge and can be mistaken for metastases. The use of PRAME immunohistochemical stain can help differentiate between the two. FLI-1 immunohistochemical expression has shown variable sensitivity in malignant melanoma, while melanocytic nevi are reported to be negative. In this study, we examined lymph nodes with metastatic melanoma and benign deposits, and found that FLI-1/Melan-A dual staining is equally sensitive and specific as PRAME in distinguishing between the two. Larger studies are needed to confirm these significant results.

TRPS1 Is Differentially Expressed in a Variety of Malignant and Benign Cutaneous Sweat Gland Neoplasms

Two distinct pathogenic pathways of digital papillary adenocarcinoma - BRAF mutation or low-risk HPV infection

Summary: This study presents a series of eight cases of digital papillary adenocarcinoma (DPA) with two distinct clinical presentations, morphologies, immunophenotypes, and molecular features. Four cases showed painless, slow-growing nodules with glandular structures, while the other four cases showed painful, rapidly growing masses with solid, tightly packed papillary architecture.

Histopathologic Aspects of Malignancy-Associated Granuloma Annulare: A Single Institution Experience

Summary: Granuloma annulare (GA) is a benign inflammatory disease with different histologic patterns, while malignancy-associated granuloma annulare (MGA) is rarely associated with cancer. This study aimed to compare the clinical and histopathological differences between GA and MGA. It was found that MGA is more common in older individuals and affects the extremities, with chronic lymphocytic leukemia being the most common associated malignancy. The histopathological pattern of interstitial was more commonly seen in MGA, while necrobiotic pattern was more common in GA.

TRPS1: A Marker of Follicular Differentiation

Summary: This study evaluates the immunohistochemical expression of TRPS1 in cutaneous neoplasms with follicular differentiation, including trichoblastoma, trichoepithelioma, and basal cell carcinoma. The study found that TRPS1 showed intermediate-to-high positivity in trichoblastoma and trichoepithelioma, while it was negative or sporadically positive in basal cell carcinoma.

Audiovisual information of oral epithelial dysplasia: Quality, understandability and actionability

The findings show that most of the materials are scientific in nature, with poor quality and lacking essential patient information. Patient and clinician perspectives of information needs concerning oral epithelial dysplasia
