Forced diuresis with matched hydration in reducing acute kidney injury during transcatheter aortic valve implantation (Reduce-AKI): study protocol for a randomized sham-controlled trial

Background: Acute kidney injury (AKI) is observed in up to 41% of patients undergoing transcatheter aortic valve implantation (TAVI) and is associated with increased risk for mortality. The aim of the present study is to evaluate whether furosemide-induced diuresis with matched isotonic intravenous hydration using the RenalGuard system reduces AKI in patients undergoing TAVI. Methods/Design: Reduce AKI is a randomized sham-controlled study designed to examine the effect of an automated matched hydration system in the prevention of AKI in patients undergoing TAVI. Patients will be randomized in a 1: 1 fashion to the RenalGuard system (active group) versus non-matched saline infusion (sham-controlled group). Both arms receive standard overnight saline infusion and N-acetyl cysteine before the procedure. Discussion: The Reduce-AKI trial will investigate whether the use of automated forced diuresis with matched saline infusion is an effective therapeutic tool to reduce the occurrence of AKI in patients undergoing TAVI.

Summary: Central aortic blood pressure evaluation using the SphygmoCor XCEL device in patients with severe aortic stenosis undergoing transcatheter aortic valve replacement showed slightly underestimated central systolic blood pressure before the procedure, with increasing underestimation after TAVR. The agreement between non-invasive cuff-brachial blood pressure measurements and invasive central aortic pressure measurements did not meet the ARTERY Society accuracy criteria.

Summary: This study aimed to investigate the mortality risk of MDD by age-groups among inpatients with preserved EF and determine ranges of diastolic function parameters by prognosis. The results showed that MDD was associated with increased mortality risk among all age groups, but its significance decreased with age.

Summary: This study found that patients with atrial fibrillation (AF) and new-onset AF (NOAF) have higher mortality after undergoing transcatheter edge-to-edge repair (TEER) for mitral regurgitation. Within 6 months after TEER, 3.5% of patients developed NOAF. AF, fluid and electrolyte disorders, and heart failure were identified as independent predictors of NOAF. These findings are important for identifying patients at risk of developing NOAF after TEER.

Summary: This study describes a novel device that can split valve leaflets after transcatheter aortic valve replacement (TAVR) procedures to prevent coronary artery obstruction. The device was successful in bench testing and preclinical trials, and was subsequently used in 8 patients with failed bioprosthetic valves. The procedure was safe, successfully prevented coronary obstruction, and had a 100% success rate.

Summary: A total of 80% of patients undergoing transcatheter aortic valve replacement (TAVR) experienced objective functional improvement 6 months after the procedure, leading to significantly lower 2-year mortality rate. A predictive model based on clinical, anatomical, and physiological parameters was developed to assess postoperative functional recovery. The model demonstrated good discriminative performance and was validated in an independent cohort, highlighting its importance in predicting and improving the prognosis of TAVR patients.
