Diagnostic accuracy of transvaginal ultrasound examination for assigning a specific diagnosis to adnexal masses

Objectives To determine the sensitivity and specificity of subjective evaluation of gray-scale and Doppler ultrasound findings (here called pattern recognition) when used by experienced ultrasound examiners with regard to making a specific diagnosis of adnexal masses. Methods Within the framework of a European multi-center study, the International Ovarian Tumor Analysis study, comprising nine ultrasound centers, women with at least one adnexal mass were examined with gray-scale and color Doppler ultrasonography by experienced ultrasound examiners. A standardized examination technique, and standardized terms and definitions were used. Using pattern recognition the examiners classified each mass as benign or malignant and suggested a specific diagnosis (e.g. dermoid cyst or endometrioma). The reference standard was the histology of the surgically removed adnexal tumors. Results A total of 1066 women were included, of whom 800 bad a benign mass and 266 a malignant mass. A specific diagnosis based on ultrasound findings was suggested in 899 (84%) tumors. The specificity was high for all diagnoses (range, 94-100%). The sensitivity was highest for benign teratoma/dermoid cysts (86%, 100/116), hydrosalpinges (86%, 18/21), peritoneal pseudocysts (80%, 4/5) and endometriomas (77%, 1531199), and lowest for functional cysts (17%, 4124), paraovarian/parasalpingeal cysts (14%, 3121), benign rare tumors (11%, 119), adenofibromas (8%, 3/39), simple cysts (6%, 1/18) and struma ovarii (0%, 0/5). The positive and negative likelihood ratios of pattern recognition with regard to dermoid cysts, hydrosalpinges and endometriomas were 68.2 and 0.14, 38.9 and 0.15, and 33.3 and 0.24, respectively. Dermoid cysts, hydrosalpinges, functional cysts, paraovarian cysts, peritoneal pseudocysts, fibromas/fibrothecomas and simple cysts were never misdiagnosed as malignancies by the ultrasound examiner, whereas more than 10% of inflammatory processes, adenofibromas and rare benign tumors including struma ovarii were misdiagnosed as malignancies. Conclusions Using subjective evaluation of gray-scale and Doppler ultrasound findings it is possible to make an almost conclusive diagnosis of a dermoid cyst, endometrioma and hydrosalpinx. Summary: This study validated the use of modified benign simple descriptors (BDs) to exclude malignancy in most women with adnexal masses, and proposed a two-step strategy using the ADNEX model to estimate the risk of malignancy. The modified BDs were applicable to 37% of tumors, 99.3% of which were benign. The two-step strategy showed an AUC of 0.94.

Using IOTA terminology to evaluate fetal ovarian cysts: analysis of 51 cysts over 10-year period

Summary: This retrospective study examined 51 fetal ovarian cysts in 48 fetuses and reclassified the cysts using IOTA terminology. The results showed that anechoic cysts and cysts with low-level content often resolved spontaneously, while cysts with ground-glass, hemorrhagic, mixed, or undefined content were frequently associated with postoperative necrosis.

1 Real-world assessment of the patient- centredness of endometriosis care: European countries benchmarked by patients

Summary: European patients seek more patient-centred healthcare by crossing borders. This study validated the ENDOCARE questionnaire in nine different languages and demonstrated its benchmark potential. Significant between-country variance in case-mix-adjusted overall and dimensional patient-centredness scores explained 3-9% of the total variance in patient-centredness across 10 European countries. Compared to the least patient-centred country, endometriosis care in Denmark, Italy, and Belgium was more patient-centred.

Summary: The aim of the study was to investigate the prognostic impact of ultrasound tumor parameters and patients' anthropometric parameters on cervical cancer. The study found that low body mass index (BMI) and ultrasound tumor volume were associated with a higher risk of recurrence. Additionally, the study identified a significant relation between ultrasound characteristics and pathological parametrial infiltration. These findings suggest that these parameters can be useful in pre-operative workup for a patient-tailored treatment in early-stage cervical cancer.

Definition and Criteria for Diagnosing Cesarean Scar Disorder

Summary: This qualitative study evaluated how experts define and diagnose cesarean scar disorder. Through a modified Delphi procedure, consensus was achieved among 31 international gynecologists on the definition, symptoms, conditions to exclude, and diagnostic criteria of this disorder. Using a standardized definition for the constellation of symptoms resulting from a symptomatic uterine cesarean scar will have important implications for patient care and future niche-related research.

Ultrasound-guided endoscopy to improve accuracy of persistent urogenital sinus (PUGS) diagnosis in adult patient: A case report

Summary: This article reports a case of persistent urogenital sinus (PUGS) in an adult female associated with complex Mullerian malformation. The condition was diagnosed through the integration of gynecological ultrasound and hysteroscopy performed by expert operators. Ultrasound guided endoscopy played a crucial role in achieving a dynamic one-stop diagnosis, avoiding delays and misdiagnosis caused by the separate application of different radiological and endoscopic techniques in preoperative assessment.
