Evaluation of the Frails' Fall Efficacy by Comparing Treatments (EFFECT) on reducing fall and fear of fall in moderately frail older adults: study protocol for a randomised control trial

Background: Falls are common in frail older adults and often result in injuries and hospitalisation. The Nintendo (R) Wii (TM) is an easily available exercise modality in the community which has been shown to improve lower limb strength and balance. However, not much is known on the effectiveness of the Nintendo (R) Wii (TM) to improve fall efficacy and reduce falls in a moderately frail older adult. Fall efficacy is the measure of fear of falling in performing various daily activities. Fear contributes to avoidance of activities and functional decline. Methods: This randomised active-control trial is a comparison between the Nintendo WiiActive programme against standard gym-based rehabilitation of the older population. Eighty subjects aged above 60, fallers and non-fallers, will be recruited from the hospital outpatient clinic. The primary outcome measure is the Modified Falls Efficacy Scale and the secondary outcome measures are self-reported falls, quadriceps strength, walking agility, dynamic balance and quality of life assessments. Discussions: The study is the first randomised control trial using the Nintendo Wii as a rehabilitation modality investigating a change in fall efficacy and self-reported falls. Longitudinally, the study will investigate if the interventions can successfully reduce falls and analyse the cost-effectiveness of the programme.

Summary: Research on the impact of the PCMH model on older adults in Singapore found that it provides a better care experience compared to usual care. Improvements were seen in patient-provider communication, care coordination, office staff interactions, support for patients in caring for their health, and provider rating. Participants also reported positive experiences with consolidated appointments, sustained patient-provider relationships, shared decision-making, and family/caregiver engagement in the PCMH model.

Summary: This study aims to segmentize the population with high inpatient service use and examine the association of segment membership and healthcare utilization and mortality. Three distinct segments were identified based on medical complexity and psychosocial needs, and there were significant differences in hospitalization rates, program utilization, and mortality among these segments. Therefore, resources and interventions can be tailored according to the differences in needs among segments.

Summary: This study aims to uncover the factors that facilitate or inhibit implementing community-based fall prevention programs in Singapore, with a focus on the interrelationship between those factors. Health care professionals familiar with fall prevention programs were invited to discuss the enablers and challenges to the implementation. Results showed that implementation is influenced by health care provider factors and patient factors, and key leverage points for potential interventions were identified.

Summary: This article introduces the common occurrence and negative effects of falls and fall-related injuries in older adults, which impact their functional independence and quality of life, as well as increase morbidity, mortality, and health-related costs. Current guidelines are inconsistent and not globally applicable. Therefore, the objective of this study is to create evidence- and expert consensus-based falls prevention and management recommendations for older adults. These recommendations consider a person-centered approach, the perspectives of older adults, caregivers, and stakeholders, recent developments in e-health, and implementation in resource-limited locations.
